How to fill in your Application for the Home-Link Register How to fill in your Application for the Home-Link Register We want to get a clear picture of the way you are living to register your needs for re-housing. The following tips will help you fill in the form and make sure you have not forgotten to tell us something important that may affect the priority we give you. All the information you provide is treated confidentially. 1. Please make sure that the answers are true and accurate, written in BLUE or BLACK ink. 2. We will only consider your application if all the questions are answered fully. If any questions do not apply to you write N/A in the relevant boxes. 3. You will need to answer some of the questions by TICKING a box or where we have asked for more detail you will need to write information in the white box. If you need more space please continue on a separate sheet of paper making sure that you put your name on all additional paper used. 4. Please make sure that all of the requested proofs have been provided. Applications will only be registered once all the supporting documents have been received. 5. If you are completing a Change of Address form and this is the only change please make sure you provide proof of your new address for each adult on the application. The Council will require photocopies of proof of identification, address and income for each adult included on your application. You only need to provide one form from each section. Acceptable forms are listed below: Proof of ID • Passport PLEASE DO NOT SEND ORIGINALS • Birth Certificate • Medical Card • Driving Licence • Benefit Award Letters (including child benefit, which would provide ID for the parent and the children) • Marriage Certificate Proof of Address • Bank or Credit Card Statements • Wage Slips (showing your name and address) PLEASE DO NOT SEND ORIGINALS • Utility Bills (eg. Gas & Electric) • Council Tax Bill Proof of Income • Wage Slips (the last two most recent) • Benefits Award Letters • Bank Statement (showing income) • Proof of Accounts (self-employed only) PLEASE DO NOT SEND ORIGINALS Children For children that are included on your housing application and are permanent members of your household we will require a copy of their birth certificate and confirmation of child benefit. If you have access to children and have included them on your application you will need to provide a copy of your Court Order or a letter confirming the dates and times you have the children along with a copy of their birth certificate. Please note that the Council may require additional documents in support of your housing application. These will be listed next to the relevant question number below. You may bring original documents to the Customer Service Centre in Huntingdon, St Ives, St Neots, Ramsey or Yaxley and a member of staff will be happy to photocopy them for you. Q1 Please tick a box that best shows the reason you are applying for housing. Q2 If no, then you do not qualify for housing. Q3 Please tick as requested. Q4 If yes, you must provide both pages of your accession state worker registration scheme cards or residence permit. Q5 If yes, you must provide a copy of your visa or Home Office documents. Q6 Please give the address where you live all or most of the time. If you have no fixed address, please write NFA and provide a correspondence address in question 10. No fixed address means sleeping rough or between several addresses in the past 6 months. Q7 Please tick as requested. Q8 Please give the address where your partner/joint applicant lives all or most of the time. If they have no fixed address, please write NFA. Q9 Please complete as requested. Q10 If you have stated NFA to Q6 please provide a correspondence address. Even if you have a permanent address but would like your mail to be sent to a different one please give the details of that address here. Q11 Enter all the details of the people, other then you and your partner or joint applicant, that are to be re-housed with you. You should not include anyone who lives with you at the moment but will not be moving with you. Q12 Please tick as requested. Q13 Please provide days and times of when your children stay. Q14 Please complete as requested and you will need to provide proof of your pregnancy. This can be either the pages of your maternity notes that show your name and expected delivery date or other official proof. Q15 If you tick yes a medical form will be sent to you. Q16 Please tick as requested. Q17 Please tick if appropriate. Q18 Please complete if used. Q19 You can tick as many boxes as appropriate. Q20 Please tick only ONE. Q21 Please complete all fields. Q22 Please complete all fields. If you are a home owner you will need to provide two up to date valuations and an up to date mortgage statement. Q23 Please tick as requested. Q24 Please tick as requested. Q25 Please tick as requested. Q26 If you tick ‘yes’ to this question please provide as much information as possible. Q27 We need to know the sizes of all the rooms used, and who is using them, for sleeping so that we can tell if you are overcrowded. Q28 ‘Sole use’ means that you and the people to be re-housed with you use the facilities within the house and you do not have to share with anyone outside your household. ‘Shared’ means that you and your household have to share the facilities with other people who are not to be housed with you. This might be: • Parents, friends or relatives who are living with you • Another family you are living with; or • The other residents in a shared home, hostel or refuge. ‘Do not have’ means that you and your household do not have the use of that facility. This might be if you are sleeping rough or living in a shared home without cooking facilities. Q29 In certain cases the Council may be able to make you an interest free loan or grant to help you repair your property. Generally if you are a tenant the responsibility for repairing your property will be with your landlord. Q30 It is very important that you fill this section in for the last 5 years. Please make sure you give the full address along with the dates and type of tenancy or write ‘living with parents’ etc. Q31 As Q30 for your partner or joint applicant. Q32 Please give full address and the name of the landlord. Q33 Please complete as requested. Q34 Please complete as requested. Q35 Please complete as requested. If you or your partner are owners you will need to provide two valuations and an up to date mortgage statement. If you have sold the property you will need to provide a copy of your completion statement. Q36 If you have a local connection to any of the Councils who are part of the Home-Link scheme please give the details. Please see the HomeLink form for definitions of a local connection. Councils within the Home-Link Scheme: • Huntingdonshire District Council • Cambridge City Council • East Cambridgeshire District Council • Forest Heath District Council • South Cambridgeshire District Council • St Edmundsbury Borough Council • Fenland District Council Q37 If you have a local connection to any village/town within Huntingdonshire please give the details as requested. For definitions of a rural connection please see the Home-Link form. Q38 Please write in the names of the places you would consider moving to. Alternatively you can complete the enclosed choice of area form. When choosing your areas you need to bear in mind whether there is suitable public transport or any facilities, eg Doctor, Post Office. Most properties will be advertised through Home-Link and you will be able to choose which properties you bid for. However, in certain circumstances some properties may be let directly to applicants. In order for us to identify a suitable applicant we will look at which areas they have chosen to live in. Q39 Please tick as requested. Q49 In is important that you read, sign and date the Declaration. If you are applying with a partner you will BOTH need to sign and date the form. By signing the application you are giving your permission for us to make enquiries and gather information relevant to your application. If you would like another person to make enquiries on your behalf regarding your application, please provide us with a name and their relationship to you on the back page of the application form. We will only disclose the information to this person if they can provide us with your Application Reference Number.
