Fall Injury Prevention By Proper Screening D izziness and vertigo are among the top three reasons why patients visit a physician. Back pain and headaches comprise the other two reasons. Symptoms such as dizziness, vertigo and imbalance actually are estimated to affect 40% of patients 40 years of age and older. Of those age 65 years or older, 25% are estimated to suffer a fall and thus a fall related injury. Falling is typically a direct consequence of dizziness, primarily in this group of elder individuals and risk of falling can increase considerably when those individuals have other problems such as high blood pressure or diabetes, or any of the gamut of neurologic defects such as Parkinson's. A sample report of US emergency room visits, from 1995-2004, confirmed that vertigo and dizziness were indeed high on the list of reported presenting complaints as reason for reporting to the emergency room. Data from the National Health and Nutrition Examination Survey demonstrated that individuals with symptomatic vestibular dysfunction have a 12-fold increase in the probability of incurring a fall. It has also been associated that more frequent medical consultation, sick leave, and interruption of daily activities in general, have been related to dizziness symptoms, particularly vestibular vertigo. Medical research on ill health is commonly disease focused as opposed to symptoms related. This generalized approach may actually underestimate the reality of such common symptoms as vertigo and dizziness, which as mentioned, rank high as patient complaints in primary care, yet evade accurate diagnosis and remain unexplained in 80% of cases. Making matters worse, even though symptoms of vertigo and dizziness can be associated with a wide variety of other conditions, all of which necessitate a multidisciplinary approach to management, said dizziness symptoms rarely prompt appropriate referral to a qualified specialist for proper evaluation. The most frequent conditions which cause such symptoms as vertigo and dizziness for the most part remain largely undiagnosed outside of specialty clinics such as my own. Because of the substantial risk of injury and the resultant decline in independence and/or quality of life following a fall, predicting who is at risk is most helpful, and for those who reduce that risk through a fall prevention program, falls and otherwise impending fall related injuries may be prevented. Balance is not a single physiologic function. The sensory inputs for balance include vision, vestibular, and proprioceptive feedback. While a person is walking, the brain must instantaneously integrate this information and execute appropriate motor planning. This function must be supported by an adequate neuromusculoskeletal system. All of these factors change with age. Further, any disease related decline in any of these systems further impairs balance. Bilateral vestibular loss is a significant contributor in 25% of elderly patients with imbalance. Untreated vertigo is a severe risk factor for falling as well. The act of walking has more advantages than most people realize and yet most of us just can't seem to find enough time in each day to walk a little more. Walking is safe and it's easy. This low-impact form of exercise has many more benefits than you would first think. Here are some of the benefits of including a regular walk into your week. Weight Loss Most people associate weight loss with several grueling hours at the gym pushing weights or running on treadmills. What these people should understand is that taking a walk through the park with your dog or with a friend can help with your lose weight quickly and easily. The cold dry winter air can take its toll on your skin, taking away all oils and water leaving your skin unprotected. This is when you experience the pain of itchy, cracked, irritated skin. Having this happen to you is not only painful - it can make you more susceptible to infection. Fortunately for you there are various steps you can take to avoid all this keeping your skin soft, moisturized, and protected from the icy cold air. My advice is to take things slowly, build your strength gradually and after a few weeks you will begin to reap your reward. Will contests (this is the legal term used to describe a family member’s challenge to the contents of a will) can be based on one or more of these claims: • The will was not properly executed • The will maker was under improper or undue influence from a beneficiary • The will maker or another person committed fraud • The will maker lacked the mental capacity to make the will help of an estate planning attorney will also ensure that your will is a properly executed and valid legal document. A will is a public document once it is filed in probate court, and the public nature of the document can give rise to disputes and will contests. In contrast, a revocable living trust is a personal and private document that does not have to be filed as a public record. Furthermore, lifetime trusts can be used to provide financially for “troublesome” beneficiaries who might otherwise spend through their inheritance. Lifetime trusts are flexible and can link financial inheritance to the accomplishment of goals that you set forth in the trust documents. Obtain qualified legal advice regarding your estate plan. Estate planning has become a popular “do it yourself” legal task, but you should at least consider having your will reviewed – if not written – by a qualified estate planning lawyer. Writing your will with the Write your will independently. To avoid claims of undue influence after your death, make sure you write your will in circumstances that are clearly free from interference by family members or other beneficiaries. Avoid having beneficiaries serve as witnesses, for example, and don’t allow beneficiaries to attend your meetings with your estate planning attorney. At the time you write and sign your will, you can ask your physician to perform a physical examination and certify that you are mentally competent to execute your will. Another option is for your attorney to ask you a series of questions before you sign your will and document that the questions were asked and answered. It may also be a good idea to make a video recording of the process of signing your will, as another way to prove mental competency. Answer your family’s questions. Consider sharing your intentions with your family and other beneficiaries. If you explain the reasons for the decisions you made regarding bequests, you may help prevent will contests after your death. Instead or in addition, you may write a letter to your beneficiaries that will be read at the same time your will is read. Keep your will dust-free. Once your Last Will and Testament and other estate planning documents are complete, don’t just file and forget them. Review your will with an attorney at least once a year and make any necessary changes in a timely manner. GenX'er: What would you think of giving your aging parents a gift that costs you absolutely nothing except a little more of your time? Would that fit your budget? More importantly, would it fit your lifestyle? - What, from among all your many commitments and over-commitments, could you cut back in order to free up extra time on a regular basis for the two people whose time is far more limited than even yours (over the long stretch)? What is it, you ask. Are you sure you're ready? It's a commitment on your part - signed, sealed, and delivered... not just some flimsily-wrapped agreement - to slow down with, around, and for your aging parents. - How much would you lose (versus how much could you gain) if you dropped just one outside involvement in order to get more involved with your aging parents? (Remember, spending your time with them fidgeting and looking at your watch as if something more pressing were waiting doesn't count!) Slow Down! Sure, your busy midlife existence runs at high speed and overdrive every minute you're not sleeping - and you probably give that sleeping part far less time than it deserves. This season, though, try giving yourself AND your aging parents the gift of your slowing down - even slightly. Consider it an exercise in seeing how much you've been missing. Don't jump at this too quickly. I mean, you wouldn't want to overdo. Just think for a moment, though: - When was the last time you told your elderly mother or father to take as long as they needed because you'd be happy to wait... and then you WERE happy to wait? - What kind of response do you imagine you'd get from your elderly parents if you said there was no rush, that you had all the time in the world - for them... and then took off your coat and settled in for whatever it was they had in mind? - How would your aging parents respond if you showed up out of the blue to take them for a long, slow drive to someplace - or to see someone - you never seem to have time to visit? The astonishment factor would probably be enough to create something cataclysmic! And, wouldn't that be great?! Do you see where I'm going here? Shifting gear matters - to both of you! Life moves far too quickly without our speeding it up unnecessarily. And, believe it or not, it's going even faster for your aging parents. I know, you think they're slowing down.. and they are.. on the outside. But by their internal clocks, time is rushing by. They can't control it. They can just try to keep up. At this point in your life, you're just the opposite. Your internal clock is still keeping a slower, more steady beat. On the outside, though, the demands on your time keep you zipping at ever increasing speeds. The challenge for you, if you choose to accept it, my midlife friend, is to consciously shift gears and slow down enough to match your parents' external pace - even every so often. This gift to them would be incredible! Do you have it in you? I'm betting you do. And just think: In the long run, the rewards far outweigh the costs. What more could an adult child of aging parents ask the perfect gift to do? While I love the easy-breezy feel and long days, I'd get pangs of anxiety about having to reveal my vampire white, tree trunk-type legs and "bat wings" because of my skimpier wardrobe. Now that fall is here, I can better hide my imperfections under my curvy boot cut jeans and sweaters and not feel so painstakingly self-conscious. None of my friends would actually know that I worry about this, and truth be told I'd be kind of embarrassed to tell them. Objectively speaking, I probably come across as a relatively confident person who doesn't seem super-concerned about my appearance (e.g. I'm usually dressed casually without a lot of makeup and my hair thrown up in a bun). And I don't discuss weight that often, because I don't like to showcase my insecurities and flaws, but WOW, do I secretly get down in the dumps about the way my body looks and feels sometimes. In the grand scheme of things, I know that it doesn't matter whether you're a size 2 or 20 as long as you're healthy, but sometimes I wish that my body type was different. Whenever I see a woman with a long, lean body I get sort of envious about how great she looks in EVERYTHING, and annoyed that I'm naturally kind of short and stocky. There's only so much you can change with diet and exercise. Then I snap back to reality, scoff at my vanity and think about the 'big picture' of how lucky I am to be healthy. Yet despite this outlook, I still find myself muttering horrible things to myself about my appearance in the mirror. I didn't quite realize how self-critical I was being until my 5-year-old daughter started repeating my comments aloud (and in front of my husband, no less!). I was horrified, not just because of the bad example I've been setting for my daughter, but because she'll realize what a hypocrite I am. Since birth, I've been telling her how perfect she is and how she should love herself, while I'm busy tossing clothes around my bedroom screaming, "I'm too fat to wear this!" Recently, a Tumblr blog went viral that was a reminder to love and embrace your body, no matter what size you are, as long as you're healthy. Brittany Miles battled and overcame an eating disorder, and her path to self-acceptance and perspective on weight is refreshing. In celebration of her positive attitude about body image, I spoke with Maria Bucaro, author of Love Your Body Now! Lose The Weight of Self-Hate Gain the Yummy-ness of Goddessness about eight key steps you can take toward appreciating and respecting your body RIGHT NOW. tion," says Bucaro. The question can be, “What do I want to eat? Do I want another glass of wine? What kind of movement do I want to do today?' "This really works and it might take a little time to hear what the body is saying, mostly because that little voice that we all have is trying to override what the body is really saying. Be still and listen." 5. Physically love your body. Your body is filled with pleasure centers. We often focus on the pain, or what is feeling unpleasant. "Begin to focus on what feels good and your body will respond," suggests Bucaro. Rub your eyes, stroke your hands, caress your belly (yes, even the rolls!) or whatever feels good for you. At the same time, talk to your body, expressing gratitude for all that it does for you. you can support each other in feeling good." Focus on what you do want and you'll get more of that in your life. Bucaro just completed 100 days of celebrating loving her body along with some other women. She also has a Facebook group where you can get inspired and support each other in body image issues. 2. Assume responsibility for everything that is going on in your reality right now. When you do, then you have the power to alter your reality by changing your thoughts. No one's opinion matters. Only your opinion matters, which means you get to decide what is beautiful. You get to choose how you see yourself. "If you want to see yourself as beautiful, then consciously begin to see yourself as you want to be," advises Bucaro. "Begin creating the beauty you truly want by holding the intention of beauty, knowing that you are beautiful right now." 3. Be kind to yourself and your body. Treat yourself like you would treat your best friend. It is not about punishing yourself with physical exercise for the sake of losing weight. It is not about lifting weights and suffering. There is nothing wrong with lifting weights or going out for a run, if you're happy to do it. It's about what feels good and is joyful. Your body is craving movement, not punishment. Your body is craving love, not battle. Treat your body well. Explore what you love to do. For example, Bucaro loves to swim, ride her bike and take nature walks. Think about what turns you on and lights you up... and go do it. 1. Stop talking about dieting, getting "stuff" done to your body and all around complaining about how you don't like what you see in the mirror. Stop thinking about it, stop talking about it with other people and especially stop talking about it with yourself. "Talking about what you don't want just keeps getting you more of what you don't want," explains Bucaro. "Start talking about what you like about 4. Listen to your body. It knows what it wants. Have fun with your body and love it. In other words, lighten up! "This is something we do in the Love Your Body Now workshops and it's amazing how powerful this is," explains Bucaro. Put some music on, and look at what you don't like. Now turn it around and make it your friend. Big nose? Wrinkles? Fat? Weird toes? Dance with it, laugh with it and have fun. It's the only body you've got, so enjoy it and be kind! "If you want to take it to the next level, do it with a friend," suggests Bucaro. "BUT make sure it's a loving and supportive friend who understands what you're doing." 8. Gratitude. Think about all of the things that your body is doing right now, without you even having to think about it. Blood is being pumped and oxygenated, waste is being taken care of, food is being digested, breathing in and out without stopping, even when you're asleep (and so much more!). "Arms hug, legs carry you to loved ones, lips kiss and smiles can change a life," Bucaro reminds us. "Heart opens, hands hold and caress and comfort, eyes look into your loved ones soul. Ears hear the music of your baby's giggle and the music of a symphony. You're a part of that symphony of life. Illness and worse are just a few minor mistakes away. Take our five best tips for managing your pet's well being and make sure that you keep veterinary calls at a minimum this winter season. We're rounding the seasonal corner into the winter months. With the winter come the usual maladies like sniffles, flu and colds that are expected for humans; however, winter is especially a tough time for pets that spend any regular amount of time outside. How can an owner effectively protect their pet from the harsher climate without compromising normal behaviors such as regular walks and outdoor independence? Few people realize this, but winter has the same impact on our pets that it does to humans. A coat of fur can only do so much; precautions must be taken to protect the health and well being of your best friends in the chilly winter months ahead. To avoid a trip to the veterinarian, the following are some points and precautions a responsible pet owner should make for ensuring health in low temperatures. Age A pet's age is a definite deal-breaker on the extent and frequency that they should be allowed outdoors in cold weather. Especially young dogs and cats should not be kept outside for extended periods of time. Also, elderly pets are equally vulnerable in the cold. Young and old pets do not have the required body fat, metabolism speed, and fur to be properly protected in low temperatures; they should be kept inside for their own good. Clothing Pet clothing is adorable, right? A puppy wrapped up snug in a miniature Bill Cosby sweater? Awww. But, be responsible with it. While clothing is a popular way of warming up your pets in cold temperature, it is counter-effective if utilized incorrectly. If a piece of clothing becomes wet, remove it. Wet clothing is as counterproductive for an animal as it is for a human. Also, pet clothing will not make the weather more bearable for elderly or young pets. Pet clothing is for pets of a healthy outdoor age, and is not a suitable substitute for a full coat of fur or insulating body fat. Foresight Aside from the cold, there are certain other seasonal behaviors that a pet owner should look out for. Always know where your pet is and what they are doing. Underneath a car is a popular place for cats to hide in cold months. The lack of frost and the warmth above is most appealing. The first step needs to be a desire to understand how the food you normally eat affects your health. A health crisis is often the motivation to examine the dietary choices you make every day. The media has stories everyday regarding the obesity epidemic in this country. Development of diabetes, heart disease and cancer will shock most people into rethinking what they are eating every day. Obviously the diets of many people in this country are not based on sound nutrition, are not balanced, are not nutrient rich, are chosen to promote health. The most advertised food choices are based on convenience; taste and promoting highly processed factory foods full of fat and sugar. The endless television ads promoting fast food, ready to eat meals, salty snacks, processed foods, high sugar cereals, processed meat products, high carbohydrate foods and sugar based beverages influence millions of people to eat an unhealthy diet. Ads promote foods based on convenience. Beautiful enhanced images of delicious appearing low nutrient high calorie food sell people on a high calorie low nutrient diet. I am often amazed at what I see in people's carts at the supermarket. Boxes of frozen pizzas, processed meats, prepared meals, sugar soda, salty snacks, and sugar cereals fill the cart. There are no fresh vegetables of any kind in the cart. Unless people make decisions to educate themselves on the food value of the items in their cart they are on a collision course to obesity and bad health. Children who are fed this type of food and eat fast food weekly are on the road to becoming obesity statistics. As these children grow they continue to eat the same type of food and carry these dietary habits to adulthood. If there is no motivation to make food choices based on the proper nutritional needs of the body a person will be unlikely to be interested in super foods. One the main benefits of basing your diet on foods from the list of super foods is weight loss. Super foods are defined as foods high in nutrients and vitamins and low in calories. To lose weight you need to eat a balanced nutrient rich, low caloric diet. So choosing foods with the highest nutrient value per calorie is the fastest way to getting to the target weight. Introducing super foods into your diet will allow you to replace the high calorie low nutrient foods gradually, making the process simple and easy to accomplish. Conventional wisdom says it takes 28 days of repeating an action for it to become a habit. To train yourself to adopt new eating habits is going to take some time and determination. If you have been eating the same way for years or decades they are habits not easy to break. Making a gradual switch with realistic planning and setting attainable goals will more likely result in success. A gradual cutting back of sugar is the important first step. Starting a food log and writing down what and how much you eat every meal will help you understand where important changes can be made in your diet. Consulting with your doctor is a good step. This may be the reason why many people make the decision to take control of their diet and food choices. If you have been told you are diabetic or are developing type II diabetes, the doctor has hopefully talked to you about losing weight and watching what you eat. Unfortunately, many doctors rely more on a pharmaceutical approach and less on stressing the importance of changing the diet. Continuing with the same diet that brought you to be diabetic and adding drugs to treat the disease without a fundamental change in your diet is the wrong approach in my opinion. They are many doctors, nutritionists, and former type II diabetics who endorse diet as a means to reversing the disease. A diet based on super foods will promote the development of your immunity cells. The keys are education, commitment and implementation of a plan to eat a diet based on foods to promote your health and eliminate those low nutrient, high sugar, highly processed foods.
