I. INTRODUCTION
1)
Important Notice
Beginning with the 2014 Examination cycle, CITA will be administering the ADEX written and
clinical licensure examinations. This manual will outline the process by which candidate will
register and participate in that process.
Every effort has been made to ensure that this manual is accurate, comprehensive, clear and
up-to-date. In the rare instances when examination related instructions need to be updated or
clarified during the examination year those changes will be posted on CITA’s website
(http://www.citaexam.com) under “Examination Manuals”. There will also be other test related
material sent. These materials will be available thru your online candidate profile as well as the
day of the exam.
All candidates who take the CITA administered ADEX examination are responsible for reading
and understanding the CITA examination manual(s), any website documented changes to the
published CITA manual(s) and for reviewing and understanding all other material provided by
CITA. If, in reviewing any CITA provided material, questions arise, it is the candidate’s
responsibility to resolve those questions by directing them to the CITA office at the earliest
possible date. Questions should be sent in writing. Conversations via the telephone will not
supersede materials published in the manual, website or candidate presentations. For
candidate’s convenience, CITA’s address, e-mail address, telephone number, website and
facsimile number appear below.
Prior to taking an examination with CITA, each candidate will be required to sign forms certifying
that he/she has reviewed the CITA manual, viewed the on-line candidate tutorials, read other
material provided by CITA and that he/she has addressed and resolved all questions prior to the
date of the examination.
During the online registration process candidates will be required to create their unique profile
which will contain all relevant contact information. It is extremely important that candidates
maintain a current email and physical mailing address with CITA. This is the only way to ensure
that there will be a timely receipt of important materials from CITA.

Purpose
This manual has been designed to assist candidates in preparing to take the ADEX clinical
manikin and patient-based licensure examinations. The examination is based on specific
performance criteria as developed by ADEX which will be used to measure the candidate’s
clinical competency.
As stated in the “Important Notice” page at the beginning of this manual, every effort has been
made to ensure that this manual is accurate, comprehensive, clear and up-to-date. However,
there may be instances when examination related instructions must be updated or clarified
during the examination year. When this situation occurs, those changes will be posted on
CITA’s website (http://www.citaexam.com) under “Examination Manuals”.
All candidates who take the CITA administered ADEX examination are responsible for reading
and understanding the CITA examination manual(s), any website documented changes to the
published CITA manual(s), viewing the on-line candidate tutorials and for reviewing and
understanding all other material provided by CITA. Candidates are also responsible for
addressing any questions that they have regarding such material by directing those questions to
the CITA office.

Failure of a candidate to review and master the guidelines provided by CITA, to the point that
such failure has significant adverse impact upon that candidate’s ability to efficiently and
effectively take the ADEX examination, may result in dismissal from and subsequent failure of
the examination.

About CITA
The Council of Interstate Testing Agencies, Inc. (CITA) is a non-profit
corporation which serves the community as an independent regional
testing agency. Beginning in the 2014 examination cycle, CITA will be
administering the ADEX examination. Regional testing agencies
contract with individual boards of dentistry to administer the clinical
examination required for licensure in those states/jurisdictions.
Regional testing agencies DO NOT have the authority to license
individuals or implement policy that goes beyond the laws of its
member states/jurisdictions. Furthermore, regional testing agencies should not be confused with
state boards of dentistry. Therefore, it is the obligation of the candidate to ascertain the
necessary qualifications and procedures necessary to obtain licensure in the intended
jurisdiction of practice, prior to the candidate undertaking any activity or activities which may
constitute the practice of dentistry.

CITA Mission Statement
“CITA’s mission is to provide psychometric, technical and administrative services in the
administration and delivery of clinical licensure examinations in dentistry and dental hygiene.
CITA will demonstrate integrity and fairness as it provides assistance to state boards of dentistry
in their mission to protect the health, safety and welfare of the public by assuring that only
competent and qualified individuals are allowed to practice dentistry and dental hygiene.”

About ADEX
The American Board of Dental Examiners, Inc. (ADEX) is a private not-for-profit consortium of
state and regional dental boards throughout the United States and its territories. ADEX provides
for the ongoing development of a series of common, national dental licensing examinations that
are uniformly administered by individual state or regional testing agencies on behalf of their
participating and recognizing licensing jurisdictions. CITA, NERB and SRTA are members of
ADEX and have adopted the ADEX Dental Examination Series.

ADEX Mission Statement
“To provide the dental community with test construction and administrative standardization for
national uniform dental and dental hygiene clinical licensure examinations. The schedule of
these examinations, when delivered in the Curriculum Integrated Format (CIF), allows for early
identification of deficiencies or weaknesses within clinical skill sets and provides opportunities
for remediation in an educational environment. These examinations will demonstrate integrity
and fairness in order to assist state boards of dentistry with their mission to protect the health,
safety and welfare of the public by assuring that only competent and qualified individuals are
allowed to practice dentistry and dental hygiene.”

ADEX Examination Series
The ADEX Dental Examination Series is the examination approved by the American Board of
Dental Examiners, Inc. (ADEX) and administered by the Council of Interstate Testing Agencies,
Inc. (CITA), the North East Regional Board of Dental Examiners, Inc. (NERB) and the Southern
Regional Testing Agency, Inc. (SRTA). The ADEX Examination Series consists of computer
simulations and clinical examinations performed on patients and manikins. The ADEX
Examination Series is utilized to assist licensing jurisdictions in making decisions concerning the
licensure of dentists. The ADEX Dental Examination Series for 2014 consists of up to five
individual, skill-specific clinical and simulated clinical examinations:
One written computer examination
-Section I: Diagnostic Skills Examination (DSE) Section (computer-based)
Two simulated clinical examinations
-Section II: Endodontic Clinical Examination Section (manikin-based)
-Section III: Fixed Prosthodontic Clinical Examination Section (manikin-based)
Two clinical examinations performed on patients
-Section IV: Periodontal/Scaling Clinical Examination Section (optional, based on the
requirements in the state where the candidate seeks licensure)
-Section V: Restorative Clinical Examination Section (includes anterior and posterior)

Candidates taking this examination do so voluntarily and agree to accept the provisions and to
follow the rules established by ADEX, CITA, NERB and SRTA for the examination as detailed in
this manual.

ADEX States and Recognizing Jurisdictions
Currently 45 states and territories (jurisdictions) accept the ADEX Dental Examination Series for
licensing determinations. Please consult the CITA website at www.citaexam.com for a current
listing of those states and jurisdictions which accept the results of the CITA administered ADEX
examination. Because of the rapidly changing nature of the licensure process in the United
States, candidates are advised to contact the board of dentistry in the state in which they seek
licensure to determine whether ADEX Exam scores are acceptable in that state.
Please note that depending on the requirements of each state, some states may require an
additional module(s) or competency. Candidates should contact the state in which they wish to
practice to confirm all requirements for licensure in that state, prior to beginning any activity or
activities which may constitute the practice of dentistry. For purposes of this discussion, the
word “state” encompasses all licensing jurisdictions which are not considered as states, i.e.,
territories and the District of Columbia. A map of those jurisdictions shall be included in the next
section.

ADEX Status
“ADEX Status” is achieved when a candidate has successfully complied with all established
rules and completed the required Sections I, II, III and V of the ADEX Dental Examination
Series with a score of 75 or more in each of the sections. While the Periodontal/Scaling Clinical
Examination (Section IV) is not required for ADEX Status, it may be required for licensure in
many jurisdictions.
It is the candidate’s responsibility to contact the licensing jurisdiction of interest to
determine current eligibility and additional requirements. Individual jurisdictions may
require an additional state jurisprudence or other additional examinations.
12)
Obtaining Licensure
Typically, applicants must complete three steps in order to obtain a dental license.
1. The candidate must take and successfully complete Parts I and
II of the National Board Dental Examinations, typically offered
during dental school.
2. The candidate must take and pass the appropriate state or
regional clinical examination. CITA, NERB and SRTA are three
of these regional testing agencies. The school where the clinical
examination takes place may have forms that need to be
completed and may require a separate fee for the use of its facilities and/or equipment
during the examination. Proof of passing the National Board examinations prior to taking
the ADEX Examination Series is not required.
3. The candidate must apply for licensure by the state board of dental examiners. The state
board of dentistry in the state in which the candidate wishes to practice will require proof
that the candidate has passed Parts I and II of the National Boards and the appropriate
state or regional clinical examination, State boards of dentistry will also require proof of
graduation from an accredited dental school and other documentation. Candidates
should familiarize themselves with the requirements of the state(s) in which they wish to
be licensed as soon as possible and complete an application with that individual
jurisdiction. Passing the ADEX Examination Series and obtaining ADEX status does not
necessarily mean that all state required clinical examinations have been completed nor
does this automatically lead to a state dental license.
Candidates should address questions to the appropriate agency:
The Joint Commission on National Dental Examinations can answer questions about
Parts I and II of the National Boards.
CITA, NERB or SRTA can answer questions about the ADEX Examination Series.
Questions regarding licensure or state requirements should be addressed to the
appropriate state board of dentistry.

II. REGISTRATION
13)
Preparing to Register for your Exam
A fully executed online application will consist of an individualized
profile complete with the appropriate documentation and photo. This is
required prior to any candidate being permitted to register for any
CITA administered examination of any Parts I, II, III, IV or V of the
ADEX examination.
Before you begin establishing your profile, we recommend that you
obtain the necessary documentation that your need based on your eligibility status, change
them into an electronic version and an electronic photo which you can upload into your profile.
It is in a candidate’s best interest to create their required profile well in advance of a published
deadline. Profiles must be verified and be approved to apply for any exam date. Late fees will
be assigned for any exam registrations that are submitted after the examination’s thirty (30) day
deadline.
Once a candidate’s profile is approved, a candidate may select an exam date and submit
payment for that exam. Candidates will have seventy two (72) hours after selecting an exam
date, to submit their necessary fees online via credit card. DO NOT select an exam date unless
you are able to submit a full payment. Any exam date request which is not properly submitted by
the candidate will automatically be removed from their profile. The candidate will be required to
re-submit another exam date, adhere to the published examination deadlines and submit the
proper fees.

14)
CITA Online Application Process and Guidelines
a) Online Profile Creation
All interested test takers may visit the CITA web site (http://www.citaexam.com/apply)
and complete the process of creating a Profile.
b) Verification
CITA will review all profiles and the satisfaction of all requirements of the candidate as
stipulated on the website and this manual, including photograph submission. Verification
can take several weeks.

c) Application Release
Once a candidate’s information has been verified, the candidate’s online profile will be
approved/verified so that the candidate may make application to the test site where they
wish to test. The candidate will then select their sections of the exam, exam site and
submit electronic payment for the examination(s) they wish to take. Candidates will have
seventy two (72) hours in which to complete the electronic payment process once the
application has been completed for the desired site. Applications that are not paid within
seventy two (72) hours are automatically dropped from a candidate’s profile.
d) Assignments up to 60
From 120 days to 60 days till the test site administration date, only candidates of record
(students registered in undergraduate/graduate programs) at the test site school will be
allowed entrance to the test site and the status of their completed applications will be
moved from “tentative” to “finalized”. Candidates may go online to determine that their
materials are in order and they have been successfully accepted and assigned to that
test site.
e) Assignments 59-30
After 60 days the assignments to the test site will be based on a first come, first served
process. The date and time at which the candidate made application to the site (post
exam selection and payment) shall serve as the qualifying date and time. Applicants will
continue to be assigned to a test site until all seats/chairs at the test site are taken.
f) Late Applications
Any application made after the published deadline will be assessed a $500 late
application fee and should be paid within seventy two (72) hrs. Candidates will be
accepted only if space is available and after any wait list has been exhausted. No late
applications will be accepted after 15 days before the first date of the exam week-end.
g) Wait List
When candidates make application they should also download a Wait List/Second Test
Site Form. This form will allow the candidate to stipulate that they wish to be placed on a
wait list should the test site become filled and they have not been assigned. If space
becomes available due to candidate withdrawal, or in the event more spaces are made
available at the tests site due to demand, candidates will be assigned to fill those
available positions from the Wait List per the stipulations and guidelines contained
therein. The "Wait List/Second Test Site" form should be completed and then uploaded
using the Scheduling Request Upload in the candidate's profile, located
at https://cita.brighttrac.com.

h) Assessed Fees
All assessed fees made prior to the 30 day deadline must be paid within seven (7) days
of the assessment of the fee. Any application with a payment balance as of the thirty
(30) day deadline will be removed from the exam and all fees will be forfeited. Late
applications must pay all assessed fees within seventy two (72) hours of the
assessment.

Step by Step Online Registration Process
Applying online is a multi-step process:
STEP 1: Go to http://www.citaexam.com/apply
Click the fill out a basic profile link and complete the form.
The email address you enter will become your username to login to your
profile and will be used to communicate your site assignment and notify you
when results are available for release. Double check your email address
and choose a secure password.
Once you have created your login and password, you will see your Dashboard which will
include the following tabs:
Dashboard. Under this tab you will find a list of items you must submit for your CITA
profile and the status of each item.
Check Mark = completed item
Exclamation Mark = item requires attention
Profile. Under this tab you can view and edit your personal information and upload your
photo, proof of graduation, etc.
Apply. Once all profile information has been uploaded and your profile has been verified
you may use this tab to apply for examinations. Detailed instructions will be presented
based on the available examinations. This tab is also where your clinical assignment will
be listed once the site schedule is finalized.
Documents. Candidates must visit this tab prior to the
examination to download and possibly fill out any
required forms and documents. Instructions about each
document will be given.
Results. Your results will be posted under this tab once
they are finalized and released.
After completing the online form and your profile has
been verified, click the Apply button to proceed to
apply for your exam.

STEP 2: Below will be displayed each time you login to your profile. The Dashboard tab is
displayed by default. Here you will find a list of current items and their status:

STEP 3: On your Dashboard page, you will be prompted to upload a photo. Click the Upload
link and follow the instructions. A current passport quality photo is required. All photos will be
reviewed by CITA and may be rejected if they are not found to be acceptable for identification
purposes. Submitting an unacceptable photo will delay your registration.
Photos must be in one of the following formats: JPG/JPEG,
GIF, or PNG.
Photos must be square and have a minimum resolution of
200 x 200 and a maximum resolution of 500 x 500. Photos
must show in your profile in the upright position.
Your photo must be a front facing headshot in the format that
would be used for a passport and your name must not be
visible on your photo. A white background should be used.
Photos must be resubmitted if any visible changes (such as hair color, facial hair, hair
length…) in appearance have taken place prior to exam. Your photo must match your
current appearance the day of the exam.

STEP 4: Your graduation status must also be validated before you can register for exams.
Verification can take several weeks depending on the method used:
Junior or Graduating Senior:
A Junior or Graduating Senior dental student may not participate in a ADEX
examination unless a certification by the dental school dean (or designee) is
submitted. A Junior or Senior form must be completed. To upload a scan, login to
your profile, then click the Profile tab followed by the Post Graduate Certificate link at
the top of the screen. Junior dental students may only take Parts II and III. Original
forms must be mailed to CITA if you are NOT a student of record at any of our
exam sites before your document may be verified.
Dental Graduate:
Dental school graduates, post graduate residents or foreign-trained graduates who
are eligible to apply to take the ADEX examination are required to submit to the CITA
office either a letter from their dental school registrar's office that verifies their date of
graduation and degree received or an official copy of their dental school transcripts in
a sealed envelope from the school that states the date of graduation and the degree
received. These need to be mailed to the CITA office. CITA will upload a scan to your
profile. You may check your Profile to see that we have received your documents.
Graduate Students of Record need to complete the Graduate Student of Record Form
in order to be recognized as a student of record at a designated exam site. This form
should be uploaded to your profile. To upload a scan, login to your profile, then click
the Profile tab followed by the Post Graduate Certificate link at the top of the screen.
Original forms must be mailed to CITA if you are NOT a student of record at any
of our exam sites before your document may be verified.
Foreign-Trained Dental Graduates:
Candidates with a dental degree from an educational program not accredited by the
ADA Commission on Dental Accreditation applying for the ADEX examination must
be authorized to take the examination by at least one state or jurisdiction that accepts
the results of the ADEX examination for satisfaction of the clinical examination
component for licensure in that state or jurisdiction. Therefore, nonaccredited
graduates must furnish an official letter from the licensing entity indicating they are
eligible to receive an unrestricted license in that state or jurisdiction upon successful
completion of the ADEX examination.
In addition, an official copy of the candidate’s dental transcripts, dental diploma and
letter from the school registrar’s office stating the dates of enrollment, date graduated,
degree received and contact information with official English translation must be
received in a sealed envelope from each entity. Misrepresentation of any application
requirements or alterations of any documents may result in the denial of the
candidate’s application or reversal of CITA status.

STEP 5: After your profile and graduation status are validated, you will be able to click on the
Apply tab to register for examinations.
Please note: There are currently options for a Full Exam (all procedures) or one
individual procedure. If you wish to take more than one procedure, then you need to
apply for each procedure separately. I.E.: If you want to register for the manikin based exam,
you need to register for the prosthodontics procedure AND then register for the endodontic
procedure.
STEP 6: Submit payment for examination fees. CITA accepts VISA and MasterCard only. Debit
cards may be used if allowable by the issuing bank and bear the VISA or MasterCard logo. All
payments are drawn immediately and must be paid in full. Failure to pay the registration fee at
the time of registration may forfeit your ability to sit for the examination. Initial registration fees
that are not paid within 72 hours are automatically dropped from your profile.
IF YOUR PAYMENT BALANCE DOES NOT MATCH WHAT YOU HAD ANTICIPATED YOUR
EXAM COST WOULD BE, PLEASE CONTACT THE CITA OFFICE WITHIN YOUR 72
HOURS. FAILURE TO DO THIS MAY DELAY ENTRANCE INTO AN EXAM. NO PAYMENT
ADJUSTMENTS WILL BE MADE ONCE AN EXAM HAS BEEN CLOSED.
NOTE: Students of record will only be charged the application fee initially. Facility and
staffing fees will be assess SEPARATELY if different then non students of record.
STEP 7: Watch your online Profile for any status changes and required document deadlines.
Final Assignments will be made no later than 15 days before the exam.
16)
Identification Card
As presented, during the application process, the candidate is required to submit a recent
electronic photo of him/herself. The photo will be applied to the candidate’s identification card,
which he/she will receive on the day of the examination. This ID is to be worn at all times during
the examination.
In order to receive the ID card and the rest of the examination materials at orientation or
registration, the candidate must provide his/her Sequential Number available through his/her
online profile (under the Apply Tab) on the online registration website, along with two
forms of personal identification. Both of these additional IDs must contain the candidate’s
signature, and one must have a recent photograph which is similar to their ID/Online Photo.

A national credit card is an acceptable secondary form of ID. An expired driver’s license,
expired passport or a social security card are not acceptable forms of ID for this exam.
ID Photos must be resubmitted online if any visible changes (such as hair color, facial hair, hair
length…) in appearance have taken place prior to exam. Your photo must match your current
appearance the day of the exam.
The candidate’s name on both forms of ID must match exactly the name used for registration. If
the name on the identification presented differs from the name (other than middle name or
initial) used for registration, official documentation or authorization of a name change must be
presented for admittance to the examination. If a candidate is not admitted because he/she fails
to provide this documentation, his/her examination fee will be forfeited.
Note: An identification card is not used for the DSE Section. However, two forms of ID as

Fee Deferrals and Refunds
Requests MUST be made in writing to CITA via email, fax or US Postal Service and MUST state
the reason for the request or transfer. Notification will be sent immediately after a determination
is made. Should a fee deferral be granted, the terms and conditions for a future examination will
be included. It should be noted that a “request for” and the “granting of” a deferral does
not guarantee seating at the requested examination site. Such seating will be assigned in
accordance with CITA’s Examination Seating Assignment Priorities as stated in this manual.
Fee deferrals and refunds will NOT be given for a patient's failure to appear, nonacceptability of a patient, or a candidate's inability to secure patients for the examination,
as patient approval is considered part of the examination.
Requests for the examination fee to be deferred to a later examination within the examination
cycle or be refunded will be as follows:
1.
A full refund of all fees paid minus a $200.00 administrative fee will be granted if the
request is received at least thirty (30) days before the initial scheduled examination date; or
2. 50% of all fees paid minus a $200.00 administrative fee will be granted if the request is
received at least fourteen (14) days before the initial scheduled examination date.
Please note that requests for a fee deferral or refund may not be carried over into the next
year’s examination cycle. Requests for a fee deferral or refund received on or after the dates
outlined above WILL NOT be honored, and all fees will be forfeited. A refund or fee deferral
request received after the dates outlined above that is due to extenuating circumstances may be
reviewed on an individual basis by the CITA Board of Directors.
19)
Procedure Cancellation Policy
Each candidate will select at the time of application, the procedures that they wish to complete
on the day of the exam. If a candidate wishes to withdraw from any procedure, they may do so
up until the time they have presented their initial patient for their first procedure they have
registered for on a given exam day. Once a patient and paperwork have been presented, the
exam has begun and no withdrawals from any procedure will be honored for that day. The
inability to complete any exam procedure that day will be graded as incomplete and shall
constitute a failure on that examination procedure and/or section. No refund will be
considered at the examination site for failure to initiate or complete a procedure/section.

Each candidate is expected to be familiar with the forms that will be used for the ADEX exam.
Extras will be available at the exam site however it is recommended that you down load and
complete some exam forms prior to arriving the day of the exam. These forms can be found in
the documents section in your online candidate profile.

Eligibility for the ADEX Exam
Dental Students
During the student’s junior year, the manikin-based portion of the examination is typically taken
at the student’s school of attendance if in a CITA member state, but at the student’s discretion,
may be taken at an educational institution within any state which is a member of and
administers the ADEX examination. Educators and students have favored the administration of
the manikin examination during the junior year of study, due to the fact that the manikin
examination is closer to the student’s preclinical laboratory experience and working with
typodont simulation.
Therefore, junior or senior students of record, attending dental schools accredited by the
American Dental Association Commission on Dental Accreditation (CODA) or the Commission
on Dental Accreditation of Canada (CDAC), are eligible to apply to take the CITA examination
when the Dean (or designated school official) certifies, in writing, that the candidate is a junior or
senior student of record and that the candidate is sufficiently prepared to participate in the
examination.
In the event the student is unsuccessful on their first attempt of the manikin examination, CITA
offers two (2) additional opportunities to retest. Although other testing agencies allow retest
opportunities, those retest opportunities are typically not at the student’s institution of
attendance and require that the student travel to another testing facility which may be
geographically remote. CITA believes that students should first be afforded an opportunity to
retest at their school of attendance, if at all possible, and therefore, many of the CITA states
offer multiple retest opportunities at the same school as the student is attending. In the event
there are inadequate retest numbers to justify the conduct of an examination, students may be
required to travel outside their school of attendance.
The initial patient-based portion of the examination is administered at the student’s school of
attendance if in a CITA member state, during the student’s senior year, usually in January,

February or March. Again, CITA offers multiple retest opportunities for the patient-based
examination typically at the student’s school of attendance.
Candidates and educational institutions find that the examination being administered over two
(2) didactic years is less disruptive to the educational institutions and their efforts to train their
students within their established curriculums. In addition, the students favor spreading the
examination process out over two (2) years of study and relate that it is less stressful and less
intensive than when administered during one (1) curriculum year.
While CITA has made effort to have retest opportunities at the same site as the students’ school
of attendance, the opportunity to retest on any unsuccessful performance(s) may be at sites
other than the student’s school of attendance. It will be the responsibility of the candidate to
arrange all travel plans in preparation of any examinations given by CITA.
b. Dental School Graduates, Post Graduate Residents
Individuals who are dental school graduates, post graduate residents or foreign-trained
graduates may participate in any examination that is administered by CITA as long as they meet
the requirements set forth by CITA.
Dental school graduates, post graduate residents or foreign-trained graduates are eligible to
apply to take the CITA administered ADEX examination and are required to submit to the CITA
office either a letter from their dental school registrar's office that verifies their date of graduation
and degree received or an official copy of their dental school transcripts in a sealed envelope
from the school that states the date of graduation and the degree received.
Candidates are assigned seating in accordance with the Examination Seating Assignment
Priorities stated in this manual.
The examination is administered over a two (2) day cycle. Candidates are not required to take
both sections (manikin based and patient based) of the two (2) day cycle, but rather may
choose to separate the sections during the examination series. However, individuals who elect
to take the examination in separate sections must complete the entire CITA examination within
18 months of their first attempt at any Part of the examination.
c.
Foreign-Trained Dental Graduates
Candidates with a dental degree from an educational program not accredited by the ADA
Commission on Dental Accreditation applying for the CITA examination must be authorized to
take the examination by at least one state or jurisdiction that accepts the results of the CITA
examination for satisfaction of the clinical examination component for licensure in that state or
jurisdiction. Therefore, non-accredited graduates must furnish an official letter from the licensing
entity indicating they are eligible to receive an unrestricted license in that state or jurisdiction
upon successful completion of the CITA examination.
In addition, an official copy of the candidate’s dental transcripts, dental diploma and letter from
the school registrar’s office stating the dates of enrollment, date graduated, degree received and
contact information with official English translation must be received in a sealed envelope from

each entity. Misrepresentation of any application requirements or alterations of any documents
may result in the denial of the candidate’s application or reversal of CITA status.
CITA reserves the right to deny admission to its examination should, in CITA’s sole discretion, it
has concern regarding the candidate’s mental, physical or emotional well being, or questions
regarding the candidate’s preparedness or educational training to the extent that it may
endanger patients engaged in the examination process.
22)
Dental School Certification
Junior or Graduating Senior students of record, attending dental schools accredited by the
American Dental Association Commission on Dental Accreditation (CODA) or the Commission
on Dental Accreditation of Canada (CDAC), are eligible to apply to take the CITA administered
ADEX examination when the Dean (or designated school official) certifies, in writing, that the
candidate is a junior or senior student of record and that the candidate is sufficiently prepared to
participate in the examination.
23)
Dental Student Disqualification
A candidate may be disqualified from participating in the examination series by the dean of
his/her dental school at any time after certification if the candidate ceases to be a senior student
of record or the dean (or designated school official) determines that the candidate is ineligible
for any reason. In such cases, fees paid by candidates who are disqualified shall be forfeited. A
candidate who is disqualified for the remainder of the academic year during the Curriculum
Integrated Format shall have access to the Traditional Format in a subsequent academic year,
if he/she graduates and presents a diploma. In such a case, the candidate must submit a new
registration and appropriate documentation and fees.
A candidate disqualified from an individual examination section by his/her dental school dean
(or designated school official) may continue with the other examination sections. However,
he/she will need to re-register and submit the appropriate fee and documentation of
requalification for those sections later being retaken.
24)
Request for Non-Standard Accommodations
CITA, in accordance with the Americans with Disabilities Act, will provide reasonable and
appropriate accommodation for candidates with documented disabilities. CITA will provide
reasonable accommodation, auxiliary aids, or services that are necessary to the extent required
by law provided the requested accommodation, auxiliary aids, or services would not
fundamentally alter the measurement of the skills or knowledge the exam is intended to test.
Candidates with disabilities who require accommodation while taking the CITA exam may
apply to CITA for consideration of the accommodation by submitting the “Special
Accommodation Request Form” and documentation no later than 45 days prior to the
exam. For an accommodation request to be considered the “Special Accommodation Request
Form” must be completed and submitted to CITA with supporting documentation in accordance
with the conditions and guidelines stated.

The candidate is responsible for obtaining documentation of disabling conditions that require
accommodation. Specific guidelines for acceptable documentation of disability can be found
under the “Disability Documentation Guidelines” section of the “Special Accommodation
Request Form”. It is recommended that these guidelines be shared with the evaluators
providing the documentation for the applicant, as incomplete or inadequate documentation
written in support of accommodation may be denied by CITA as insufficient.
a.
Special Testing Provisions and Auxiliary Aids
As stated, CITA will provide appropriate auxiliary aids for such persons with impaired sensory,
manual or speaking skills unless providing such auxiliary aids would fundamentally alter the
measurement of the skills or knowledge the examination is intended to test. To ensure that
auxiliary aids or other requested modifications are available and can be provided, candidates
requesting such modifications or auxiliary aids must;
• Submit, in writing, a request for the auxiliary aid or modification stating the exact auxiliary
aid or modification(s) needed. Requests received after the registration deadline date and
retroactive requests will not be considered.
• Provide documentation of the need for the auxiliary aid or modification, indicating any
portion of the dental examination for which such aid or modification will be needed.
• Provide a letter from an appropriate healthcare professional documenting the disability.
This letter must be received by CITA no later than 45 days prior to the date of the exam.
• In providing such auxiliary aids or modifications, CITA reserves the ultimate discretion to
choose between effective auxiliary aids or modifications and reserves the right to maintain
the security of the examination. All information obtained regarding a candidate’s physical
and/or learning disability will be kept confidential, with the following exceptions:
• Authorized individuals administering the examination may be informed regarding any
auxiliary aid or modification.
• First aid and safety personnel at the test site may be informed if the disability might require
special emergency care.
CITA reserves the right to administer the DSE Section in an alternative form other than by
computer and will arrange with the candidate on an individual basis. CITA reserves the right to
verify all information submitted by an applicant in support of a request for accommodation, and
additional information from evaluators providing the supporting document may be requested.
CITA has the right to refuse an accommodation request and/or deny the candidate’s eligibility
status if it is found that either the candidate has deliberately misrepresented the information or
the profession providing the information.
b.
Requests for Special Accommodations Due to Religious Constraints
Candidates requesting special accommodations due to religious constraints must submit in
writing a request for Religious Accommodation and specifically the accommodations they need.
This request should be submitted by mail at least 45 days prior to the first day of the exam to
the CITA Director of Dental Operations.

Examination Seating Assignment Priorities
Up to 60 days till the test site administration date, only candidates of record (students registered
in undergraduate/graduate programs) at the test site school will be allowed entrance to the test
site and the status of their completed applications will be moved from “tentative” to “finalized”.
Candidates may go online to determine that their materials are in order and they have been
successfully accepted and assigned to that test site.
After 60 days the assignments to the test site will be based on a first come, first served process.
The date and time at which the candidate made application to the site (post exam selection and
payment) shall serve as the qualifying date and time. Applicants will then be assigned into the
test site until all seats/chairs at the test site are taken.
Any application made after the published deadline will be assessed a $500 late fee and should
be paid within seventy two (72) hours. Candidates will be accepted only if space is available. No
late applications will be accepted after 15 days before the first date of the exam week-end.
26)
Waiting-List
When candidates make application they should also download a Wait List/Second Test Site
Form. This form will allow the candidate to stipulate that they wish
to be placed on a wait list should the test site be filled and they
have not been assigned. If space becomes available due to
candidate withdrawal, or in the event more spaces are made
available at the tests site due to demand, candidates will be
assigned to fill those available positions from the Wait List per the
Examination Seating Assignment Priorities contained therein. The
"Wait List/Second Test Site" form should be completed and then
uploaded using the Scheduling Request Upload in the candidate's
profile, located at https://cita.brighttrac.com/me/
27)
Change of Address
During the online registration process candidates will be required to create their unique profile
which will contain all relevant contact information. It is extremely important that candidates
maintain a current email and physical mailing address with CITA. This is the only way to ensure
that there will be a timely receipt of important material which CITA may send by mail or email.
28)
Communications with CITA Office
CITA will respond to any questions which are not specifically addressed in the manual, on the
website or candidate presentations. Candidates are expected to have thoroughly read the
manual and reviewed the candidate presentations prior to directing any questions to CITA staff.
After having read or reviewed the required materials, should a candidate wish to have specific
clarification on any questions or issues regarding the ADEX examination, they should make
those requests in writing. Conversations via the telephone will not supersede materials
published in the manual, website or candidate presentations.

Limited Liability Insurance
CITA has a blanket Professional Liability Insurance policy that covers all dental candidates, and
their assistants, for all ADEX examinations. The cost of that coverage is included in CITA’s
examination fee. Therefore, candidates and/or their assistants are not required to obtain
additional limited liability insurance.
30)
Three Time Failure Rule
Candidates failing one or more of the same Parts of the ADEX examination on three (3)
successive attempts must begin the entire examination process again and retake all Parts of
the examination. Any Parts on which the candidate may have been previously successful will
not be recognized or counted toward successful completion of the retest of the entire clinical
examination process.
When this situation occurs, the candidate will be considered an initial applicant and must
submit an initial application together with appropriate documentation and applicable fees in
accordance with the policies and procedures of initial applicants.
31)
Examination Cancellation Policy
CITA reserves the right to cancel or postpone
any examination where the number of candidates
registered to take the examination does not, in
the sole discretion of CITA, financially justify the
administration of the ADEX examination or in the
event of an emergency or other unforeseen
circumstance that is beyond CITA’s control.
Emergency or unforeseen circumstances may include, but are not limited to, acts of nature, acts
of terrorism, events resulting in the destruction of CITA office or testing site facility, loss or
delays in the delivery of necessary equipment and/or supplies by a shipping agent, failure of the
testing site facility to provide expected and necessary services, equipment, supplies or
personnel or other similar events.
Under no circumstance does CITA assume liability for costs incurred by candidates in preparing
to take a CITA examination. This policy extends to situations where CITA might be forced to
cancel an examination because of an emergency or unforeseen circumstance, such as those
listed above, or for the lack of participants as explained above. However, if such an
examination cancellation were to occur for those reasons stated or any reason in CITA’s sole
discretion, CITA would either refund those candidates application fees or reassign candidates to
the next available examination site or reschedule the examination at the earliest possible date.

III. REPORTING AND APPEALS
32)
Release of Scores
Scores for all ADEX examinations are posted to candidate
profiles within ten (10) business days after the candidate’s
examination is completed. Candidates will need their email
and password to access their online candidate profile and
results.
Student candidates’ scores are released to the dental schools
who have entered into a confidentiality agreement with CITA.
School Coordinators at these schools will be given online
access by CITA to view their student’s exam progress. These scores are released online at the
same time as the student scores and can be view by the authorized school liaisons via their
School Coordinator portal.
Scores are released to other state boards when a candidate’s conduct or performance raises
issues of character and/or fitness that CITA feels such information should be made known to a
licensure board.
Upon completion of a scheduled exam, scores are automatically released to the current
member states of CITA via email within five (5) business days after scores are posted to a
candidate’s profile. These scores are independent of the scores a candidate needs to send to
fulfill their licensing requirements. Please refer to the section on Notification of ADEX status for
details. See our Score Verification Procedures for details of how to send your scores to other
jurisdictions.
33)
School Notification of Scores for Student Candidates
Inasmuch as the opportunity for remediation within the dental school
and opportunities for curriculum development are intended to be
significant benefits resulting from the administration of the CITA
examination, the individual scores of a student candidate will be
released by CITA to the candidate’s dental school only if the dental
school has entered into a confidentiality agreement with CITA (which
protects the student from further disclosure of that candidate’s scores,
or for the use of those scores other than their intended purposes of
candidate remediation). When such disclosure is made, School
Coordinators at these schools will be given online access by CITA to
view their student’s progress and scores. These scores are released
at the same time as the student scores and can be view by the
authorized school liaisons via their School Coordinator portal.

Release of Scores to Non Member States (Score Verification Procedure)
Candidates who wish to have their scores released and/or a copy of the examination manuals
transmitted to jurisdictions that are currently not a member state of CITA, must submit a
Notarized Duplicate Score Request Form to the CITA office along with a Certified Check or
Money Order for the set fee amount. A fee of $25.00 will be charged per address to send
specified Exam Scores and/or CITA/ADEX Status to non-member state boards. An additional
$100.00 for the examination manual ($50.00 manikin-based; $50.00 patient-based) if required,
must be included with the request.
CITA will only send specified scores to State Dental Boards for licensure purposes. All other
requests must be made in writing and approved by the CITA Board of Directors.

Notification of ADEX Status to CITA Member State Boards
Once a candidate has successfully passed all Parts of the CITA administered ADEX
examination (Parts I, II, III and V) a certification of ADEX Status is automatically distributed to all
current CITA member state boards within the designated CITA time frame. Note that Part IV is
optional in some states. These results typically are accepted by state boards for a period of five
(5) years from the date of each candidate’s successful completion of the CITA examination, or
for a different time period as determined by the individual state boards.
Candidates should contact the individual state boards of dentistry to ascertain that board’s
acceptance period for this examination and other requirements that the candidate must fulfill to
meet its standards and requirements for licensure. In other words, completion of the ADEX
examination alone, WILL NOT qualify a candidate for licensure, as other requirements
within each of the jurisdictions MUST be fulfilled prior to the candidate engaging in any
activity or activities which may be construed as the practice of dentistry. It is the
candidate’s sole responsibility to determine that all requirements have been met in the
jurisdiction in which they wish to practice, prior to performing those acts which may
constitute the practice of dentistry.
Determinations as to who is qualified for licensure are controlled by individual state law;
consequently, the requirements may not be uniform from state-to-state. Each licensing
jurisdiction may use the examination results to the extent authorized by its statutes.
NOTE: CITA will send an electronic listing of those candidates who have attained ADEX
Status to all their current CITA member state boards within fifteen (15) business days
from the completion of their last requirement for ADEX Status. Candidates who take
sections in two or more testing agencies may be required to request their scores in
writing from each agency. See Score Verification Procedures for details.

Re-Examination and Remediation
Candidates failing any one or more of the same Parts of the ADEX examination on three (3)
successive attempts may be required by the state board of the state in which they are seeking
licensure, to have completed a course in remediation prior to
retaking the ADEX examination or applying for licensure in that
state or which may have rules regarding remediation for
candidates who have been unsuccessful on examination
Sections on multiple attempts. It is the responsibility of the
candidate to obtain and complete all requirements for remedial
education in accordance with the requirements of the licensing
jurisdictions in which they seek to obtain licensure.
Therefore, candidates should contact the board of dentistry in the states or jurisdictions in which
they require licensure to determine requirements for remediation. It is the candidate’s
responsibility to obtain and complete all requirements for remedial education in accordance with
the requirements of the licensing jurisdictions. Remediation may be provided at a candidate’s
school as part of the Curriculum Integrated Examination process
CITA does not require documentation of remedial education prior to reexamination for any
section of the ADEX Dental Examination Series.
CITA does not assume any responsibility for providing this information or for monitoring the
completion of such requirements prior to examination.
37)
Appeals Process
In the ADEX process a provision is made so that a candidate may appeal the
results of his/her examination if he/she believes the results were adversely
affected by extraordinary conditions during the examination that affected the
final outcome of the candidate’s examination. Appeals are reviewed on the
basis of facts surrounding the decision made during the examination.
Appeals based on patient behavior, tardiness, or failure to appear will not be
considered. The appeals process is the final review authority, and if the appeal is denied, there
is no further review process authorized by or conducted by CITA, NERB or SRTA.
In addition, CITA maintains an examination paperwork review process whereby a candidate
may request a review of his/her individual exam results based upon a review of paperwork or
score tabulations. This is a formalized process and is conducted by a special committee whose
charge is to review the facts and determine if the examiners' findings substantiate the results.
Any request for such a review must be received at CITA’s central office no later than fourteen
(14) days following the official date on which the scores were mailed to the candidate.
CITA’s special committee is required to complete its review within sixty (60) days from the time
of receiving a formal request; during that time, the candidate may apply for a re-examination. If
the candidate files a formal request, then retests and passes the examination before the

request has been fully processed, the review will be terminated and the $250.00 filing fee
will be forfeited by the candidate.
In determining whether to file a petition for review, the
candidate should be advised that all reviews are based
on a re-assessment of documentation of the
candidate’s
paperwork
for
the
examination.
Candidates should understand that the review does
not include a regrading of their performance.
The review WILL NOT take into consideration other
documentation that is not part of the examination
process, such as post-treatment photographs impressions, models, character references or
testimonials, dental school grades, class ranking, faculty recommendations, or opinions of other
"experts" solicited by the candidate. In addition, the review will be limited to a consideration of
the results of only one (1) examination at a specific test site. Candidates will not participate in
the review process and will be notified in writing, within sixty (60) days of receiving the review
request, as to the results of the review.
Candidates who contact CITA’s administrative office regarding their examination results must
clearly indicate in a written form whether they simply wish to express a concern related to the
examination or whether they are interested in initiating a formal review petition. A $250.00
filing fee will be charged by CITA to file and process a formal review petition.
If a candidate wishes to pursue an examination paperwork review, the appropriate form must be
completed and submitted to the CITA office along with the $250.00 petition fee All reviews of
candidate appeals include the score certification procedure described above and are based on
a reassessment of the documentation of the candidate’s performance on the examination. The
review is limited to a determination of whether there exists substantial evidence to support the
judgment of the examiners at the time of the examination.
Again, the review will not take into consideration other documentation that is not part of the
examination process. Opinions of the candidate, auxiliaries, faculty members, patients,
colleagues, examiners acting outside of their assignment area, and records of academic
achievement are not considered in determining the results of the examination and do not
constitute a factual basis for an appeal. Consideration can only be given to documents,
radiographs, or other materials that were submitted during the examination and remain in the
possession of the testing agency. Any other information such as radiographs, photographs, or
models of a patient taken after the completion of the examination will not be considered in the
appeals process. Any candidate who receives a failing score on an ADEX examination may, on
his/her own behalf, submit a candidate appeal of that failing score.

Test Development
The examination series is developed and revised by the ADEX Dental Examination Committee.
This committee is comprised of representatives from every ADEX member state, as well as the
participating regional test administering agencies. The committee has considerable content
expertise and also relies on practice surveys, current curricula, standards of competency and
the American Association of Dental Boards (AADB)’s Guidance for Clinical Licensure
Examinations in Dentistry to ensure that the content and protocol of the examination are current
and relevant to practice. Examination content is also determined by such considerations as
patient availability, logistical restraints and the potential to ensure that a skill can be evaluated
reliably. The examination content and evaluation methodologies are reviewed annually and
periodically changes to reflect current best practices.

Overview of Examination Components
Section I: Diagnostic Skills Examination (DSE) (computer-based)
The computer-based ADEX Diagnostic Skills Examination (DSE) Section is administered at a
PSI Testing Center of the candidate’s choice, upon authorization by CITA and will be available
beginning on or about February, 2014. The DSE Section may be taken either before or after the
patient-based and manikin-based examination sections. It is given on one day and is
approximately 4 hours long.
Candidates may take the DSE section up to three times within the 18-month exam period.
(Remember: all sections must be completed successfully within 18 months after the first section
of the series is initiated.) Scores on the ADEX DSE are generally reported at the end of the first
full week of the month following the month in which the DSE was taken.

The Prosthodontics and Endodontics Examination Sections will be administered at the
candidate’s dental school on a specified date(s) between January and August 2014, as
determined by CITA and the dean of the dental school.
Candidates may take these examination sections up to three times. The first opportunity to take
the manikin-based sections will be on a specified date(s) in January 2014, and subsequent
retest opportunities will be available on a specific date(s) during February - September 2014.
Both retake opportunities will be administered at regional locations that may or may not be the
candidate’s own school. If a candidate has neither passed nor exhausted his/her three
examination opportunities following the January - September testing dates, he/she still has up to
18 months from the date when he/she took the first section of the series to complete a third
attempt.

Sections IV and V: Restorative and Periodontal Clinical Examinations
(patient -based)
The Restorative and optional Periodontal/Scaling Clinical Examination Sections will be
administered at the candidate’s dental school on a specified date(s) in January - September
2014, as determined by CITA and the dean of the dental school.
Candidates may take these examination sections up to
three times. The first opportunity to retake the patientbased sections will be on a specified date(s) beginning
January 2014, and retest opportunities will be available on
subsequent date(s) during February - September 2014.
Both retake opportunities will be administered at regional
locations that may or may not be the candidate’s own
school. If a candidate has not either passed nor exhausted
his/her three examination opportunities following the February - September timeline, he/she still
has up to 18 months from the date when he/she took the first section of the series to complete a
third attempt. Candidates should consult the web site of the agency administering the ADEX
examination for all sites and dates on which the examination will be administered.

As previously stated, the examination consists of five (5) individual, skill-specific Parts: one (1)
computer-based DSE component, (2) manikin-based simulation, and two (2) patient-based
examinations. Each examination is listed below:

The ADEX examination has been developed, and is revised as needed, by the Examination

published literature and textbooks on psychometric principles and the American Association of
Dental Examiners’ (AADE) publication entitled “Guidance for Clinical Licensure Examinations in

the practice of dentistry. Determining the examination content is also guided by such
considerations as patient availability, logistical restraints, and the potential to ensure that a skill
can be evaluated reliably. The examination content and evaluation methodologies are reviewed
on an ongoing basis and are revised annually.

Examination Site Registration and Administration
a. Registration and Orientation
Registration and orientation typically takes place the morning or afternoon of the
examination day. For the patient based examinations candidate registrations will occur in
the clinic at the Check In Station. Registration for the manikin based sections will occur in
the simulation laboratory where the examination will be conducted or at another room within
the facility. Location and time are determined jointly between the Chief Examiner and the
Test Site Liaison. All candidates are required to complete registration for the examination for
which they have been assigned during the published registration time and place. Those
who do not register during the published registration time line will not be permitted to
participate in the examination. The candidate must present two forms of identification,
both with the candidate’s signature and one with a recent photograph. Candidates who do
not have these required forms of identification will not receive the examination materials
during the registration process and will not be admitted to the examination.
b. Examination Process
The administrative process of the examination sections is outlined and discussed in the
Patient Based and the Manikin Based Candidate manuals. Candidates should download,
print and read these manual prior to arriving at the examination site. Failure to read the
examination manuals prior to attendance at an examination can significantly impact a
candidate’s performance.
c. Typodonts and Manikins
CITA uses a typodont manufactured especially for CITA by
Nissin Dental Products Inc. They can be purchased from
Kilgore International at http://www.kilgoreinternational.com by
ordering the CITA-EP KIT - COMPLETE TESTING KIT
Only typodonts and teeth supplied by CITA may be used for the
examination.
d. Dates and Sites for Clinical and DSE Exams
For information about the clinical patient based and the clinical simulated manikin based
examination sites, dates and fees, visit the CITA website: www.citaexam.com.
The computer-based Diagnostic Skills Examination (DSE) scheduling will begin in mid-January
2014 for DSE exams beginning in February
2014. A current listing of the locations of PSI
Testing Centers at which the computer-based
DSE Section is offered throughout the year can
be accessed by going to the PSI website at www.psionline.com. Appointments must be
scheduled with a minimum of 24 hours notice. Appointments are made based on availability.

Candidates must provide no less than 48 hours notice (Monday-Friday) to reschedule/cancel
their testing appointment. Rescheduling/cancellation is done through PSI’s Central Registration
Office NOT the local testing center.
Failure to provide 48 hours notice will result in forfeiture of the initial DSE test fee. The
initial DSE fee is included in a candidate’s initial exam application fee. Candidates will need to
reschedule and pay a retest DSE fee of $350 with CITA. Requests for waivers must be
submitted to PSI in writing within 72 hours of the testing appointment and must include a doctor
note verifying a medical emergency. Candidates who fail to appear for their scheduled test
appointment will be reported as a no-show and a candidate will need to reschedule as a retest
candidate through CITA and pay the appropriate DSE retest fee of $350.
Candidates are provided the option of receiving a fax or email confirmation of their PSI test
appointment. This confirmation will provide the candidate the PSI exam title, date, time, and
location of their PSI test appointment. Failure to receive a fax or email confirmation does not
invalidate the Candidate’s testing appointment. Candidates are responsible for noting the
date, time and location of their PSI testing appointment.
e. Completion of the ADEX Exam
Senior Dental Students, Dental School Graduates, Post-Graduate Residents and ForeignTrained Graduates
All four or five sections of the ADEX examination must be successfully completed within
eighteen (18) months of the date of a candidate’s initial attempt of any Part of the ADEX
examination. A maximum of two (2) retest opportunities are allowed for candidates who are
unsuccessful on any Part of their first attempt.
If Parts I, II, III, VI (optional) and V of this examination are not successfully completed within the
timeframes set forth above, regardless of the reason, all Parts of the examination must be
retaken. For each examination, a new exam request must be submitted, together with the
applicable fees.
Once a candidate has successfully passed all Parts of the ADEX examination (Parts I, II, III, IV
(optional) and V), a certification of ADEX Status is automatically distributed to all current CITA
member state boards within the designated CITA time frame. See the Notification of ADEX
Status section for details. Note that Part IV is optional and not required for ADEX status. A list
of the member states may be found on CITA’s website at http://www.citaexam.com. These
results are typically accepted by state boards for a period of five (5) years from the date of each
candidate’s successful completion of the ADEX examination, or for a different time period as
determined by the individual state boards.

f. Junior Students
Junior students may participate in Parts II & III of the ADEX examination and will be eligible to
take Parts IV & V during their senior year. They will be required to successfully complete all
Parts of the ADEX examination within eighteen (18) months AFTER initially taking their first part
of the examination.
If Parts I, II, III, IV (optional) and V of this examination are not successfully completed within the
timeframes set forth above, regardless of the reason, all Parts of the examination must be
retaken. Note that Part IV is optional and not required for ADEX status but may be required by
some jurisdictions. For each examination, a new exam request must be submitted, together with
the applicable fees.

Special Accommodation Information
CITA, in accordance with the Americans with Disabilities Act, will provide reasonable and
appropriate accommodation for candidates with documented disabilities. CITA will provide
reasonable accommodation, auxiliary aids, or services that are necessary to the extent required
by law provided the requested accommodation, auxiliary aids, or services would not
fundamentally alter the measurement of the skills or knowledge the exam is intended to test.
Candidates with disabilities who require accommodation while taking the CITA exam may apply
to CITA for consideration of the accommodation by submitting the Special Accommodation
Request Form and documentation no later than 45 days prior to the exam. For an
accommodation request to be considered the Special Accommodation Request Form must be
completed and submitted to CITA with supporting documentation in accordance with the
conditions and guidelines stated.
The candidate is responsible for obtaining documentation of disabling conditions that require

Evaluators conducting assessments and providing a diagnosis of any disability must be qualified
to do so. Documentation submitted to CITA should include information about the evaluator’s
licensure and/or certification. For the diagnosis of learning disabilities, examples of qualified
evaluators include licensed school, education and clinical psychologies, neuropsychologists,
psychiatrists, learning disability specialists, or medical professionals experienced within the field
of learning disabilities. For the diagnosis of Attention Deficit/Hyperactivity Disorder (AD/HD) or
other mental disorders that require accommodation, examples of qualified evaluators include
licensed school, education and clinical psychologists, neuropsychologists, psychiatrists or other
medical evaluators with experience and training in psychological/psychiatric evaluation.

Documentation submitted must be less than three years old.
The Evaluation and Assessment Procedures MUST BE Comprehensive and
Appropriate for Adult Subjects
Documentation must be thorough and comprehensive.
AD/HD and learning disability
documentation must minimally include appropriately named measures of intellectual ability
and/or information processing such as the Wechsler Adult Intelligence Scale, Third Ability, or the
Woodcock-Johnson Psycho-Educational Battery-Revised Tests of Cognitive Ability, and
appropriately normed measures of academic ability such as Woodcock-Johnson PsychoEducation Battery-Revised Test of Academic Achievement. Screening tests such as the Wide
Range Achievement Test are inappropriate as the sole measure of a person’s academic skill
development. A narrative describing the test procedures and their instruments used must be
included. Documentation submitted without scores of tests administered will be denied, and
scores submitted without a narrative report that supports diagnosis will be denied.
The Documentation MUST Provide Evidence of Support for the Specific
Accommodation Requested by the Candidate and MUST List the Specific
Accommodation Required
It is the candidate’s responsibility to ensure that the evaluator completing the documentation
understands the nature of the exam and specifies the accommodation requested for each
section of the exam. Since this is a clinical exam, the accommodation routinely made for written
exams may not apply.
When a written exam is involved, it is imperative that the
recommendation include whether the candidate requires a separate testing environment.

CITA Pre Exam Checklist
PRIOR TO EXAM
Read the entire Candidate Manual for the ADEX Dental Examination Series.
Complete the online registration by following the instructions in (Registration Procedures) of
this manual.
Submit Dental Assistant form and/or Interpreter form three weeks prior to the exam if
applicable.
Submit Examination Disclaimer form (patient based only) and the Preparation and Orientation
form no later than one week prior to the exam.
PSI TESTING CENTERS
Select the PSI Testing Center where you will take the DSE Section. After your registration has
been processed and CITA has sent you an authorization letter, schedule your appointment with
PSI by phone (1-800-211-2754) or online at www.psionline.com.
Take two forms of personal identification to the PSI Testing Center: one with a recent photo,
and both with your signature. Acceptable forms of ID include: valid current driver’s license,
passport and military ID. A credit card is acceptable as a secondary form of ID. An out-of-date
driver’s license is not considered valid ID for this purpose. If your name has recently changed
due to marriage, divorce or other legal reasons, bring a copy of the marriage certificate or court
document to the PSI Center.
TAKE TO THE CLINICAL EXAMINATION SITE AND THE EXAM REGISTRATION
Two forms of identification, both with your signature and one with a recent photograph.

PATIENTS
Complete appropriate CITA forms for each patient prior to exam.
Ensure that the patient meets the ADEX requirements as published in the Candidate Manual.
Bring all necessary radiographs to the testing site.
Review all the criteria that are to be evaluated in the clinical sections of the examination
series.
Inform the patient that this exercise is not a complete oral care treatment.
Ensure that that a back-up patient(s) is/are available if needed.
