The State Auditor’s 2010 Report of Passport Health Plan was the catalyst for change here at Passport but, as with any business, after 14 years, change is needed and necessary to grow. As the Commonwealth’s only provider-sponsored, community based health plan, we take great pride in where we are today – well positioned for an exciting future. Our member and provider satisfaction rates continue to remain high. We’ve developed new processes to better serve our members and providers, while working to control costs and maintain our commitment to quality and excellence. We also used the audit as a wakeup call to recalibrate our definition of success. 2011 was all about becoming more efficient and innovating as a top-performing health plan. We are very pleased to report early progress towards our goals, including our responsibility to be cost effective for the Commonwealth. The latest look at our projected costs indicate Passport Health Plan delivers quality, member and provider satisfaction, and improved access in a sound and clearly cost effective manner. First, Passport’s governance was completely restructured. A new Executive Leadership Team was appointed and the Board was reorganized to include representatives from rural counties, the Partnership Council, and the community at large. Next, we quickly met and in many cases exceeded all recommendations for improvement, and worked collaboratively to build relationships. In July, we hired local associates from Passport’s third party administrator. We increased our workforce to be able to take on new challenges and to help improve service, and began transitioning all systems and processes under Passport’s authority. We moved to a new location in Southern Jefferson County and introduced a new logo that more accurately reflects our commitment to future innovation and collaboration in health care. William B. This year’s seminar was a huge success, receiving high marks of praise from nearly 100 attendees. Provider partners were introduced to innovative concepts to help their office staff and patients successfully transfer to a patient-centered practice ready to meet the future challenges of healthcare. 10 To succeed in our mission, we are dedicated to finding new, innovative ways to control costs in Kentucky’s Medicaid budget while never compromising the quality of our services. This year, we continued to focus on administrative and medical expenses. The fiscally responsible initiatives started in 2011 included streamlining processes, restructuring our workforce, and further analyzing medical cost management. FOCUSING COSTS ON PATIENT CARE Part of the difference involves how we work closely with physicians. Our emphasis on preventive care allows us to shift taxpayer dollars to helping members seek treatment before their illnesses are advanced and more costly. 0 0 FINANCIAL REPORT For the past couple years, on average, 94 cents of every dollar we received went to patient care. Cultural & Linguistics Services Program (CLSP) Efforts Expanded Our CLS program worked to improve doctorpatient communication by decreasing language barriers and increasing cultural awareness. Sponsoring Important Community Health Initiatives Every year, we fund a number of local pilot projects that are implemented to improve the health and quality of life of Medicaid members and uninsured Kentuckians. It’s one way for us to give back to the community that so loyally supports our efforts. Improved Health Outcomes Program (iHOP) grant recipients include: Kangaroo Care – improvements in breastfeeding rates in Kentucky birthing hospitals. Americana Community Health – worked to determine and fix barriers to health care access for low-income and uninsured immigrants and refugees living in South-Central Louisville. Chronic Care Coordination Project – improved clinical outcomes for Medicaid and underinsured individuals with chronic illnesses such as diabetes. Helping Parents Raise Healthy, Confident Children – formed a coalition in the Bardstown, KY area to fight childhood and adolescent obesity. Passport Health Plan is also proud to support the University of Louisville Department of Pediatrics Forensic Medicine Unit through iHOP. This group of dedicated professionals work in tandem with police and human services providers to investigate reports of abuse and protect every child in the Commonwealth. “Kangaroo Care began with grant support from Passport Health Plan. The breastfeeding rates increased from 45% to 66% in 7 months. This year, we won 1st place in the 2011 Cardiovascular Advertising (CVA) Awards, and 2nd place in the “ReadsEasy” Publication category for the 2010 National Health Literacy Innovator (HLI) Awards! Specialized Outreach Events Our outreach specialists attended 722 events in 2011, an all-time record! We traveled 77,647 miles to reach our members in all 16 counties. NATIONAL LEADERS IN MEDICAID Throughout our evolution, we continue to lead the nation in Medicaid managed care. This year, the National Committee for Quality Assurance (NCQA) ranked Passport Health Plan #13 among Medicaid health insurance plans in “NCQA’s Medicaid Health Insurance Plan Rankings 2011-2012.” This ranking is based on clinical performance, member satisfaction, and NCQA Accreditation scores. It’s not just idle rhetoric to say that our staff feel a true calling to live our mission, to improve the health and quality of life of our members. This year, our Public Affairs Manager Marcelline Coots was presented with the “Making a Difference” award from the Association for Community Affiliated Health Plans. This award recognizes an employee of an ACAP health plan whose work clearly exceeds expectations and whose services exemplify the ACAP principles of Advocacy, Care, Access and Public good. Accountable care is a concept Passport Health Plan is already very familiar with – it involves focusing on member needs and linking provider reimbursement to health outcomes to a defined geographical population. Passport’s existing clinical programs involve a medical home aspect. We pride ourselves on working collaboratively with members and providers to close the gap and improve communication, ultimately improving health outcomes and lowering costs. As we look to the future, Passport Health Plan is poised to lead Kentucky and the nation in accountable care that will truly improve the quality and safety of patient care, and lower health care costs. Here are just a few examples of the long-term success of the medicial home model: Imbedded Case Management Program This 2011 pilot program has successfully improved member compliance by placing an onsite case manager in PCPs’ offices to identify and resolve care gap issues. As a public health professional, I’m always interested in articles like this because I see the sick and uninsured on a daily basis. I am equally interested as a consumer: I want to be assured that Medicaid funding is being used appropriately. I’ve worked in public health for 30 years, so I’ve witnessed the other attempts “firsthand” to manage Medicaid spending and maintain quality care. Passport Health Plan is the only one that has worked. Passport Health Plan members get care from a large pool of credentialed physicians, nurses, and other healthcare professionals, and they have access to a multitude of benefits and special programs. The Plan’s associates participate in events throughout their 16county service area. Finally, they treat the members with dignity and respect. The role that consumes much of my time and is incredibly important to me is grandparent raising my granddaughter Shawna. She was born 19 years ago and I have been raising her since she was four, even though there have been difficult times, I don’t regret it for a moment. Shawna has suffered from several physical and mental disabilities that are challenging, but manageable because of Passport Health Plan making good care possible. The truth is that I could not have afforded to keep her if not for Passport. Their nurses and social workers have been an unbelievable blessing to us, because they always go above and beyond what they are required to do. Because of them and other people who work there, I have been able to get and afford all the treatment Shawna needs to live a good life. We have been able to see the right doctors and get the medicines she needs to be stable. Shawna’s case manager talks to me on the phone and helps me get appointments and explains medicines and treatment plans as well as blood work results. I am also a member of Passport’s Quality Member Access Committee (QMAC). I love participating with this committee because we know our opinions are important and that we’re a voice for the members! Any time we have questions the Passport staff answers them or will find the answer for us. I know that what we say at these meetings will be respected and heard. Shawna is alive today because she was one of the lucky children who had Passport Health Plan and still does. I tell other grandparents raising their grandchildren to check to see if their grandchildren qualify because I know from personal experience that it will make a difference. Sincerely, Anna W.
