General information
We advise you to read the Sponsor’s Guide before completing this form. Note that the words sponsor and guarantor are synonymous.
As a sponsor, you are responsible for completing and signing this form. You must send us all requested documents in the format
indicated or else your application will be returned to you.
Steps to follow
1. Gather all required documents and supporting items (originals or photocopies) for all parties involved (sponsor, cosignatory and sponsored
persons).
2. In the column on the right side of the form, check the boxes corresponding to the documents that you are submitting.
3. Enclose payment of the applicable fee.

Translation
If you are submitting documents or parts of documents in a language other than French or English:
1. You must have these documents or parts of documents translated by an approved translator such as a member of a professional
corporation (certified translator) or a person authorized to act professionally as such. You may consult the directory of members of
the Ordre des traducteurs, terminologues et interprètes agréés du Québec at www.ottiaq.org. Translations done by any other person will
be refused.
2. You must provide:
• the original of the translation
– The translation must be done by an approved translator (see above) and include his or her name and seal.
– The text, seals and signatures of the original document must be translated.
• the document in its original language, in the format indicated in this form (original, certified true copy or photocopy).
Important
• The documents that you submit will not be returned to you. You are advised to keep photocopies of all your documents.
• If you do not submit the requested documents, your application will be returned to you.
• If you submit false documents or give information that you know to be misleading, your application will be rejected.
• The originals of documents you submitted may be requested at any time during the processing of your application.
