LexisNexis Claims MVR (Driving History) is an automated method of verifying a person's driving history. The Claims MVR contains your
order information, individual driver information, and driving history.
Sections in the MVR Report

Claim Number/Quoteback
(1) Claim Number - Insurance company claim number that was entered on the request form.
(2) Quoteback Account number and suffix of adjuster ordering the report, followed by the order ID.
Driver Information
This section lists personal information about the driver from your order, and from the DMV.
Driver's License Information
This section reports additional information about the license from the state DMV. If the license has more than 3 restrictions, the
additional restrictions will be shown in the Miscellaneous and State Specific Information section.
Miscellaneous and State Specific Information
This section includes information provided by the state or other information supplied by LexisNexis.
State Information
States may send the following types of information in this section:

State disclaimers

(4) Vio/Sus Date: Date the violation or suspension occurred.
(5) Conv/Rein Date: Date of the conviction or license reinstatement. "CPS/SVC" indicates that LexisNexis has provided the
Standard Violation Code and description.
(6) Description: State DMV description and, when provided, LexisNexis Standard Violation Code (SVC) description. If "CDL: C"
appears in this area, it indicates that this is a commercial violation. This information is supplied by the state. LexisNexis provides the
"CDL:" indicator to note the commercial incident.
(7) Vio/Conv Code: State DMV violation/conviction code.
(8) Pts: Total points assigned by the DMV to the driver. Points are not present for every record and state.

LexisNexis may send messages regarding the following:

Restricted information
Examples of LexisNexis messages:

"***Restricted Information"
This message appears if the driver has restricted personal identification information from being released on his or her MVR.
Error messages:
If your order encounters an error during processing, an error message will appear on your report indicating the type of problem encountered.
In such cases, you will not receive MVR results. If the message begins with "APP ERROR:" or "COMM ERROR:", it indicates that a
communications problem occurred before your order could be processed by the MVR application. You should try sending your order again at
a later time. If you receive any other error messages, please contact LexisNexis Customer Support at 1-800-934-9698 or via e-mail
