INSTRUCTIONS FOR ORDERING FOOD at Odyssey Charter School
1. Please complete a breakfast and/or lunch menu for EACH STUDENT for the entire month:
a. Write student’s FIRST and LAST name.
b. Write student’s homeroom teacher’s name.
c. Write student’s homeroom room number.
2. Circle the days that the student wants to order food. (Cannot order parts of meals. Full meals
only. The only a la carte items are daily milk or Friday pizza slices.)
3. If ordering lunch on a Friday:
a. Please circle “Full Lunch” for a complete lunch including 1 slice of pizza,
OR
b. “Pizza Only” if you only want to order pizza alone.
c. Fill in the TOTAL number of slices requested. (Full lunch comes with 1 slice. Additional
slices are $1.00 each.)
4. Return the menu(s) to the HOMEROOM TEACHER(s) by the due date in order to receive food in
the following month.
Example: The below student is named Joe Student. He is in Mrs. Teach’s room, in room number
101. He ordered lunch on Nov. 7th, 12th, 18th, 19th, and 21st. On Friday the 7th, Joe will get a full
lunch, including salad, fruit, milk, and two (2) slices of pizza for $2.95 + $1.00 for a total of $3.95.
On Friday the 21st, Joe will get 3 slices of pizza only for $3.00. Joe gave the below menu to Mrs.
Teach by October 29th. Joe’s sister Sue will fill out a separate menu and give it to her homeroom
teacher.
