It is our immense pleasure to invite you and your colleagues to our Tenth Annual Heart
Failure Holiday Symposium December 5-6, 2014, at Northwestern Memorial Hospital’s
Conference Center in Chicago.
Learning Objectives
At the conclusion of this activity, participants will be able to:
Now in its tenth year, this symposium has become one of the “must-attend” meetings in heart
failure. Critical updates and thoughtful contemporary reviews delivered by national experts in
the field have been the hallmark of this program. This year we will once again assemble leading
cardiologists and cardiac surgeons with expertise in heart failure and cardiac surgery to provide
updates on the very latest scientific and technological advances for managing patients with heart
failure. We have invited several of our highest rated and favorite faculty alums from the past
ten years to participate in this year’s program. This “Who’s Who” in heart failure will undoubtedly
provide perspectives second to none on the progress in the field over the past ten years.
Following the success of last year, an expanded moderated poster session for cardiology
fellows interested in heart failure will serve as a kick off for this 2 day symposium. Meeting
tuition and travel expenses will be awarded to the fellows in training with the best posters.
Additionally, a special session addressing patient-centric outcomes in heart failure management,
which will include multi-disciplinary experts from across the country, will take place on Saturday
morning. This session will explore the psychosocial and cognitive challenges associated with
heart failure, including readmissions, as well as address approaches to shared decision making
along the continuum of heart failure.
Highlights of the 2014 Heart Failure Symposium will include:
• The public health problem of heart failure
• Prevention of heart failure
• The increased awareness of the intersection of pulmonary hypertension and heart failure
• Appropriate use of surgical interventions for patients with advanced heart failure
• Updates on the management of specific cardiomyopathies
• Atrial fibrillation and heart failure
• Imaging technologies to diagnose and monitor heart failure
• Case based presentations
• Ample time for vigorous panel discussions and audience Q & A periods
We have reserved The Signature Room at the 95th® located in the John Hancock building, with its
beautiful view of the Chicago skyline, for a reception on Friday evening. There are few December
venues more stunning than The Signature Room. This has become a favorite amongst participants
and will provide the opportunity to network with conference faculty and participants.

Participate in the diagnosis, evaluation and management of complex heart failure syndromes through case studies including pulmonary hypertension, arrhythmia, device therapies (CRT/ICD) and diuretic resistance
7.
Discuss imaging approaches that are available to diagnose, treat and follow patients with heart failure
8.
Describe future directions for the treatment of heart failure and opportunities for research
Target Audience
Faculty Disclosure
This continuing medical education program is designed to
meet the educational needs of cardiologists, cardiothoracic
surgeons, family practice, internal medicine, hospitalists,
advanced practice nurses, registered nurses, physician
assistants, pharmacists, cardiology and cardiac surgery
fellows and residents in training involved in the diagnosis
and management of adult patients with heart failure or at
risk for heart failure.
Current guidelines state that participants in CME and
CE activities should be made aware of any affiliation
or financial interest that may affect a speaker’s
presentation. Speakers are required to disclose any
significant relationships with commercial companies prior
to the symposium. The course syllabus will identify any
significant relationships and whether a faculty member
will be discussing off-label usage.

The Heart Failure Symposium invites you
to submit abstracts for consideration in the
poster session during breakfast on Friday,
December 5, 2014. Abstracts previously
presented at other national or international
meetings will be accepted. Accepted abstracts
will be available via the online syllabus. The
deadline for submission is Monday, October 20,
2014. Notifications for abstract acceptance
will be announced Monday, October 27,
