In supplying the information below and affixing my signature, I hereby allow the named player to be a member of
the Bullets Soccer Club for the Spring 2015 season. I further authorize the staff of JAA/BSC to act on my behalf
according to their best judgment in case of any incident requiring medical treatment or player supervision.
I hereby waive and release this Club from any and all liability for injuries incurred during or accompanying the
course of these activities. I further certify that this participant is physically fit to participate.

Uniforms: Each player provides his/her own cleats & shin-guards.
The club provides player jerseys.
Shin guards ($15) & black socks ($7) may be purchased
at sign-up or from The Store for your convenience.
U8 - U12 - player provides black shorts & socks
U15 – XARA shorts purchased thru the club ($25)
U18 & Adults – XARA shorts & socks thru the club ($35 for both)
(**The SC80 team will sport new uniforms this season)

FEES: U6 thru U10 - $85 / U12 & up – $100
$20 discount for additional players per family

Games begin on March 14 and end on May 19. The Goal Cup Tournament will be 5-11 thru 15-19
The end of the season party is scheduled for Thurs., May 21. Predominately, the youth will play on
Saturdays and the adults on Sunday afternoons. Both might have a possible weeknight game.
