How to schedule your Personal Health Assessment Screening and Report Delivery appointments for School District of New London *New for 2014 – Report Delivery sessions with Allison Beck, ThedaCare Health Coach, to go over PHA results are available in either one-on-one sessions, or group sessions. One-on-one sessions are required for employees who are first time PHA participants. Spousal report delivery is optional. Those who are repeating the PHA may choose one-onone or group report delivery session. Group sessions will be more generalized in scope and the focus will be learning healthy habits to improve or maintain your PHA score (no individual health information will be shared during group sessions). Please specify if you need a one-on-one, or group, report delivery session when calling to make your PHA appointments. 1. Press “3” to schedule your PHA Screening appointments 3. For confidentiality purposes, and to verify your identity, you will be asked to provide, or verify, your demographic information. 4. Let the scheduler know you are with the School District of New London and which NLSD site you wish to have your PHA Screening and Report Delivery appointments done (see below for available dates/locations). The scheduler will help you find available time slots for these two appointments. 5. Please write down your appointment information below. 6. The questionnaire needs to be completed for you to be able to see your results/score. Log into your Millennium homepage at www.networkhealth.com Sign in to ‘My Account’ Click on “Take your ThedaCare PHA” (found under Activities/Points section on bottom of Millennium homepage). 4. Click on “Start Your Online Questionnaire HERE” (to complete your online questionnaire), located in the yellow box. 5. Enter your unique Access Code: hnYJDDXr (Hint: The Access Code is case sensitive). 6. Enter the required Account Information. Click “Proceed to Registration” (Hint: Use your “Legal” First and Last name.) 7. See box below. First Time User Repeat User Hint: Your Email (if previously entered) and Login will Enter Email Address and Create Login and auto-populate. Password. 1. 2. 3. (Hint: Please write down for future use.) Enter New Password and confirm password 8. Click “Sign up”. 9. Choose the most current Lab Date (even if it differs from your lab draw date) and click “Go”. 10. Click on “Begin your Questionnaire”. (Hint: You must answer ALL of the questions before submitting your questionnaire.) 11. After submitting your Questionnaire, on the main menu click on the “Printable Report” button to view your Personal Health Assessment Report online. * If you experience any difficulties with submitting your questionnaire, use the “Contact Us” button (located on the top right toolbar of the website) and you will be contacted by a representative within 24 hours to help resolve your issue.
