Student VTEA Form
Students enrolled in following courses need to fill out VTEA form once per semester. A significant portion of ELAC funding is
based on the collection of VTEA Form. Please use the form link below. The default 4 digit PIN is student's birth month birth
day (MMDD). It should take no more than 5 minutes to fill out.
A sample form can be found here.
To start the VTEA form, click the link below. It opens 7am to midnight, 7 days a week.

Q and A
Question: I cannot log into the form. What should I do?
Answer: Can you log into SIS? If you can, use the same PIN for the form. If you cannot, go to Admissions to reset PIN.
Question: How do I know if I have completed the form for the current semester?
Answer: Start the form. If the title says "Please create a new survey", it means you have not done the form yet. If the
title says "Please edit your survey", it means you have done the form for the selected semester.

Question: How to find out what my student ID is?
Answer: Go to SIS and enter your Social Security # and 4 digit PIN (MMDD), it will display your student ID.
