You can see any doctor, but you pay less when you go to an in-network doctor. To find one: Enrolled in a Kaiser Plan? If you chose a Kaiser medical plan, you must receive all of your care through Kaiser healthcare providers. There are no out-of-network benefits, except in an emergency. Treatment costs vary by location, provider type and more. Even in-network doctors working in different parts of town may charge different amounts. For many procedures, you can use the tools on the Anthem or Cigna websites to find out in advance what the doctor may charge for your visit. Contracts or agreements between doctors or healthcare facilities and insurance carriers may change or terminate at any time. In addition, a network facility can contract with a non-network specialist. Be sure to check with the facility or provider to understand the costs and services, especially if you are planning a surgery or a major treatment. Generally, in-network doctors, specialists and facilities have contracted rates with your insurance carrier and can provide better pricing for the same service or treatment. The plan covers preventive care at 100% when you receive care from an in-network provider. ■■ Know what’s covered. In-network preventive care services, such as physical examinations, related laboratory and X-rays, well-women tests, mammograms, prostate and colorectal (including colonoscopy) screenings, and immunizations, excluding travel vaccines, are covered at 100%, subject to certain age-appropriate and frequency guidelines. KNOW WHERE TO GO FOR IMMEDIATE CARE ■■ In an emergency: Life-threatening emergencies should be handled by your local emergency room or emergency medical services (dial 9-1-1 in most areas). ■■ In a non-emergency (but care is needed right away): You have options: --Visit a network urgent care center. Go to your carrier’s website to search for in-network urgent care centers. --Call your in-network doctor to see if you can schedule an immediate or after-hours appointment. Like salary deferrals into your 401(k) plan account, your HSA and the money in it are yours and can’t be forfeited. Unlike a Healthcare Flexible Spending Account, funds automatically roll over at the end of each year, and you can take your account with you if you change health plans, leave the company or retire. ■■ Your account earns interest. HSA balances are interest-bearing and FDIC insured. Discovery Benefits calculates and compounds interest monthly, and credits it to your HSA each month on the last business day of the statement cycle. ■■ You can invest the money in your account. Once your balance reaches $1,000, you can invest in a selection of mutual funds available through Discovery Benefits. Any interest and investment gains you earn are tax-free, as long as the money is used to pay for eligible healthcare expenses. When you’re prescribed a medication, talk to your doctor about lower-cost options. If you enrolled in one of the Anthem or Cigna medical plans, use your Express Scripts prescription ID card when purchasing your prescription. If you enrolled in the Kaiser plan, use the Kaiser Medical ID card to purchase your prescription. You have two options for filling a prescription: ■■ When you need it immediately: Use an in-network pharmacy to pay lower costs. --Find a participating pharmacy. For Anthem or Cigna plans, go to express-scripts.com. For Kaiser, go to kp.org. --Present your prescription drug card when picking up your prescription. ■■ For medications you take regularly: Sign up for mail order. --This convenient, money-saving service gives you up to a 90-day supply at a reduced cost compared to retail pharmacies. --Once you’ve received your first prescription through mail order, you can refill and renew your prescriptions, as well as receive free standard shipping. DENTAL BENEFITS FINDING A DENTIST Before you go to the dentist, ask yourself: ■■ Does the dentist participate in the network? You can see any dentist, but you pay less when you go to an in-network dentist. To find one, go to deltadentalins.com. The Plan covers preventive care at 100% when you receive care from any Delta Dental contracted dentist. Non-contracted dentists may balance-bill above the Delta Dental plan allowance. ■■ What will the dentist charge for a service? Treatment costs vary by location, provider and more. Even in-network dentists working in different parts of town may charge different amounts. A simple way to determine what insurance will pay is to ask your dentist for a free pre-treatment estimate from Delta Dental. USE YOUR PREVENTIVE DENTAL BENEFITS Preventive care is also important for your teeth and gums. ■■ Use any dentist. The Plan covers preventive care at 100% when you receive care from any Delta Dental contracted dentist. Non-contracted dentists may balance-bill above the Delta Dental plan allowance. ■■ Know what’s covered. Preventive dental care includes twice-annual exams and cleanings, two annual bitewing X-rays, and coverage for sealants until the age of 19. You can see any eye doctor, but you pay less when you go to an in-network provider. To find an in-network eye doctor, go to vsp.com. FLEXIBLE SPENDING ACCOUNTS INCUR ELIGIBLE EXPENSES between JANUARY 1 – DECEMBER 31, 2015 only. When you have eligible healthcare or dependent care expenses, you can: ■■ Pay with your FSA debit card at the point of service.
