Any changes after this period will require a 1 Important News for 2015 2 Plan Updates and Changes for 2015 These include adding, or removing, or changing benefits, dependents, Qualified Life Event (QLE) such as changes in marital status, birth/adoption of a child, spouse‟s work status or coverage, etc. ALL employees should participate by reviewing current benefit elections and make changes if necessary. In order to participate in Flexible Spending 3 Important Reminders Accounts, you must take action. 4 Open Enrollment Checklist To add or change your benefit elections, you must submit your elections online via Ultipro Employee Self-Service https://n13.ultipro.com no later than November 21, 2014 at 5:00 p.m. Important News for 2015 Effective January 1, 2015, MetLife Expatriate Benefits is the new provider for Medical, Dental, Medical Evacuation and EAP plans. You will receive a new ID card and will also have a local representative. Refer to Schedule of Benefits for detailed plan information. Benefits have been renewed with the current providers for Short Term Disability Insurance, Long Term Disability Insurance, Basic Life Insurance and Basic AD&D Insurance, Voluntary Life Insurance and Voluntary AD&D Insurance coverage effective January 1, 2015. ACTION IS REQUIRED DURING OPEN ENROLLMENT IF: You wish to enroll, cancel, or change coverage elections for medical, dental, vision, voluntary life insurance You wish to participate in the Healthcare Flexible Spending Account or Dependent Care Flexible Spending Account in 2015. Flexible Spending Account elections do not carry over from year to year. Medical, Dental and Voluntary Life Insurance elections will automatically carry over into the 2015 plan year if you do not take action during Open Enrollment. Flexible spending account elections will not automatically carry over into 2015. All changes will be effective as of January 1, 2015 for the January 1, 2015 – December 31, 2015 plan year. To view your current benefits, visit Ultipro and hover over the „Myself‟ icon, under the „Benefits‟ section, select „Current Benefits”. To add or change benefits, visit Ultipro and hover over the „Myself‟ icon, select “Open Enrollment”, select the Open Enrollment link located under “Description”. Please refer to 2015 Premium Rate Sheet. 100% Employee Paid Voluntary Participation HCA Limit: $2,550 DCA Limit: $5,000/$2,500 if married and filing separately NEW HC FSA LIMIT FOR 2015 Health Care FSA limit increases to $2,550 NEW REIMBURSEMENT OPTIONS Check (manual check) default reimbursement option. You submit a claim to HealthSmart and they mail you a check for approved reimbursable expenses. “Benny” Card (debit card) voluntary reimbursement option. A benefit card that acts like a debit card. The card allows you to pay for qualified expenses automatically at the point of sale, directly from the appropriate account. It electronically accesses funds held in the employees Flexible Spending Account, and reduces the need to pay cash up front, submit paper claims and then wait for reimbursement. Retirement (Choose from One of Two Providers) TIAA-CREF Fidelity Investments Only employees with US reportable income are eligible for these plans 403(b): 100% Employee Paid 403(b) Roth: 100% Employee Paid NEW LIMITS FOR 2015 The elective deferral (contribution) limit for employees who participate in 403(b) increases to $18,000 and $6,000 for the catch-up contribution. PAGE 3 2015 OPEN ENROLLMENT NEWSLETTER Important Reminders METLIFE EXPATRIATE BENEFITS MetLife International offers members an online portal, MetLife Expatriate Benefits, with access to global doctor and hospital directories, ID cards, online claims submissions, online explanation of benefits, mail delivery options, and much more! In order to register, visit www.metlifeexpat.com, select „eBenefits‟, enter information found on the front of your Global ID card, then enter your group policy number and 10 digit personal certificate number. Be sure to include all of the zeroes. Finally, enter personal information to verify identify and create a User ID. An automated email from our administration team will be sent that includes a temporary password to login. BENEFICIARIES Note: Beneficiary designations made in Ultipro are not valid. Open Enrollment is a great time to review your beneficiaries and make updates as needed. Beneficiaries are relevant for Basic and Voluntary Life Insurance policies, Business Travelers Insurance, and Retirement accounts. To review your current beneficiaries, contact AURA Human Resources. Basic and Voluntary Life & AD&D Insurance HR advises you to keep your beneficiary designations up-to-date for Basic and Voluntary Life Insurance plan(s). To access the Beneficiary Designation form, visit the Chile page on the Open Enrollment Website. Complete and submit the form to HR. Business Travel Insurance All employees are eligible for the Business Travel Insurance plan. This plan also requires a beneficiary designation. HR advises you to update your beneficiary for this plan. To access The Hartford Business Travel Insurance Beneficiary Designation form, visit the Chile page on the Open Enrollment Website. Complete and submit the form to HR. Retirement Accounts HR advises all 401(a), 403(b), and/or 457(b) plan participants to update your beneficiary for the plan(s). You may work directly with Fidelity (800) 313-0860 or TIAA-CREF (800) 842-2252 to update your beneficiary. Complete and submit the form(s) directly to the vendor. FLEXIBLE SPENDING ACCOUNTS Don‟t forget to submit a claim for your 2014 Healthcare and/or Dependent Care Flexible Spending account no later than February 28, 2015. Eligible dates of services, not date of payment, must be incurred within the elected plan year (January 1, 2014 – December 31, 2014) to be eligible. In the event you do not incur the eligible expenses necessary to exhaust your election or claim the eligible expenses, the remaining balance will be forfeited to your employer‟s plan. To access the 2014 HealthSmart FSA Claim form, visit http://hr.aura-astronomy.org/?q=node/117. Up to $500 in leftover FSA funds can be rolled over into your appropriate FSA account in 2016.
