ESPP Enrollment Instructions New ESPP Participants ESPP Election Form (included in your New Hire Packet) – Please note:  This form does not automatically enter your ESPP elections on the Company’s ESPP plan administrator’s website (UBS). The form merely serves as confirmation of your intent if your position will need to be coded as an ‘insider’.  The form allows you to enroll during a blackout period if you are deemed to be an ‘insider’ (one who will have access to privileged information about the company’s products and/or financial information).  You will need to create an account on the UBS website (see instructions below). If you start employment with Varian on a Monday, you should be able to access your UBS One Source account on Wednesday of that week. (The employee eligibility file goes to UBS on Tuesday nights.)  Upon logging into the UBS One Source website for the first time, you will need to complete the online U.S. Account Authorization Form (W-9). Should you wish to enroll in the next ESPP offering period, you must enroll online via the UBS One Source website to enter your election before the open enrollment window closes. Enter your ID number (this is the number on your badge below your name) Click ‘Submit’. 04/2013 2 **************************************************************************************** The next screen requires confirmation of the ID number you entered; if correct, click yes: **************************************************************************************** You will then be asked a Security Question. The screenshot following is an example of a security question. This question may be asked again when you access your account or sell your stock. Please continue to complete each screen to move through the on-line registration process. A temporary PIN will be created for you in real-time. You will then use your badge number as your UBS log-on ID and your temporary PIN to log-in to the UBS One Source website. If the PIN does not generate through this link, please call the UBS Call Center at 1-877-752-8276 and a representative will be able to assist you. Please Note: A ‘Welcome Kit’ will NOT be mailed to you. If you get this message, please call the UBS Help Desk and ask the representative to assist you in establishing your password. 04/2013 3 Terms to Note: “Carrying Forward Your Contribution Election”: If you want to continue your participation in future ESPP offering periods automatically, you can make this election by checking this box. • If you do not check the “Carry Forward” box, you will not have payroll deductions taken for future offering periods. • Once you check this box, you will be enrolled in future ESPP offering periods unless or until you uncheck this box. “Stop and Hold”: “Stop and Hold” means that you want to temporarily freeze your contributions in the current offering period. These funds will be held until the end of the offering period and then used to purchase shares. Please note that once you make this election to stop and hold, you cannot change it. “Stop and Refund”: “Stop and Refund” means that you want to end your participation in the current offering period and have your money refunded to you.
