Completed Application Form 2. Application letter of no more than one, single-sided page (8 ½ “x 11” sheet). Take the time to proof-read your letter for spelling and grammar errors. Present yourself well and be sure to include all of these items: a. Your name and W# at the top b. Reasons you should be selected for a scholarship c. If applying for Oscar Edwin Olson or Aftermath Club scholarships, as described on the next page, include specific criteria in your application letter and click the appropriate box(es). We will not, nor is it our responsibility to, review your application materials to determine if you are applying for these scholarships. 3. Faculty Letter of Recommendation a. Letters need to be signed by the faculty member, and may be in a sealed envelope at the faculty member’s discretion. b. Please make your request early as faculty are busy c. We will not, nor is it our responsibility to, review your application materials to determine if you are applying for these scholarships. ☐ AFTERMATH CLUB SCHOLARSHIP: Check this box if you have read the criteria below and have addressed in your letter how you meet ALL of these criteria, (including your major and professional goals). 1. Must be a student in the fine and performing arts whose work shows promise of a professional career OR a student majoring in education who plans to teach one of the disciplines in fine or performing arts, 2. Have a minimum cumulative 3.0 GPA, 3. Be a resident of Whatcom County, 4. Be a graduate of a Whatcom County high school (state in your letter which school), and 5. Be a continuing or returning student at WWU □ OSCAR EDWIN OLSON ARTS SCHOLARSHIP: Check this box if you have read the criteria below and have addressed in your letter how you meet ALL of these criteria. 1. Recipients must be declared WWU College of Fine and Performing Arts majors (talk about your major in your letter), 2. Students must be children of single, divorced, or widowed parents. Eligible students include those who at any time in their first eighteen years (1) lived with one unmarried parent, or (2) had divorced parents, or (3) had one or more deceased parents. (Parent is defined as adoptive parent as well as biological parent.) Briefly describe your situation in your application letter. 3. Students must be eligible for at least one dollar of financial aid (FAFSA must be on file at the time of this application!) □ Check this box to verify you have a completed FAFSA in the Financial Aid Office for 2015-2016. You must have a FAFSA on file to be eligible for consideration for the Oscar Edwin Olson Arts Scholarship. IMPORTANT INFORMATION Scholarship notification will be by your official WWU student email If you are awarded a scholarship, PLEASE NOTE: Disbursement of scholarship award is contingent upon receipt of no more than one, single-sided page (8 ½ “x 11” sheet), original, hand-signed thank you letter to the scholarship DONOR. More information on thank you letters will be provided with the award notification. NOTE: Failure to provide an original, hand-signed, single-sided, one-page (8 ½ “x 11” sheet), thank you letter by the 4:30 PM, May 22, 2015 deadline will result in revocation of the award. NO EXCEPTIONS. NO EXCUSES. Scholarships are disbursed in equal amounts, quarterly (Fall, Winter, Spring). You must notify the Dean’s office in writing (email is ok) if you will not be attending all 3 terms. The notification must be received prior to 4:30 PM, May 22, 2015 to prevent loss of all or part of your scholarship award.
