Maximising independence Enliven is experienced in supporting people with a range of complex needs, including 24 hours per day/seven days per week support. We work with the caregivers your client chooses, our own support workers, or a combination of both. We can attend pre-discharge meetings to get to know the individual, and arrange for our support workers to be part of the discharge plan and training. Enliven has contracts with the Ministry of Health and District Health Boards This means that if the person with the injury is already receiving support at home from Enliven then another provider doesn’t need to be involved. We already know the person, their lifestyle and environment and what support they required before their injury. Supporting them is a team of Resource Coordinators who make sure support is in the right place at the right time. We also understand that many people with injuries want to choose their own support person, and we can work with this. Enliven is experienced in supporting people achieve the best possible outcomes. Avoiding hospital admission Enliven contacts all clients who have been referred for an Interim Support Package within four hours of discharge. Services commence within 24 hours. We work with clients to set goals and develop a plan that gets them back to their pre-injury state. Where this can’t be achieved, our service coordinators are quick to recognise any complications or need for additional services, and will contact ACC.
