Note: Diploma in Art Teaching and Diploma in Music Teaching students are not eligible to apply as they are already fully sponsored by Ministry of Education (MOE). B) Period of Award Each award is tenable for one academic year (AY2015/2016), i.e. July 2015 – June 2016, unless otherwise specified. C) Submission of Application Complete and submit this application form, together with the following supporting documents by the closing date: Curriculum Vitae which should list your participation in the arts such as exhibitions, performances, competitions and other activities, as well as any awards related to your field of intended study. Copies of your past examination result slip(s)/ transcript(s). Copies of your educational and professional certificates, including records of co-curricular activities. Copies of all employed family members’ latest payslips, CPF statements, Income Tax assessment or income certification letter by employer. For unemployed or self-employed family members, please fill up the attached “Income Declaration Form”. Copies of divorce / death certificates where a parent is no longer around, and/or court papers (if any). Other information about yourself that may support this application (if any). Confidential reference letters from two referees are required. Referees may include lecturers/ teachers/ employers (excluding family members) who are familiar with your artistic experience and/or achievements. Your referees must sign the reference letter and submit it in a sealed envelope to NAFA either (a) by mail, (b) by hand, or (c) through the Scholarship applicant, before the closing date. Important notes *Per-Capita Income (PCI) is computed based on the total gross income of immediate and non-immediate family members divided by the total number of immediate and non-immediate family members, where, (i) lmmediate family members include spouse, parents and all children who may or may not be living together with the student. (ii) Non-immediate family members include grandparents, siblings, spouses of siblings, siblings of parents, and any other relatives who are living together with the student. (iii) Income = gross (i.e. including employee's CPF contribution) income contributions from self-employment, business or salaried employment (which includes basic salary, allowances overtime pay, etc) as well as other sources of income (e.g rent). Payments in kind, reimbursement for transport and other expenses and National Service (NS) allowance earned by NS men are excluded. Additional requirements on nationality, household income, discipline of study and tuition grant eligibility may be specified by the various donors on the selection of the various Scholarship, Merit Award and Bursary recipients. There is no bond for recipients. However, recipients are required to attend the Scholarships and Awards tea session organised by the Academy.
