support districts in developing transition plans designed to improve postsecondary outcomes for students with disabilities.
You will need to bring:
 1 student’s ETR/IEP age 14
or older from your district
 Any documents developed
in assisting the student &
family planning for postsecondary goals
 Middle or High School
course of study
