The deposit is used to secure the return of the rental equipment in good condition. If the equipment rented is not returned within 5 days from the return date specified on the rental agreement form you will be charged the replacement cost and this will be charged from your deposit.(Please Initial_______) DUE DATES and LATE FEES All equipment is due on the date specified on the Rental Agreement Form, and must be returned by that due date. A fee of $20 per each additional day past the rental agreement date will be charged upon late return of any equipment for up to 5 days. If equipment is not returned within 5 days from the date specified on the rental agreement form you will be charged the replacement cost of ALL the equipment not returned and this cost will be calculated and deducted from your cash deposit. (Please initial_____) DAMAGE AND REPLACEMENT FEE If any part of equipment is damaged, lost, and/or rendered permanently unusable, the charge will be equivalent to the replacement value of the entire item. If a part is broken or lost and can be replaced, the cost of the part will be assigned as a damage fee. Renters will be notified immediately upon returning equipment if there will be a fee assess and fees will be deducted from your cash deposit (Please initial_____) RELEASE OF CLAIMS I understand that the use of recreational, outdoor rental equipment involves risk, including the risk of injuries, illnesses, and even death. I assume these risks and release any claims that I or anyone I allow to use this equipment may have against OV Cornhole for any injuries or illnesses that I may suffer as a result of using OV Cornhole’s recreational equipment.
