2.Signed Statement of Agreement
3.Proof of age (photocopy of birth certificate)
4.Two different color photographs (no smaller than 5"x7", minimum 1200 x 1650
pixels at 300 dpi, by e-mail, disc, or hard copies)
5.One page biographical profile in narrative form
6.Non-refundable application fee of $15 (personal check payable to
Milwaukee Symphony Orchestra)
7.CD recording of a performance of the first movement of (1) a selected concerto
with a second pianist performing the orchestra parts and (2) selected solo of
the contestant’s choice.
