Overview
The Texas Department of
Aging and Disability Services’
Quality Reporting System (QRS)
website has information about
nursing homes.
Each home is given a quality rating
based on the results of its most
recent licensing inspection,
complaint investigations, and
resident assessment data. QRS
provides information about specific
deficiencies and quality comparisons
among nursing homes.
How to use
the Quality
Reporting
System (QRS)
Consider using QRS as a tool as you
work with your family, medical team,
advocates, and others to select a
nursing home.

Helping Texans find
and compare long-term
service providers
Another resource when selecting a
nursing home is the Long-Term Care
Ombudsman Program. A long-term
care ombudsman can provide a
list of nursing homes, guidance on
visiting and selecting a home, and
questions to ask providers.

Enter or select your search parameter and click
the “Go” button. (NOTE: Using ZIP codes
or cities in your search will return a smaller
subset of providers.)

Click on the type of nursing home and/or type
of funding that you are interested in (Figure 2).

Long Term Care Quality Reporting System
Click on one of the first four links to search by
county, city, ZIP or area code, or click on the last
link see all the cities that have such services.
Click on a nursing home name to see detailed
information about that home (Figure 3).
On the home’s detailed page
• click on the definition links to learn more
about each category and what the scores mean.
