How to send an Electronic Information Prescription on NaSH
1. CAN BE USED FROM ANY SANDYFORD SITE! GREAT FOR THE HUBS 
2. Once in patient details select ‘Special Forms’ from left hand column
3. Select ‘show/hide tree’ button
4. Click cross next to ‘NaSH’ to expand list
5. Select ‘Referral’
6. To create new referral click ‘New’ button
7. Enter date of referral (should automatically show today’s date)
8. In ‘Referred By’ section:

Enter your name in ‘Clinician’

From ‘Referral Source’ dropdown list select ‘Internal Referral’
9. In ‘Referred To’ section:
-
From ‘Clinic’ list select ‘Sandyford Central’

From ‘Service Ref’d To’ dropdown select ‘Information Services’
10. In ‘Clinical Details on Referral’ section:
-
Enter information required by patient in ‘Presenting Complaints’ box
-
Ideal contact with patients is by e-mail – if permission given please ensure e-mail
address obtained and enter ‘E-mail OK’ in ‘Addl Information’ box
11. Once all fields completed, click ‘save and sign off’ button
 What happens after?

Library staff will confirm receipt of EIP by email to clinician recorded in referral

‘Date Received by Clinic’ will be recorded and viewable on referral page

In ‘Referral Actions’ section:

‘Referral Notes’ will read either: Information posted/Information emailed/No
information found/No perms

In the case of ‘No information found’ or ‘No perms’ the referring clinician will be
contacted to advise
