How to file a medical claim
Starmark no longer provides a printed medical
claim form, since the majority of claims are
filed electronically.
However if you ever need to file submit a
claim directly, please follow these steps:
1. Copy the medical billing statement you
received from your doctor that shows receipt of
payment for services provided.
2. Mail the copy to Starmark in the pink claims
envelope provided in your Administration kit, or mail
your copy to:
Starmark,
P.O. Box 2942,
Clinton, IA 52733-2942.
3. Include both your Starmark group number
(XX12345X) and your SSN or Unique Member
Identifier (UMI).

Claims submitted in this manner are routed first for
re-pricing and to apply any discounts you may be
entitled to. Starmark will then send you an
Explanation of Benefits (EOB) along with a
reimbursement check, just as we would with a claim
submitted electronically.
