PRIZE CLAIM FORM
Claiming first division lottery prizes
CONGRATULATIONS
ON BEING A WINNER!
This form is required by all winners claiming a
prize that cannot be paid at a Tatts Outlet.
Prizes less than $1,500 may be claimed at any
Tatts Outlet. At their discretion, some Tatts
Outlets may pay prizes up to $3,999.99.stlets
Prizes less than $200 may be claimed at Tatts
Quick Outlets (7-Eleven) in Victoria.
To claim your lottery prize,
complete the following steps:
1. Complete the Prize Claim Form
FF Section 1: Fill in your personal details.
FF Section 2: Nominate your preferred method of
prize payment.
FF Section 3: Sign the Claimant Declaration.
FF Section 4: Provide details of lottery ticket/s.
FF Take a photocopy of the Prize Claim Form for
your records.
2. Sign ORIGINAL winning lottery ticket/s
FF Write your name, address and signature on the
back of the ORIGINAL winning lottery ticket/s.
FF Take a photocopy of your winning ticket/s for
your records.
3. Submit Prize Claim Form in person or
by post.
FF In person – Visit Level 1, 411 Collins Street,
Melbourne, VIC 3000, during office hours.
FF By post – Tatts recommends using Registered
Post for added security. Mail Prize Claim Form
and original winning ticket/s to:

First division prizes are available for payment from
the first business day two weeks following the draw.
Claiming other division lottery prizes
Lower division prizes are available the first business
day following the draw, with the exception of The
Pools where prizes are available the Tuesday after
the draw.
Claiming prizes more than six months after the
draw date
Please refer to the state in which you purchased
your ticket.
