be indicated on multiple original cash register receipts from a participating Publix. Your completed request must be submitted online by 5/25/15 and mailin submissions must be postmarked no later than 5/25/15 and received at the address below by 6/8/15. Limit one (1) $10 or $20 Publix Gift Card per
household address (No P.O. Boxes, except North Dakota, only street and rural addresses are acceptable). Place in a stamped envelope and mail to: Refresh
