FRITHWOOD SURGERY PPG LOTTERY CLUB RULES
1
The ‘Lottery’ will operate as a Small Lottery as defined by the Betting and Gaming Act 2005. Participants must by over the age
of 16 years and registered as a patient at Frithwood Surgery. Members must complete an application form and have read and
agreed the club rules.
2 The object of the ‘club’ is to raise funds for Frithwood Surgery to purchase equipment and/or services not supplied by the NHS.
3 The ‘lottery’ shall be administered by a sub-committee of the PPG consisting of two Trustees, a representative of the Surgery
and an administrator. The administrator will keep records of the participants and subscriptions and notify the winners. A PPG
Lottery Sub-Committee member will be present at the draw. The draw will take place on the first working day of each month,
commencing in January 2015, at Frithwood Surgery in the presence of the staff and persons/patients in the surgery at the time
of the draw.
4 The lottery will operate on an annual basis, the subscription being £24 payable in advance by cheque. This will give each
member one number which will be placed into the monthly draw providing that funds are cleared through the member’s bank
account. This year’s annual subscription deadline is 12th December 2014.
5 Unless otherwise advised a member will be deemed to have left the club if their subscription renewal remains unpaid by the
first week in December at the end of the lottery year. Members wishing to leave the club will do so at the end of each year and
should notify the club of termination.
6 A register will be kept to record the details of each member and the number(s) allocated.
7 The prizes consist of 50% of one twelfth portion of the income each month
First prize 50% of the monthly prize fund (£50 if 100 members - £100 if 200 members)
Second Prize 30% of the monthly prize fund (£30 if 100 members - £60 if 200 members)
Third Prize 20% of the monthly prize fund (£20) if 100 members - £40 if 200 members)
Prize winners will be notified in writing, by telephone or email.
8 The administrators will make all reasonable efforts to pay or post prizes to the winners. Notice of prize winning numbers will be
posted on the Frithwood Surgery website, on the noticeboard in the surgery and in the Newsletter. Any unclaimed prizes will
be placed into the Frithwood Surgery PPG funds if unclaimed after a period of 6 months following the draw had elapsed.
9 The decision of the Frithwood PPG Lottery Fund Committee shall be final in all matters requiring adjudication.
