What is a Personal Health Assessment
(PHA)?
A Personal Health Assessment (PHA) is an
online health questionnaire. It takes 10-20
minutes to complete and includes questions
about your physical health, stress levels,
job satisfaction, daily habits, and general
feelings about your health. The questions
are designed to help you understand your
overall health status and areas where you
may need some improvement.
What will I receive for completing the PHA?
By completing the PHA, you will reduce your 2013 medical plan payroll contribution
(up to $260 a year if you participate; $520 a year if you and your covered spouse/
domestic partner participate). In addition, you may reduce your future health care
costs by taking steps to improve your health now and avoid future risks. If you have
opted out of our health plans, you will receive a $100 Synopsys-sponsored gift card
if you complete the PHA by the deadline (June 30, 2012).

The Synopsys PHA is offered through
our wellness vendor, Healthyroads Inc.,
an Internet-based, personal health
improvement program. It is designed to
provide individuals with the online tools to
take charge of their personal health.

Is there a deadline for completing the
PHA?
The deadline is June 30, 2012, 11:59 p.m.

Why is Synopsys asking me to take the
PHA?
Synopsys is offering the PHA as a
completely voluntary resource to help you
take charge of, and improve, your health.
In addition to learning about your current
health, you will learn what you can do to
reduce your risk for serious health issues
in the future. The general results of the
PHA will also enable Synopsys to gather
data and information to help us identify
our population’s health risks. Knowing our
population’s risks will allow us to implement
appropriate wellness programs and provide
essential tools and resources.
Can my spouse/domestic partner
take the PHA?
If your spouse/domestic partner is
covered by a Synopsys medical plan,
he or she is eligible (and encouraged)
to take the PHA to learn about his/
her own health status. Like you, your
spouse/domestic partner will receive
his/her own personalized Healthy Living
Score and Personal Health Plan and
have access to the same programs
and resources for improving health.
If your spouse/domestic partner
completes the PHA by the deadline,
you will receive a payroll contribution
credit in 2013 of $10 per pay period.
If both of you complete the PHA, you
will receive a contribution credit of $20
per pay period.
Can my children take the PHA?
No, dependent children who you cover
on your Synopsys medical plan are not
eligible to take the PHA.
I’ve opted out of Synopsys medical
plans. Can I take the PHA?
Yes, if you have opted out of our health
plan you (but not your spouse) are
eligible to receive a $100 Synopsyssponsored gift card.

How does this help Synopsys when
employees or spouses/domestic
partners take the PHA?
Synopsys is self-insured. The
assessment helps you and Synopsys
save money over the long-term when
you and your family make changes
that help improve your health or catch
issues early before they become
bigger problems. For example, what
if you learned from the PHA that you
have high blood pressure or high
cholesterol? You could take steps to
control those risk factors before they
become more serious conditions like
heart disease.
What happens if I choose not to take
the PHA?
The PHA is completely voluntary; if
you choose not to take the PHA, your
medical coverage will not be affected
in any way, but you will not receive a
credit on the payroll contributions you
will pay in 2013. Synopsys will know
only whether or not you took the PHA;
the Company will not see your personal
health information.
What information do I need to
complete the PHA?
The PHA will ask about you, including
your health history and health habits.
Answering these questions will not
require any advanced preparation. The
PHA will ask you to indicate your vital
health statistics such as your weight,
height, blood pressure, cholesterol,
and blood sugar. You can get your
vital health statistics by visiting your
doctor for your annual preventive
care checkup. All of Synopsys’ health
plans cover preventive care checkups 100%. If you do not have this
information available, you can estimate
and still receive meaningful results.
Why does the PHA ask personal,
non-health questions, such as my
ethnicity and marital status?
Personal factors like ethnicity and
marital status play a part in your total
health picture, and that information
helps in assessing and predicting
your future health risk. In addition,
some ethnic groups are more at risk
for certain health conditions, so it’s
important to include that information.
How will Synopsys use my personal
health information?
None of your personal health
information will be shared with
Synopsys. Healthyroads will provide
Synopsys with information involving
your participation in our programs so
that Synopsys can provide you with a
payroll contribution credit on your 2013
contributions for benefits or provide
you with a gift card.

Will my information be used
to limit my medical coverage
because of pre-existing
conditions?
None of the health information
you provide will affect your
current or future medical
coverage at Synopsys. The
intent of the PHA is to provide
you with a helpful overview of
your health and give you the
tools to understand what steps
you can take to reduce your
future health risks.

Talk to your child about what
healthy snacks he or she
would like to eat. Make a list
of these snacks. Then make
sure to have these snacks
prepared and ready for your
child to grab.
