The Specialized Carriers & Rigging Association is pleased to publish this Oversize/ Overweight Permit Manual. Each
state analysis includes information in a standardized format: contact, legal limits, special permit limits, general
restrictions, types of permits available, fees, escort needs, fines, and restricted travel areas. Telephone numbers,
locations, and hours of operation are listed for ports of entry and permit branches. However, readers are always
advised to check with the state offices on current laws and procedures. This project could not have been completed
without the advice and consultation of many state officials. We thank all of those who provided permit manuals,
maps, laws, regulations, and various other forms of documentation.

A "Superload" is any load that exceeds 15 feet high, 16 feet wide, 110 feet
long, and/or 120,000 pounds, or fails the overload analysis. Any load that fails
the overload analysis must be reviewed by an Indiana Department of
Transportation (INDOT) engineer. Loads that are reviewed by INDOT engineers
require additional processing time. The new permitting system allows you to
have the INDOT engineering analysis done ahead of time, before you need the
permit, and receive a superload preapproval number. Once you have this
preapproval number, you can get additional permits using the same vehicle
configuration and route for the next 30 days without any additional INDOT
analysis or delays.
PERMIT RESTRICTIONS
General - Permits are valid for fifteen days on most permits. Travel is allowed from one-half hour
before sunrise to one-half hour after sunset Monday through Friday (For loads exceeding 16’ wide, 15’
high or over 200,000 pounds).
Divisible load permits are not available except on "extra heavy duty highways". You must have a
special weight permit commonly called Michigan Train Permits (317) 615-7400. To obtain a special
weight permit you must register and pay an annual fee to Indiana. Routes are designated by statute.
Insurance — Minimum amounts of $25,000/50,000/10,000 for personal liability and property damage;
a certificate of insurance and company agreement must be filed with the permit office.
Permit Information Required — Tractor description (make, year of tractor, license number, Vehicle Identification
number); overall dimensions; mileage, axle weights; gross weight; axle spacings; number of tires and size);
load description; origin and destination; routes to be traveled; effective dates.
Weekend/Night Travel - Hours of Operation
Up to 110’ long 10’ wide, 13’6” high, and under 200,000 lbs Continuous travel 7 days a week.
Over 110’ long, 10’-14’4” wide, up to 15’ high 7 days a week (1/2 hr before sunrise - 1/2 hr after sunset)

Inclement Weather – Not allowed to operate during hazardous weather, road and/or traffic conditions

Ninety-day — (oversize only) up to 110' long, 12'4" wide, must be legal height and weight.

Extra Heavy Overweight Trip (special weight permit) — $42.50. For this type of permit company
must register annually and pay $25 annual registration fee per company.
Toll Road Gate:
$20 annually per gate per truck.
There are four types of mobile home permits, 12’4” wide permits,
14’4” wide permits, 16’ wide permits and mobile home dealer permits. Currently, you
may purchase single permits for 12’4” wide and for 14’4” wide you may purchase 90day or annual permits. 90-day or annual permits are not available for 16’ wide mobile
homes. You must peruchase single permits for these.

Labeling — Load must be marked front and rear with an oversize load sign. Sign must be 18" high and
between 6' and 8' long, have yellow background, and have 12" high black letters with a 2" brush stroke.
Oversize movements must be marked with six (6) red flags at least 18" square.
Escort Vehicles and Personnel — Escorts must have an amber flashing light on top of the vehicle
visible for 500'. Escorts must travel at least 500' from the load. “Oversize Load” sign must be visible to
approaching traffic.
Permitted vehicles must have escorts when they exceed 12'4" in width, 110' in length, or 14'6" in
height. One escort vehicle must be front of the load. One front and one escort must be in the rear of
the load on 2-lane highways when load width exceeds 14’ 4” to 17’. The same dimensions on a 4-lane
highway and interstates require one rear escort. If the load exceeds the 14'6" height requirement the
front escort must have a height stick. Loads over 17’ on 4-lane highways and interstates require 2
police escorts.
Certification – Pilot Car Escort Certification is not currently required.
Law Enforcement Escort – Some OS/OW loads may require a police escort or
pilot vehicles bearing warning signs and/or flashing lights.
FINES
If an operator is caught in violation of the overdimensional permit issued, the permit will be confiscated
and the company must write a letter to Motor Carrier Services stating corrective action has been taken to
prohibit the violation from recurring. No additional permits will be issued until Motor Carrier Services
receives a letter and corrective action is satisfactory.

Weight violations fall into three categories — Class A, B or C infractions.

Fines are calculated based on the vehicle’s legal weight. Operators may be cited for the greater fine of
an over-axle or over-gross violation. Drivers may not shift load to avoid an over-axle violation.
Violation to Permit
Anytime a permit is violated or a carrier is found to be hauling an OSW permit the carrier is put on
violation and no further permits can be ordered until the violation is corrected.
To be removed from violation we will need an explanation of the violation, what the carrier’s company
policy was concerning the violation, and what the company will do to ensure no further violations occur.

Once we receive and approve your letter, your company will be taken off violation and can obtain any
permits needed. However, Indiana does assess civil penalties up to $5,000 for violations of OSW permits
or operating without OSW permit. We will send a certified letter of a show cause hearing to determine the
civil penalty amount.

Indiana Code
Moving heavy equipment at railroad grade crossing.
(a) A person may not operate or move a caterpillar tractor, steam shovel, derrick, roller, or any
equipment or structure weighing more than ten (10) tons and having a normal operating speed of
not more than (6) six miles per hour or a vertical body or load clearance of less than (9) nine inches
above the level surface of a roadway upon or across tracks at a rail road grade crossing without first
complying with this section.
(b) Notice of intended crossing under this section shall be given to a superintendent of the railroad, and
a reasonable time shall be given to the railroad to provide protection at the crossing.
(c) Before making a crossing under this section, the person operating or moving a vehicle or equipment
described in subsection (a) shall first stop the vehicle or equipment not less than (10) ten feet and
not more than (50) fifty feet from the nearest rail or railway. While stopped, the person shall
proceed until the crossing can be made safely.
(d) A crossing may not be made when warning is given by an automatic signal, crossing gates, a
flagman, or otherwise of the immediate approach of a railroad train or car.

The form may also be completed electronically at www.in.gov/dor. There are three options when ordering
permits. You may go online and obtain permits, fax your application to Motor Carrier Services or
you can use a Permitting Service. To fax your applications, complete form M-233 then fax to
(317) 8212336. When obtaining your permits on-line you must first create a User ID by clicking “First Time
User”. To accomplish this go to www.in.gov/dor, select Motor Carrier Professionals from the left side
navigation menu, and then select OSW. Next select OSW under online applications.
The OSW online service application allows carriers to electronically request, pay for, and print
oversize and/or overweight permits. You may also amend permits and register for special weight
permits. The electronic permit system is the fastest way to request and obtain your permits.
SPECIAL NOTES
Recent Changes – Effective April 1, 2012 Indiana OSW permits will no longer require an authorizing
signature. The signature line has been removed on the M-233P to facilitate this transition. The driver may
now legally display the permit by showing either:
1. A printed copy of your M-233P, or
2. A PDF format digital document on an electronic device: smartphone, laptop, tablet or other suitable
device capable of clearly displaying the PDF of your M-233P
The driver must continue to keep the M204 (general provision sheet) in the truck. The M204 may be kept
as a paper copy or as a PDF on an electronic device.
