The building will be available 1 hour prior to the beginning of judging, and will be closed
30 minutes following the end of each day’s activities.

trials. Entries are limited as specified above.
ENTRIES CLOSE 6:00PM CST, WEDNESDAY, January 21, 2015, at
Secretary’s address, after which time entries cannot be accepted, canceled,
altered or substituted except as provided for in Chapter 11, Section 6 of the
Dog Show Rules. Please waive signature requirements on all forms of
delivery. Mail requiring signatures will be returned to sender.

Please note: Limited crating space may be available in the
main room of the building. Any crates in this area must be
placed on top of crate mats. DMOTC reserves the right to
ask any exhibitor whose dog is creating a disturbance, or
whose crate may potentially damage the building floor to
move his/her crate(s) in the back room.

Attention Exhibitors
The show site will be available to exhibitors one hour before the start of judging each day. The
building will be closed and dogs must be removed from the show site each day within 30 minutes of
the completion of judging.
Please leave the motel and motel grounds clean. Any complaints from proprietors will be handled
by the Show Committee.
Telephone, email and unsigned entries cannot be accepted. Entries not on an Official AKC Entry
Form and photocopies of entry forms without Agreement and Rules on the reverse side of the
Official AKC Entry Form are NOT ACCEPTABLE.
Make all checks and money orders payable to Des Moines Obedience Training Club. Canadian
exhibitors must make fees payable in US funds. Payment must accompany entries.
Mail all entries to Polly S. Mumma, PO Box 8055, Des Moines, IA 50301. Entries received without fees
will not be accepted. Acknowledgement of entries will be made as soon as practical. Unless an
exhibitor specifically requests postal communication, submission of an email address on the entry
form will serve as authorization to send communication, including entry confirmation via email. It is
the exhibitor’s responsibility to ensure that their email software will accept messages from
[email protected]
Entry fees shall not be refunded in the event that a dog is absent, disqualified, excused by
veterinarian or judge, or barred from competition by action of Show Committee or AKC. The wellbeing of dogs, exhibitors and spectators is of paramount importance, and in the event it is necessary
to cancel or stop the show before completion, no refund of entry fee will be made.
DMOTC does not agree to arbitrate claims as set forth on the Official AKC Entry Form for
these events.
Owners are responsible for errors on completed entry forms, whoever may make such errors, and no
entry fee will be refunded in event of such errors or cancellation of entries after the published
closing date. Changes, additions or corrections to entry forms must be received in writing prior to
close of entries. Those received by telephone are not acceptable.
Post-dated and returned checks do not constitute a valid entry. A collection fee will be added to the
amount of each returned check.
No refund for duplicate entries. Care will be taken to find duplicates, however, if an entry has been
processed, no refund will be made.
No entry shall be made and no entry shall be accepted which specifies any conditions as to its
acceptance.
It is expressly understood that exhibitors alone are responsible for the behavior of their dogs and/or
children. Any exhibitor whose dog and/or children create unnecessary disturbances or repeatedly
engage in unsafe or disruptive behavior may, at the discretion of the Trial Committee be asked to
leave the trial site. There will be no refunds of entry fees if a person is asked to leave the trial site.
Dogs may arrive any time prior to their scheduled judging. Dogs not required for further judging may
be excused. Judges will not wait for any dog holding up a class. Owners or agents are responsible for
the presence of their dogs in the judging ring when their classes are called to be judged.
Class prizes will be distributed in the ring to the person showing the dog. Claims must be made in
the ring before the judging for that show is over, and the Secretary advised at once. All prizes must
be claimed at the show. If not, they become the property of DMOTC. No prizes will be mailed.
DMOTC, the show secretary, members of the show committee, their agents, employees, etc. assume
no responsibility for any loss, damage or injury sustained by spectators, exhibitors, handlers, their
employees or children, etc. or to any of their dogs or property. Entrants and spectators to this show
do so entirely at their own risk.
This show is governed by The American Kennel Club. Exhibitors should read the AKC agreement on
the entry blank in the premium list.
$1.00 per exhibitor applied for admission. General admission fee for non-participants is $1.00 per
day.
Bitches in season are not permitted in the Trial. Exhibitors may request a refund of entries,
minus a $10 per entry processing fee, for bitches that come into season after the closing date, as
long as the trial secretary is provided documentation from a veterinarian at least 30 minutes prior
to the beginning of the day’s judging.
Des Moines Obedience Training Club policy allows the Obedience/Rally trial secretary to enter
these events.
Refunds will not be given for overpayments of $5 or less.
CATALOGS
Catalogs may be ordered with your entries for $3.00 per copy. They may be picked up the day of
the trial. A limited number of catalogs will be available to purchase at the trial for $5.00 per
copy.

Parking is not available for RV’s.
MOTELS
The following motels have indicated that dogs would be allowed. Please inform them of your dogs
when you make your reservation. They reserve the right to change their pet policy and rates at
any time. It is your responsibility to verify this information. Some establishments may charge
extra for pets in the room. Please pick up after your dog(s) and do not leave them unattended in
the room so that we will not be refused cooperation by these establishments in the future.

All owners/exhibitors should be aware that DMOTC considers the entire motel and grounds to be
within the trial limits. You will be responsible for all damage to your room and may be subject
to disciplinary action in accordance with the rules of the AKC. Any abuse of the motel/adjoining
grounds will be brought to the attention of the Trial Committee. Any exhibitor’s entries may be
refused for previous misuse of the motel facilities. This is an AKC supported measure.

No entry fees will be refunded if the trials cannot open or be completed by reason of riots, civil
disturbance, fire, an act of God, public emergency, an act of a public enemy, or any other cause
beyond the control of the organizing committee. DMOTC reserves the right to cancel these trials
in the event of extreme weather conditions and no entry fee refunds will be made in such case.

AGREEMENT
I (we) agree that the club holding this event has the right to refuse this entry for cause which the club shall deem sufficient.
In consideration of the acceptance of this entry and of the holding of this event and of the opportunity to have the dog judged
and to win prizes, ribbons, or trophies, I (we) agree to hold the AKC, the event-giving club, their members, directors,
governors, officers, agents, superintendents or event secretary and the owner and/or lessor of the premises and any
provider of services that are necessary to hold this event and any employees or volunteers of the aforementioned parties,
and any AKC approved judge, judging at this event harmless from any claim for loss or injury which may be alleged to have
been caused directly or indirectly to any person or thing by the act of this dog while in or about the event premises or
grounds or near any entrance thereto, and I (we) personally assume all responsibility and liability for any such claim, and I
(we) further agree to hold the aforementioned parties harmless from any claim for loss, injury or damage to this dog.
Additionally, I (we) hereby assume the sole responsibility for and agree to indemnify, defend and save the aforementioned
parties harmless from any and all loss and expense (including legal feeds) by reason of the liability imposed by law upon any
of the aforementioned parties for damage because of bodily injuries, including death at any time resulting there from
sustained by any person or persons, including myself (ourselves), or on account of damage to property, arising out of or in
consequence of my (our) participation in this event, however such, injuries, death or property damage may be caused, and
whether or not the same may have been caused or may be alleged to have been caused by the negligence of the
aforementioned parties or any of their employees, agents, or any other persons. I (WE) AGREE THAT ANY CAUSE OF
ACTION, CONTROVERSY OR CLAIM ARISING OUT OF OR RELATED TO THE ENTRY, EXHIBITION OR
ATTENDANCE AT THE EVENT BETWEEN THE AKC AND THE EVENT-GIVING CLUB (UNLESS OTHERWISE STATED
IN ITS PREMIUM LIST) AND MYSELF (OURSELVES) OR AS TO THE CONSTUCTION, INTERPRETATION AND
EFFECT OF THIS AGREEMENT SHALL BE SETTLED BY ARBITRATION PURSUANT TO THE APPLICABLE RULES
OF THE AMERICAN ARBITRATION ASSOCIATION. HOWEVER, PRIOR TO ARBITRATION ALL APPLICABLE AKC
BYLAWS, RULES, REGULATIONS AND PROCEDURES MUST FIRST BE FOLLOWED AS SET FORTH IN THE AKC
CHARTER AND BYLAWS, RULES, REGULATIONS, PUBLISHED POLICIES AND GUIDELINES.
INSTRUCTIONS
1. (Variety) if you are entering a dog of breed in which there are varieties for show purposes, please designate the particular
variety you are entering, i.e. Cocker Spaniel (solid color black, ASCOB, parti-color), Beagles (not exceeding 13 in., over 13
in. but not exceeding 15 in.), Dachshunds (longhaired, smooth, wirehaired), Collies (rough, smooth), Bull Terriers (colored,
white), Manchester Terriers (standard, toy), Chihuahuas (smooth coat, long coat), English Toy Spaniels (King Charles and
Ruby, Blenheim and Prince Charles), Poodles (toy, miniature, standard).
2. The following categories of dogs may be entered and shown in Best of Breed competition: Dogs that are Champions of
Record and dogs which, according to their owners records, have completed the requirements for a championship, but whose
championships are unconfirmed. The showing of unconfirmed Champions in Best of Breed competition is limited to a period
of 90 days from the date of the show where the dog completed the requirements for a championship.
3. (Event Class) Consult the classification in this premium list. If the event class in which you are entering your dog is
divided, then, in addition to designating the class, specify the particular division of the class in which you are entering your
dog, i.e. age division, color division, weight division.
4. A dog must be entered in the name of the person who actually owned it at the time entries for an event closed. If a
registered dog has been acquired by a new owner it must be entered in the name of its new owner in any event for which
entries closed after the date of acquirement, regardless of whether the new owner has received the registration certificate
indicating that the dog is recorded in his name. State on entry form whether transfer application has been mailed to the
AKC. (For complete rules, refer to Chapter 11, Section 3.)

AGREEMENT
I (we) agree that the club holding this event has the right to refuse this entry for cause which the club shall deem sufficient.
In consideration of the acceptance of this entry and of the holding of this event and of the opportunity to have the dog judged
and to win prizes, ribbons, or trophies, I (we) agree to hold the AKC, the event-giving club, their members, directors,
governors, officers, agents, superintendents or event secretary and the owner and/or lessor of the premises and any
provider of services that are necessary to hold this event and any employees or volunteers of the aforementioned parties,
and any AKC approved judge, judging at this event harmless from any claim for loss or injury which may be alleged to have
been caused directly or indirectly to any person or thing by the act of this dog while in or about the event premises or
grounds or near any entrance thereto, and I (we) personally assume all responsibility and liability for any such claim, and I
(we) further agree to hold the aforementioned parties harmless from any claim for loss, injury or damage to this dog.
Additionally, I (we) hereby assume the sole responsibility for and agree to indemnify, defend and save the aforementioned
parties harmless from any and all loss and expense (including legal feeds) by reason of the liability imposed by law upon any
of the aforementioned parties for damage because of bodily injuries, including death at any time resulting there from
sustained by any person or persons, including myself (ourselves), or on account of damage to property, arising out of or in
consequence of my (our) participation in this event, however such, injuries, death or property damage may be caused, and
whether or not the same may have been caused or may be alleged to have been caused by the negligence of the
aforementioned parties or any of their employees, agents, or any other persons. I (WE) AGREE THAT ANY CAUSE OF
ACTION, CONTROVERSY OR CLAIM ARISING OUT OF OR RELATED TO THE ENTRY, EXHIBITION OR
ATTENDANCE AT THE EVENT BETWEEN THE AKC AND THE EVENT-GIVING CLUB (UNLESS OTHERWISE STATED
IN ITS PREMIUM LIST) AND MYSELF (OURSELVES) OR AS TO THE CONSTUCTION, INTERPRETATION AND
EFFECT OF THIS AGREEMENT SHALL BE SETTLED BY ARBITRATION PURSUANT TO THE APPLICABLE RULES
OF THE AMERICAN ARBITRATION ASSOCIATION. HOWEVER, PRIOR TO ARBITRATION ALL APPLICABLE AKC
BYLAWS, RULES, REGULATIONS AND PROCEDURES MUST FIRST BE FOLLOWED AS SET FORTH IN THE AKC
CHARTER AND BYLAWS, RULES, REGULATIONS, PUBLISHED POLICIES AND GUIDELINES.
INSTRUCTIONS
1. (Variety) if you are entering a dog of breed in which there are varieties for show purposes, please designate the particular
variety you are entering, i.e. Cocker Spaniel (solid color black, ASCOB, parti-color), Beagles (not exceeding 13 in., over 13
in. but not exceeding 15 in.), Dachshunds (longhaired, smooth, wirehaired), Collies (rough, smooth), Bull Terriers (colored,
white), Manchester Terriers (standard, toy), Chihuahuas (smooth coat, long coat), English Toy Spaniels (King Charles and
Ruby, Blenheim and Prince Charles), Poodles (toy, miniature, standard).
2. The following categories of dogs may be entered and shown in Best of Breed competition: Dogs that are Champions of
Record and dogs which, according to their owners records, have completed the requirements for a championship, but whose
championships are unconfirmed. The showing of unconfirmed Champions in Best of Breed competition is limited to a period
of 90 days from the date of the show where the dog completed the requirements for a championship.
3. (Event Class) Consult the classification in this premium list. If the event class in which you are entering your dog is
divided, then, in addition to designating the class, specify the particular division of the class in which you are entering your
dog, i.e. age division, color division, weight division.
4. A dog must be entered in the name of the person who actually owned it at the time entries for an event closed. If a
registered dog has been acquired by a new owner it must be entered in the name of its new owner in any event for which
entries closed after the date of acquirement, regardless of whether the new owner has received the registration certificate
indicating that the dog is recorded in his name. State on entry form whether transfer application has been mailed to the
AKC. (For complete rules, refer to Chapter 11, Section 3.)
