2015 SCHOLARSHIP APPLICATION
DIRECTIONS AND LIMITATIONS
All applicants must submit the following information for the application to be considered complete.
Any application missing information or documentation will not be considered or judged.
1. Photocopies (front and back) of both the applicant’s and the sponsor’s current uniform
service identification card.
2. An essay, written by the applicant, typewritten in 12 point font, double spaced, and limited to
200-300 words (one page).
3. Pages 1-3 of the scholarship application.
4. The certification page must be signed and dated by both the applicant and the sponsor. If
the sponsor is deceased, students should have the surviving parent sign the form.
5. Application and all required documents must be mailed to the Scholarship Committee with a
postmark no later than March 2, 2014.
6. Recommendation letter
7. School representative section signed and completed.

Selection & Notification Procedures
Scholarships will be awarded to the best qualified applicants as determined by the impartial judges
of the WPOSC Scholarship Committee.
Applicants will be notified of the Committee’s decisions no later than April 1, 2015. The presentation
of the scholarships will be made during an award ceremony on May 14, 2015. Scholarship recipients
will be given further details upon notification of the award decision.
Funding Procedures
Scholarship funds may be used for tuition and fees only. A full tuition scholarship is defined as one
which pays all tuition, fee’s, room and board. An acceptance form must be completed and signed
by the recipient prior to release of the funds to the college, university, community college, vocational
or trade school to be attended. All scholarship funds will be sent directly to the school for credit to
the student’s account. If the applicant fails to enroll, attend, or receives a full scholarship from
another source, the school will return the funds to the WPOSC Scholarship Committee. Returned
funds may be awarded to alternate scholarship recipients.

CERTIFICATION LETTER
I certify that all information in this application is accurate to the best of my knowledge and
the essay is entirely my own effort. If I accept a Wright Patterson Officers’ Spouses' Club
Scholarship (WPOSC), I further certify I will abide by the following conditions:
1. Reviewed the eligibility demands and meet the requirements.
2. Scholarship funds are to be used for undergraduate studies within the 2014-2015
academic year. Scholarship funds are to be applied to tuition and fees only. Any excess
funds must be returned to the WPOSC.
3. If I am selected as a WPOSC scholarship recipient, I will return the Scholarship
Acceptance Agreement no later than June 1, 2015. I will be required to provide the
name and address of the accredited college, university, or technical school I will attend
for the 2015 fall term. After June 13, 2015 any unclaimed scholarship funds revert to the
WPOSC Scholarship Fund and may be reallocated to an alternate winner.
4. If I accept an appointment to a military service academy or receive a full ride scholarship
I will become ineligible for the WPOSC scholarship award.
5. I am a responsible citizen in good standing in the school and the community.
6. It is my responsibility to notify the WPOSC Scholarship Committee of any change of status
(e.g., change of schools, change in address, etc.). Failure to do so may result in the
forfeiture of my scholarship award.
7. If the above conditions are violated, the scholarship award funds must be returned to the
WPOSC Scholarship fund.
