Testing, Shipping, Dry Ice and Credit Terms
If you require a pre-shipment sample of a larger donor pool pending your acceptance of the pool,
please be advised that should the sample meet your requirements, you will be required to purchase the
pool, as we are unable to create and maintain these pools indefinitely.
Each plasma sample shipped by CliniSys Associates, Ltd. will come with a sample list, indicating
the single classification value available for that sample (except in the case of normal plasma samples which
have none). I.E. each oral anticoagulant sample that is shipped will arrive with a sample list indicating its
INR value.
You may only specify a single classification on any sample that you order. You may request
additional tests on that sample and we will perform those tests at the rates shown below and report them
on each sample but you may only specify a single classification. We will not accept orders with multiple
classifications.

For instance, if you require 1 mL of oral anticoagulant plasma with an INR of 3.8 and would also like to
know the % activity of Factor V in the sample, the cost per sample would be the listed price of $19.00/mL
plus an additional $25.00 per sample charge, resulting in a total of $43.00 per sample. The INR of 3.8
would be the initial classification while the Factor V level would be reported as additional data with no
classification possible. If you required 15 lupus samples of 1 mL each within the DRVVT ratio range of 1.2
to 1.3 the charge would be $19.00/mL plus $17.00 per sample for a total of $35.00 per sample.
All orders are custom filled with the standard fill being in units of 1 mL and upwards. Orders requiring
fractional mL fills will be charged $7.00 per sample for extended processing.

A Certificate of Analysis (COA) will be provided, free of charge, for each shipment, at the specific request
of the purchaser.
Virological testing is available with any shipment for which it is requested. However, as virological testing
is not a clinical standard for many of the plasma samples we receive, we charge an additional $40.00 per
sample for testing with our test kit. If you require FDA level virology testing the charges are as follows:

For shipments requiring testing with a specific reagent other than the standard reagents used in our
laboratory, there will be a onetime setup charge for the new reagent of $775.00, in addition to the cost
of the specified reagent and its associated controls. There will be a $6.00 per sample charge for running
the samples with the new reagent.
We have arranged with FedEx to be included in their Priority Alert Plus system. This system gives special
tracking and dry ice addition capabilities for any high value biological package moving within the FedEx
system.

CliniSys Associates ships plasma on 30 day net credit terms. We can accept most credit cards (MasterCard,
Visa, Discover and American Express). We charge 3.0% per month on extended term financing. If an
account goes over 30 days past due we suspend all production and shipping for that account and remove
any outstanding purchase orders from the production schedule.
Production and shipping will be reinstated effective the date the account is brought completely current.
