Registration opens at 12:30 p.m. If you need a form on which to record

DSAG is a 501(c)(3) organization and your donation
is completely tax deductible. Donations can be
made on behalf of groups that register online.
Waiver: In consideration of me and/or my minor child being permitted to participate in the Buddy Walk, I hereby—for myself, my heirs, and personal representatives— assume any and
all risks which might be associated with the event. I further waive, release, discharge, and covenant not to sue the Down Syndrome Awareness Group of East Tennessee (DSAG) or the
National Down Syndrome Society (NDSS), its officers, employees, sponsors, organizers, volunteers, or other representatives or their successors and assigns, for any and all injuries or
damages of any kind whatsoever suffered by myself and/or my minor child as a result of taking part in the events and any related activities. I also authorize DSAG and NDSS the use of
any photo, film, or videotape taken of me or my minor child at the event for any purpose.
Expiration
Each individual with Down syndrome will receive a free Buddy Walk t-shirt. In addition, families that raise a minimum of $100 will receive up to 4 free Buddy Walk t-shirts.
Note: Free family t-shirts can only be distributed ONCE per individual with Down syndrome. Other family members or walkers may purchase t-shirts at the Buddy Walk marketplace.

About the Buddy Walk
The Buddy Walk® was developed by the National Down Syndrome
Society in 1995 to celebrate Down Syndrome Awareness Month
in October and to promote acceptance and inclusion of people
with Down syndrome. The Buddy Walk has grown from 17 walks
in 1995 to more than 250 walks planned for 2014 worldwide.
Last year, over $12 million was raised nationwide to benefit local
programs and services, as well as the national advocacy and
public awareness initiatives of NDSS that benefit all individuals
with Down syndrome.
About DSAG
Through education, research, and advocacy, the Down Syndrome
Awareness Group of East Tennessee (DSAG) works to ensure
that people with Down syndrome have the opportunity to achieve
their full potential. Incorporated in 1991, DSAG is a parent-led
organization that serves an 18-county area. The groups provides
information and support to families, professionals, and the
community in order to enhance the quality of life for individuals
with Down syndrome. DSAG of East Tennessee is an affiliate
of the National Down Syndrome Society.

Register Online for Easy Fundraising
Buddy Walk participants are encouraged to use the
Buddy Walk website to register online. Registration is
free, easy and completely secure. Customize your site
with photos of your buddy and a personal message
to your supporters. Set your team’s goal and watch
the donations come in! Fundraising begins as soon
as you set up your site and begin sending out the
link to family and friends. Fundraising for Buddy
Walk 2013 ends and the site will be shut down
October 31, 2014.

Wearables
This year’s Buddy Walk will feature a variety of
activities for children of all ages. Carnival-type
foods will be available for a reasonable cost. A
New Family Tent will be available for new parents
of children with Down syndrome. Also enjoy
entertainment and a silent auction.
Each team may order custom team t-shirts in a wide
variety of colors.
Medal Ceremony
3:30 pm
Medals will be available to all pre-registered
individuals with Down syndrome.
In order to ensure participation in the Medal
Ceremony you must pre-register for the
Buddy Walk.
Pledge Prizes/Group Registration
Every family/group will be eligible for prizes for
fund raising. The top groups will be awarded
gift cards as follows:
1st prize: $500
2nd prize: $400
3rd prize: $300
4th prize: $200

DSAG Dollars will be
awarded to groups that raise
funds for the dollar amounts
of: $100, $250, $500, and
$1000. DSAG Dollars can

Each team may order new canvas wristbands
customizable with text, logos and photos. The cost is
$5 per bracelet, and $2 of each band purchased goes
back to DSAG!
