12th Annual LGBTQA History Month Dinner October 30, 2014 The Cornhusker Hotel, 6:30 pm 346 Nebraska Union Lincoln, NE 68588-0453 Phone: 402.472.1652 Fax: 402.472.8140 Dear LGBTQA+ Resource Center Supporters, On behalf of the LGBTQA Resource Center and the UNL Committee for GLBT Concerns, we would like to formally invite you and/or representatives from your business, organization or department to attend UNL’s 12th Annual LGBTQA History Month Dinner. The dinner will be held Thursday, October 30th, at The Cornhusker Hotel. Please complete the following co-sponsorship form and reserve your table by October 2nd to guarantee inclusion in the program! Group tables: Please indicate whether you would like a whole (10 seats) or half table (5). Advertising: We have reduced rates on program ads with the purchase of a table. For example, the price to purchase a ¼ page ad is $60 without a table purchase or $35.00 with a table purchase! Individual tickets: Please complete this section if you want individual tickets or need more than 10 seats but fewer than 15. For example, if you have 12 people attending the dinner, you can request 2 half tables with an extra seat at each table. Entrée Options: There are three entrée options: pecan-crusted chicken, sirloin beef, or a mixed vegetable grill. Please indicate the total number of each that you require. All entrees are being prepared gluten-free, and the vegetarian option is both gluten-free and vegan. Let us know of any special accommodations that need to be made for an individual (e.g. nut or strawberry allergy, etc.). Each entrée will be served with a salad and a dessert. A fruit cup will be provided for dessert upon request. Guests: To streamline the check-in process on the evening of the dinner, we ask for each guest’s name and their entrée choice. We ask for email addresses so we can send dinner details the week before. Otherwise we send dinner details to the contact person and ask that it be shared with attendees. Donated Seats: If you have extra seats that will not be used by someone from your organization, please let us know either on the form or contact us by email or phone by October 24th. When a t t e n d e e s a r r i v e , we provide the appropriate meal ticket when they will tell us their name. If you do not know all of the names of the individuals sitting at your table, leave it blank and we will follow up with the contact person a week before the dinner. If you need to make a substitute, let us know! Please send an email t o [email protected] or we will contact you a week before the dinner if we don’t have all the information requested. We ask that all guests are confirmed by October 2nd.. If a guest is unable to make the dinner or you have a substitution after October 2nd, please call us at 402-472-1652. The alternate attendee can also tell us which table they are at and whose ticket they will be using. We will also try to fill any unused seats so appreciate your letting us know if someone cannot attend. The Cornhusker Hotel is located at 333 South 13th Street in Lincoln, NE. The dinner will be in the Grand Ballroom. Raffle tickets will be sold for prize drawings. Thank you!!
