Return to independence
An initial visit is completed by a health professional
and services are in place within 48 hours of referral.
Each client’s individual plan focuses on maximising
independence. The Enliven team is experienced in
helping clients set their own goals, whether short term
or long term rehabilitation is required.
Maximising independence
Enliven is experienced in supporting people with a range
of complex needs, including 24 hours per day/seven days
per week support. We work with the caregivers your client
chooses, our own support workers, or a combination of
both. We can attend pre-discharge meetings to get to
know the individual, and arrange for our support workers
to be part of the discharge plan and training. Enliven
will work in partnership with the injured person and their
family to achieve the best outcomes for them.

lead provider that does not subcontract – a referral to

Enliven knows about goal setting and
maximising independence
Enliven pioneered restorative home-based support
in New Zealand in 2004. Since then, we have been
working alongside people, supporting them as they set
goals to maximise their independence. This approach
means Enliven had health professionals on its team
before any other home-based providers.

Enliven consistently delivers inside ACC’s contract
requirements for the time taken between referral and
the first visit. You can trust Enliven to see that the person
who needs help will get it promptly.

Enliven has contracts with the Ministry of
Health and District Health Boards
This means that if the person with the injury is already
receiving support at home from Enliven then another
provider doesn’t need to be involved. We already know
the person, their lifestyle and environment and what
support they required before their injury.

Enliven has a trained team
Enliven’s trained Support Workers are supervised by health
professionals in the role of Service Coordinators. Supporting
them is a team of Resource Coordinators who make sure
support is in the right place at the right time. We also
understand that many people with injuries want to choose
their own support person, and we can work with this.
Enliven is experienced in supporting people achieve the
best possible outcomes.
Avoiding hospital admission
Enliven contacts all clients who have been referred for an
Interim Support Package within four hours of discharge.
Services commence within 24 hours. We work with clients
to set goals and develop a plan that gets them back to their
pre-injury state. Where this can’t be achieved, our service
coordinators are quick to recognise any complications or
need for additional services, and will contact ACC.
