October 14, 2014 A special message for my patients After careful consideration I have decided to join the team at Hannam Fertility Centre. I will therefore be closing my office at First Steps and relocating my practice to 160 Bloor St East, Toronto, effective immediately. If you choose to have me continue to provide care to you at my new practice location, I will take all steps necessary to make this transition as smooth as possible. Orla Doyle from Hannam Fertility Centre can answer any questions you may have. I have included Orla’s contact details below and please feel free to initiate contact with her by phone or email. Your electronic medical record (EMR) including results of investigations, laboratory reports, and consultation letters, will remain at First Steps Fertility and be available for other consultants should you choose not to re-locate your care to my new practice location. However, at your request, a copy of your EMR will be transferred by First Steps to Hannam Fertility Centre to facilitate your ongoing care with me. Please note that if you are a patient active in a treatment cycle, you should complete your current cycle at First Steps Fertility. If you then choose to transfer care to Hannam Fertility Centre following completion, contact Orla with your next day one. (416-595-1521 ext 1208).
