This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee
is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) This
charge includes, but is not limited to availability of staff 24 hours a day, staff and facilities to respond to initial requests for
services, consultation with responsible party to determine services desired. Coordinating service plans with cemetery,
crematory and/or other parties involved in final disposition of the deceased. Filing of death certificate and obtaining permits

Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select
certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to
choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other
requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement
we provide describing the funeral goods and services you selected.
This list does not include prices for certain items that you may ask us to buy for you, such as newspaper notices, minister
honorariums, cemetery fees or flowers. The prices for those items will be shown on The Statement of Funeral Goods and Services
Selected. We charge you for our services in buying these items.

Our charge for forwarding of remains to another funeral home includes our staff’s transportation of the deceased to the
funeral home (within a 30 mile radius); basic services of funeral director and staff; embalming or other preparation of
remains, if relevant; consultation with responsible party; coordinating arrangements with receiving funeral home; clerical and
administrative services in preparation and filing of the death certificate and necessary permits; assistance with forms for
social security and or veterans benefits, if needed; online obituary placement for a 12 month period, if requested; dressing
and casketing of the body as requested; air tray unit (required by Airlines), and transportation of the casket by service van to
local airport, and TSA Certified Cargo Screening by third party facility if shipping by air (Does not include casket selection,
state filing fee, certified copies of the death certificate or the actual air freight charges by the airlines)
Receiving Remains From Another Funeral Home

Our charge for receiving remains from another funeral home includes the basic services of funeral director and staff;
coordinating the details with the other funeral home, family and cemetery involved; online obituary placement for a 12 month
period, if requested; transportation by service van of casket from airport to our facility or directly to cemetery as requested;
temporary housing of the body at our facility, if needed. (Does not include cemetery charges, air freight charges, viewing of
the body or funeral ceremonies)
Direct Cremation (Without viewing of the body, formal services or ceremony)
Prices range from $1,079. to $9,838.
Our charge for a direct cremation includes our staff’s transportation of the deceased to the funeral home (within a 30 mile
radius); use of our refrigeration facility for up to 5 days; consultation with responsible party to determine services desired;
clerical and administrative services in preparation and filing of the death certificate and necessary permits; assistance with
forms for social security and or veterans benefits, if needed; online obituary placement for a 12 month period, if requested;
and the actual cremation process of the body. For your convenience or ceremonial requirements the ashes will usually be
returned within 3 business days of receipt of body and necessary authorizations, excluding weekends or holidays.
If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and
can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we
provide are cardboard. However, other cremation caskets are available. A complete price list will be provided at the funeral
home.

Anatomical Donation to Medical School

Our charge for anatomical donation of the body to the medical school includes our staff’s transportation of the deceased to
the funeral home (within a 30 mile radius); the basic services of funeral director and staff; embalming or other preparation of
remains, if relevant; (the Medical School may require embalming); or refrigeration and housing of the body until acceptance
by medical school, consultation with responsible party; clerical and administrative services in preparation and filing of the
death certificate and necessary permits; assistance with forms for social security and or veterans benefits, if needed; online
obituary placement for a 12 month period, if requested; and transportation of the body by service van to the medical school.
