This document provides all the information you will need before submitting your application. You must provide sufficient logbook evidence for all privileges you are applying for. Details are included in the guidance document. 2: The Australian Statutory Declaration should be completed as shown in the example in the guidance document. When completed correctly, this covers the requirement for certification of the documents required for this application. 3: Since 2008, you must meet the ICAO English Language Proficiency standards, as per CAO 40.0. before CASA can issue a flight crew licence. Please indicate in Section A5 the option that applies to you. 4: CAR 5.09(1)(c) states that CASA must ensure an applicant is a fit and proper person before issuing a permission. Note that an ASIC is current for a 2 year period; an aviation security check, required for an AVID, is valid for 5 years; the AFP security check is only valid for a period of 3 months. Therefore the information requested in Section A6 is still required, even if you hold a valid ASIC or AVID. Please provide details in Section A6 of any regulatory action, traffic and criminal charges, convictions or imprisonment. Action includes, but is not limited to, suspension and cancellation of any aviation authorisation and includes Australian and overseas actions, charges, convictions or imprisonment. 5: CASA requires verification of your New Zealand flight crew licence before we can transfer your qualification. You need to submit Form CAA602 for a flight crew licence verification to CAA New Zealand before submitting this form to CASA. If CASA does not have a CAA New Zealand Licence Verification Report, your application will be returned as non-accepted. 6: Payment for this application can be made online. Go to the CASA webpage and click the Payment button. You must attach a copy of the receipt with this application. 7: This form can be completed electronically and saved locally to your computer. Once the ARN has been entered into the first page, it will automatically update in the applicant ARN fields on subsequent pages. CASA recommends that applicants complete as much of the form electronically as possible. 8: Please ensure your application and the checklist are completed correctly and that all required supporting documentation is provided. Incomplete applications will not be accepted and may be returned to you for amendment. Fields and sections marked with an * are mandatory.
