2013 Annual Enrollment How to Earn Your Health Incentives As part of Healthy Together, Xerox’s wellness initiative, you can reduce your costs for 2013 medical coverage for taking positive actions toward healthier living You can earn incentives when you complete a wellness screening. Note: the deadline to complete wellness screenings has been extended to December 10, 2012. The wellness screening has two parts: 1. A confidential online health questionnaire, and 2. A free lab test (blood draw) through three convenient options: On-site, at several Xerox locations At one of over 2,000 Quest Diagnostic® Patient Service Centers across the U.S. From your primary care doctor as part of your annual preventive care visit (your doctor can complete a Physician Results Form with your screening results) You and your covered spouse/domestic partner must complete both the online health questionnaire and the lab test by the newly extended December 10 deadline to earn the health incentive for 2013. The wellness screenings also will determine your eligibility for an additional tobacco-free incentive. Your spouse or domestic partner must be covered by a Xerox Services medical plan to be eligible. Note that you may also be eligible to receive the tobacco-free incentive if your physician certifies that it is inadvisable or unreasonably difficult for you to quit tobacco at this time due to a medical condition. See the instructions on page 5 for registering for your wellness screening and completing the online health questionnaire. Health Incentives for 2013 You can earn up to $600 toward your 2013 medical coverage costs or up to $1,200 if you’ll be covering your spouse/domestic partner for medical benefits in 2013. Here’s how. However, you can still earn incentives. See below for more information. Separate incentive information will be available for union employees prior to their Annual Enrollment period. New Hire Incentives* New hires (except the groups noted below) starting September 15, 2012 or later: When you complete two online health education courses, you’ll be eligible for a Health Incentive of $5 per paycheck toward your 2012 Company-sponsored medical plan contributions and $12.50 per paycheck toward your 2013 Company-sponsored medical plan contributions. And, if your spouse/domestic partner is covered by a Xerox Services-sponsored medical plan and completes two courses, you’ll receive an additional $12.50 per paycheck toward your 2013 Company-sponsored medical plan contributions. Courses must be completed by the end of your 30-day election period. You can also earn an additional $4 per paycheck toward your 2012 Company-sponsored medical plan contributions and $12.50 per paycheck toward your 2013 Company-sponsored medical plan contributions by attesting to be tobacco-free by when you enroll for benefits. And, if your spouse/domestic partner is covered by a Xerox Services-sponsored medical plan and you attest that he or she is tobacco-free, you’ll receive an additional $12.50 per paycheck toward your 2013 Companysponsored medical plan contributions. Courses must be completed by December 10, 2012 or by the end of your 30-day election period. You can also earn an additional $6.25 per paycheck toward your 2013 Company-sponsored medical plan contributions by attesting to be tobacco-free by when you enroll for benefits. And, if your spouse/domestic partner is covered by a Xerox Services-sponsored medical plan and you attest that he or she is tobacco-free, you’ll receive an additional $6.25 per paycheck toward your 2013 Companysponsored medical plan contributions. Limited medical plan participants hired prior to 2013 who will also be eligible for the Consumer Choice Plan in 2013: You may participate in the wellness screening. Your incentives will be based on the plan that you elect for 2013: If you enroll in the Consumer Choice Plan, your incentives will be based on completion of the wellness screening. (See page 1.) If you elect a limited medical plan, you and your spouse/domestic partner may complete two online health education courses to earn incentives of $6.25 per paycheck each toward your 2013 Company-sponsored medical plan contributions. (See page 5 for instructions on how to take the courses.) You can also earn an additional $6.25 per paycheck toward your 2013 Companysponsored medical plan contributions by attesting to be tobacco-free by when you enroll for benefits. And, if your spouse/domestic partner is covered by a Xerox Services-sponsored medical plan and you attest that he or she is tobacco-free, you’ll receive an additional $6.25 per paycheck toward your 2013 Company-sponsored medical plan contributions. Activity Health Incentive (toward your Company-sponsored medical plan contributions) Employees enrolled in a Limited Medical Plan for 2013 Note: If you are eligible for both a Limited Plan and the CCP and choose to enroll in the CCP, you will be eligible for the incentive opportunities described under Health Incentives for 2013 on page 1. Courses must be completed by December 10, 2012 or by the end of your 30-day election period. 3 Union Employees Incentives Somerset union employees and Staten Island union employees will receive separate incentive information prior to their Annual Enrollment period. Attention Residents of Hawaii Medical premium cost incentives are not available for residents of Hawaii. You are welcome to complete the wellness screening, but premiums are already lowered by law, so completing the screening will not affect your costs. 4 Registering for Your Wellness Screening Go to https://my.blueprintforwellness.com/, or call 1.866.908.9440. When prompted for your registration key, enter “Xerox.” You’ll then be asked to enter your “Employee ID,” which is your 8-digit WIN ID. Once you sign in using your Employee ID, you’ll verify some information. Your spouse/domestic partner can register by using your Employee ID, with “S” added to the end. For example, if your ID is 12345678, your spouse/domestic partner would use 12345678s. (The “s” is not case sensitive.) Note: Your spouse/domestic partner must be enrolled in medical coverage before he/she will be able to register for a wellness screening. If he or she isn’t currently enrolled, you’ll need to enroll him/her when BenefitsWeb becomes available on October 29. Then return to https://my.blueprintforwellness.com/ in seven to 10 days to see if he or she has been added as eligible for the screening. Attention Residents of Puerto Rico and Northern California: Because the Quest Diagnostics Patient Service Centers in your area are serviced by a subsidiary of Quest Diagnostics, you have a different process for the lab test. First, visit https://my.blueprintforwellness.com/ and register. As part of the registration process you’ll be asked to make an appointment. The appointment date must be at least 10 days after the date you register. Approximately seven to 10 days after you register, you will receive a letter and a paper requisition form. Take the paper requisition form with you when you go to the Quest Diagnostics Patient Service Center for your lab test. Taking the Online Health Education Courses To encourage employees to become informed consumers of health care, Xerox offers online health education courses through Asset Health, a leader in health care education. You can access the courses at www.assethealth.com/HealtheLearning. Employees and spouses/domestic partners may take any of the online health education courses at any time, but only new hires starting on September 15, 2012 or later and certain other groups, are eligible to earn incentives for completing the courses. Login to the Asset Health website using your username and password. If your Xerox badge number begins with US, followed by a series of 6 numbers or letters, your username is your employee number (without the ‘US’ prefix) and your password is your five-digit home ZIP code. If you use a WIN ID, your username is your WIN ID and your password is your five-digit home ZIP code. Your spouse can log in using your username and password. From the welcome page click on “Your Settings” in the upper right hand corner. Then click on “Dependent Registration” on the left hand side. From here, complete the required information to create a new username and password and hit “Submit” on the lower right hand corner. Logout and return to the site using the new login information.
