PCR testing should typically be reserved as a diagnostic tool for sick animals to confirm or rule out the presence of infectious agents. PCR can detect the actual presence of the pathogen before antibody response occurs, making it extremely valuable for early detection. What is PCR? Polymerase chain reaction (PCR) is a molecular technique that exponentially amplifies a specific region of DNA. The large number of copies of DNA produced permits detection of infectious agents in specimens. What is an IDEXX RealPCR™ test? How is it different from conventional PCR? IDEXX RealPCR tests are run by real-time PCR. Real-time PCR is faster and more sensitive and utilises a closed-tube system that virtually eliminates contamination and false positives – a problem with conventional PCR. Real-time PCR also allows for testing for multiple pathogens on a single specimen and enables not only detection of pathogens but also quantification of the amount of genetic material in a specimen. What additional steps are taken to prevent contamination and ensure accurate results? IDEXX’s molecular diagnostic laboratories are dedicated units separate from other testing areas. They offer maximum security through separation, air pressure differences and a QA/QC management system to control the PCR process, specimen integrity and facility safety. What does a positive PCR result mean? A positive PCR result indicates that the nucleic acid (DNA or RNA) of that organism was detected in the specimen submitted, indicating infection with the organism. A positive result should always be interpreted in light of the clinical signs of illness to determine clinical significance and whether treatment is indicated. What does a negative PCR result mean? A negative PCR result indicates that the nucleic acid (DNA or RNA) of that organism was not detected in the specimen submitted, which suggests that the animal’s disease is not caused by the organism. However, a negative PCR result may be caused by the numbers of organisms being below the limit of detection of the PCR assay, decreased numbers of organisms following treatment, a chronic carrier state, or the occurrence of a new strain not detected by the PCR assay. Does PCR differentiate between vaccination and infection? IDEXX RealPCR™ For inactivated vaccines, PCR does differentiate between vaccination and infection. In general, PCR does not pick up the DNA component of inactivated vaccines administered systemically. For modified live vaccines, PCR may detect viral DNA/RNA for a few weeks after vaccination while the vaccine virus is replicating. Quantification of viral DNA/RNA in the specimen by real-time PCR may be used for some pathogens (e.g., distemper virus) to distinguish between vaccination and wild-type infection. What are IDEXX Quant RealPCR™ tests? IDEXX Quant RealPCR tests utilize the quantitative results that real-time PCR provides to give additional information to help interpret positive PCR results for certain pathogens. For example, the Canine Distemper Virus (CDV) Quant RealPCR™ Test can be used to determine if a positive PCR result is likely a result of recent vaccination or from wild-type infection. The Clostridium perfringens enterotoxin (CPE and CPEA) Gene Quant RealPCR™ tests can be used to help decide whether the enterotoxins are likely contributing to diarrhoea. Additional Quant RealPCR tests are under development. Should other diagnostic tests be performed in conjunction with PCR testing? As with other diagnostic testing, PCR should not be used in isolation. The clinical signs of the patient and additional diagnostic testing should be considered when appropriate. Real-Time PCR The real-time detection mode is made possible by using fluorescent tags built into the real-time PCR tests. Based on the real-time detection, quantitative information is readily available for downstream use. This feature has been used to develop quantitative PCR tests, which provide additional clinical information. Two Step Process PCR is a process composed of two steps: DNA or RNA extraction followed by the real-time PCR reaction. Techniques used during the nucleic acid extraction allows us to achieve high consistency and yields, minimises errors and speeds up the process. IDEXX RealPCR™ Specimen Collection Guidelines PCR testing offers a versatile testing modality for infectious diseases. However, accurate results depend on collecting the appropriate specimen type for the clinical manifestation. Provided below are detailed protocols for each sample type. All specimens should be stored and shipped refrigerated, unless otherwise indicated. Specimens should be collected prior to antibiotic, antifungal or antiviral treatment directed against the organism of interest, or collected at least 2 weeks after withdrawal of these medications. Specimens received within 72 hours of collection are preferred, but most specimens are stable for up to 10 days if stored refrigerated, unless otherwise indicated. Vaccination with an attenuated or modified live vaccine may result in positive PCR test results for up to a few weeks postvaccination. Every RealPCR™ specimen submission is processed with up to 7 quality controls. If a specimen fails the preanalytical quality control (DNA and RNA), a repeat extraction is attempted If quality control criteria are still not met for a particular diagnostic specimen, the submitting veterinarian will be contacted and encouraged to submit a fresh specimen at no additional charge. Whole Blood: 2 mL (0.1 mL minimum) EDTA whole blood (LTT). Faeces: 5 g (1 g minimum) fresh faeces in a sterile container (preferred) or empty, clean container. Urine: 2 mL (0.1 mL minimum) urine in a sterile container. Fluid (CSF, pleural effusion, respiratory wash specimens, uveal fluid, ascites, abscess aspirates, etc.): 0.5mL (0.1mL minimum) fluid in a sterile tube (LTT [preferred], RTT or plastic tube). Swabs: Submit dry, plastic-stemmed swabs, without transport media, in an RTT or in a empty, sterile tube. Respiratory specimens may include a deep pharyngeal swab (with visible organic material on swab; please rub firmly), and a conjunctival swab (wipe eye clean; swab inside of eyelid), which can be submitted together in the same tube, or a nasal swab (equine respiratory panels or systemic mycoses). When acquiring a pharyngeal swab, precautions should be taken to prevent patient from biting and/or ingesting part of swab. Collect specimens prior to fluorescein staining. Tissue Aspirates: Submitting tissue aspirates on a swab (see below) improves stability of the specimen. 1–3 air-dried, unstained slides (keep refrigerated) are also acceptable specimens but may result in lower sensitivity. Suggested Protocol for Tissue Aspirates: 1. Aspirate tissue into the hub of the needle using standard techniques. 2. Remove the syringe from the needle and fill syringe with air. 3. Reattach the syringe to the needle and squirt out aspirate onto the end of a dry, plastic-stemmed swab. 4. Submit the swab in an RTT or an empty, sterile tube; keep refrigerated. Tissue Biopsies: PCR testing can be performed on previously submitted histopathology specimens (formalin-fixed, paraffin-embedded tissue). Fresh, refrigerated tissue specimens submitted within 24 hours of collection are also acceptable. Freezing is not recommended unless the specimen can be maintained frozen until arrival at the PCR laboratory. PLEASE NOTE – Wooden stemmed Swabs are not recommended for PCR tests submitted to the laboratory. Plastic Stemmed swabs are preferred. Improper choice of a collection vial can adversely affect results. Did You Know? Using a serum separator tube (SST) is an easy way to collect a good serum sample for chemistry panels. However, for certain specialised tests, such as many endocrinology and drug tests, the gel in SSTs can interfere with results. Did You Know? Whole blood EDTA samples start to degrade as soon as the blood is outside of the animal. To preserve cell morphology, include air-dried, unstained blood slides along with the purple top tube. Slides should also be included with samples submitted for fluid analysis for accurate cytologic interpretation. Blood smears made at the time of blood collection help avoid platelet clumping problems and allows more accurate platelet interpretation. Did You Know? Including slides with your CBCs can aid in pathologist reviews of unusual cells, and with the identification of red blood cell parasites. Samples clotted during blood draw may result in… • Platelet clumps • Falsely decreased cell counts (platelets, red blood cells and white blood cells) • Haemolysis (when forcing blood into tube) Excess anticoagulant (under-filled tube) may result in… • Decreased RBC count and HCT due to dilution • Altered cell morphology • Inaccurate MCV, MCH, MCHC and HGB • Falsely prolonged clotting times EDTA contamination may cause… • Falsely decreased calcium • Falsely increased potassium • Interference with many specialised tests Use proper techniques Filling syringes: When filling syringes, aim for a good free flowing stick from the largest accessible vein. Slow draws and difficult sticks can rupture red cells, adversely affecting CBC results and certain chemistries. The longer the blood stays in the syringe, the greater the risk of clumped platelets and clots that degrade test results. Specimen Collection Guidelines Filling vacutainer tubes: If the blood clots in the syringe, do not force it into the vacutainer tubes. Forcing clotted blood into vacutainers will result in lysed red blood cells and cause inaccurate results. Always fill the tubes without additives first. This prevents carry-over of tube additives. For example, if you are filling an EDTA tube and a serum separator tube, always fill the SST first. Even a small amount of EDTA can interfere with many chemistry results. Fill purple top (EDTA) or blue top (citrate) tubes precisely. Overfilling and under-filling tubes causes the wrong ratio of additives. Excess EDTA in an under-filled tube will give inaccurate CBC results. Overfilling the EDTA tube may cause the sample to clot. To ensure the correct volume of blood, allow the vacuum of the tube to pull the blood out of the syringe without additional force. Centrifuging: Make sure samples are fully clotted before tubes are centrifuged. Note that some samples may take longer to clot than others. Did You Know? In tests that require serum, it is important to collect and centrifuge the sample properly. If a tube is spun too soon after drawing the blood, you will send plasma to the laboratory and not serum. This will ensure a clear understanding of your needs, and will help you to select the correct samples for histopathological examination, as well as identify any need for samples for other testing (e.g. bacteriology, virology, serology, toxicology). Alternatively you may consult page 32 of this DOPS. Turnaround Time Most evaluations will be completed within 24-72 hours of receipt in our laboratory (unless otherwise indicated). Additional fixation or decalcification will take longer. We will notify you if an unusually long delay is anticipated. Collection Technique Samples are collected for histological examination by standard surgical techniques or at postmortem examination. Labelling Criteria Please ensure all specimen jars are labelled with Patients name, date Type of specimen, (Site / Source) Requisition Information A thorough clinical history and details of the physical examination are essential for the correct histological interpretation of tissue changes. Information required includes signalment (species, breed, age, sex), a description of the appearance and distribution of lesions, duration of the condition, biopsy sites or post mortem tissue, response to prior treatments, current treatment regimens and any other relevant information. You may include any questions to be answered on your requisition form. Please send radiographs of bone lesions when they are being submitted for histological examination (see histopathology – bone). Fixation Guidelines Tissue samples should be fixed in 10% buffered formalin. Did You Know? The 10:10:10 Rule? For optimum fixation and sectioning use 10% formalin; 10:1 ratio; and a biopsy size 10mm cubed Place specimens in a wide necked container (approved for use with formalin), with the ratio of formalin to tissue > 10:1. Submit entire lesions and tumours with adjacent excise tissue. For rapid fixation of larger lesions and tumours, cut a section 0.5-1cm wide through the centre of the specimen. Make impression smears from the cut surface of tumours and submit for cytology in a separate bag. Open hollow organs, such as intestine, prior to placing them in fixative. Small fragile specimens (bone marrow, Tru-cut liver or kidney) can be wrapped in a gauze envelope so that they do not disintegrate during transport. High priority samples can be dispatched on the day of collection as they will fix on their way to the laboratory. Samples of lower priority can be fixed for 24 hours at clinic prior to dispatch to the laboratory. 88 Histopathology Microbiology Transport Guidelines All samples should be placed in a well sealed leak proof bag containing enough absorbent material for the volume of formalin. Fixed tissue which is to be mailed may be placed in a leak proof plastic bag or container with a formalin soaked gauze to keep the tissue moist. (ensure adequate fixation has occurred prior to transportation) Histopathology Fee Policies Fees are determined by number of sites, lesions or organs indicated on the requisition form. For ‘Single Tissue’ biopsies prices are quoted per anatomical site. If the number of sites/lesions is not indicated, we will assume each specimen is from a different site, and will charge separately. Surgical margin analysis does not attract an additional fee. Cancellation Fee No fee is applied if cancellation is requested prior to processing. If we have started processing the sample, a fee of $31 Ex GST will be charged to cover costs incurred. Necropsy Samples IDEXX no longer offers in-laboratory necropsy service, but there are a variety of options for submitting necropsy samples. Normal Flora, Predictable Susceptibility Patterns and Non-pathogenic Organisms IDEXX follows guidelines set by NATA, combined with our years of experience in performing susceptibility testing. We believe these are “best practice” microbiology techniques, and would be happy to discuss the following policies with you. Sterile Tubes Use glass or plastic tubes with no additives. Plastic yellow-topped tubes with clot activator are not acceptable for cultures because clot activator binds bacteria, which inhibits growth. Fluids, urine and tissue can be submitted in sterile containers (moisten tissue with sterile saline or water to prevent drying and loss of viability). Fluids Additional Notes Margins are complimentary if requested. Cage Birds include budgerigars, pigeons, finches and small birds from zoological gardens or fauna parks. Large birds from zoological gardens or fauna parks such as waterfowl, poultry, ostriches or emus are not considered cage birds. Make sure all collection devices containing fluids are sealed and leak proof before submitting. Note: Specimens that are >48 hours old are not suitable for culture, and loss of viability should be expected. Blood Culture Aerobic and anaerobic cultures are performed on all blood cultures. Biochemistry Only Submit sample in lithium heparin tubes (green top). Gel separator tubes can be used. Gel tubes can be spun before submission, and this will reduce storage artefacts. Plain serum tubes can also be used, but result in a smaller sample for analysis, and may reduce the number of measurements that can be performed. Small sample volume may limit the number of biochemistry tests that can be performed. If submitting a small sample, please note your required tests in order of preference on the submission form. Haematology Only Submit slide(s) and whole blood. Make slides immediately after collection, using blood that has not been exposed to anticoagulant. Preferably make two slides, one by the usual slide-and-slide method, one by a coverslip-and-slide method. Air dry slides immediately, but do not fix or stain. Submit the whole blood in lithium heparin. Gel separator tubes can be used but must NOT be spun down. EDTA can also be used for most birds (excluding ratites, crows and ravens), but is not the preferred sample. EDTA should not be used for reptile blood, use of lithium heparin is recommended. Haematology and Biochemistry Required Submit blood films (see haematology above) and blood in lithium heparin. Do NOT spin gel separator lithium heparin tubes if only a single tube is submitted. At least one unspun lithium heparin tube must be submitted. Small sample sizes may limit the number of biochemistry tests that can be performed. Retention of Samples After we report analytical results, we routinely retain blood specimens in our storage facilities for seven (7) days, after which the samples may be destroyed. Histopathology blocks are retained for at least 12 months. Hazardous Materials We may at our option either dispose of or return to you unused portions of samples found or suspected to be hazardous or to contain hazardous materials according to state or federal guidelines upon completion of the analytical work. We may invoice you for the reasonable cost of disposal or returning the sample. Specimen Containers Specimen packaging is the responsibility of the sender and must conform to IATA regulations Section 650, or other regulations as amended from time to time. IDEXX may provide specimen containers and IATA packing materials upon request. IDEXX reserves the right to charge a fee for specimen containers and packing materials. Courier Services IDEXX may provide access to courier services, both internal services and commercial services. Subject to availability, regional commercial courier companies can provide overnight delivery of specimens to IDEXX's laboratories from anywhere in Australia. This standard service cost is included in the test fee. IDEXX assumes the risk of loss or damage to a sample at the time possession of the sample is delivered to an IDEXX employee. Retention of Reports Unless otherwise agreed in writing, IDEXX shall retain copies of analytical reports for a period of one (1) year, after which the reports may be destroyed. IDEXX stores all analytical reports in electronic format. A hard copy is available upon request. IDEXX reserves the right to charge for additional copies of reports once the original report has been submitted. Limited Warranty and Limits of Liability In accepting analytical work, IDEXX warrants the accuracy of test results for the sample as submitted. We accept no legal responsibility for the purposes for which you use the test results. The Complete Diagnostic Solution™ 93 CBC’s Interpretive options and Reticulocyte counts, what you need to know Interpreted profiles IDEXX offers a broad range of interpreted and un-interpreted profiles to support your daily diagnostic needs. All interpreted profiles contain a comprehensive CBC including a scientist blood film review and interpretation of the haematology findings by a pathologist. The blood film examination ensures detection of subtle inflammatory disease (i.e. neutrophil left shifting and toxic change), red cell morphology changes (e.g. schistocytes, acanthocytes), platelet abnormalities (e.g. macroplatelets), and rare atypical cells (e.g. immature lymphocytes, mast cells) that are clinically relevant in ill patients. The pathologist interpretation is valuable for interpretation of the significance of these findings in relation to the clinical presentation and for suggestions as to the next diagnostic steps. Un-Interpreted Profiles IDEXX un-interpreted profiles include profiles designed both for health screening and preventative testing as well as cost effective options targeting specific disease presentations, where the referring veterinarian prefers to interpret the pathology. Un-interpreted profiles include a standard CBC which may be upgraded to a comprehensive CBC by the addition of a BFI (blood film interpretation). This would be recommended in all patients who are clinically unwell. The total annual health profile (TAHP/TAHPC) is our recommended profile for health screening and preventative testing in clinically well patients. These patients often have minimal abnormalities expected in their haematology and biochemical data. In this scenario, the standard CBC offers good screening information, and over time the combined CBC and biochemical results from each patient build a database, or “baseline” that can be used for early detection of clinically significant abnormalities that may not yet fall outside population reference intervals. Example of the benefits of baseline monitoring Two examples of this would be a slowly dropping haematocrit as a sign of chronic / inflammatory disease, or a mild but progressive elevation in creatinine, an indicator of decreasing renal function. The database in health can also be used for comparison when the patient becomes unwell to help identify clinically significant abnormalities. For example recognition that a stable, marginally elevated ALP in an older dog that then becomes unwell is unlikely to be clinically significant as the cause for the presenting illness. 94 Dr Karen Jackson Specialist Clinical Pathologist Dr Graham Swinney Specialist Canine Medicine If unexpected changes are identified in the standard CBC or biochemistry of the TAHP/TAHPC, the profile can be upgraded by adding a BFI code to receive a scientist blood film review with morphological comments, and interpretation of the haematology findings by a pathologist. If interpretation of the biochemistry results is also required then the HPI code can be added for full interpretation. Here for a regular health check? IDEXX has many uninterpreted profiles which are often targeted to patients with specific disease entities (e.g. pancreatitis, cardiac disease) and for patients that are clinically unwell. In this situation the blood film may hold clues as to the nature of the patient’s illness, and thus we recommend adding the BFI code at the time of submission ensuring scientist blood film review and pathologist interpretation of the haematology findings. A reticulocyte count is included in all IDEXX haematological profiles and only IDEXX in house haematology analysers provide an absolute reticulocyte count in clinic. Traditionally reticulocytes were assumed to be important in the assessment of anaemic animals to help determine whether the anaemia was regenerative or nonregenerative. Clearly this is an important determination. However, reticulocytes are also very useful in the assessment of many physiological and pathological conditions which may not manifest as clinical anaemia. Animals may not be actually anaemic when compared to population reference intervals and yet an elevated reticulocyte count may alert the clinician to look for underlying haemolytic (e.g. immune-mediated, oxidative, infectious, neoplastic, or toxic) or haemorrhagic (e.g. GI ulceration, neoplastic, coagulopathic, parasitic) processes. In these diseases the patient is losing red blood cell mass but the bone marrow response is sufficient to keep parameters such as red blood cell count, haemoglobin and haematocrit within the population reference intervals (i.e. compensated RBC loss/destruction). The clue that there is a problem is the absolute reticulocyte count. Furthermore, reticulocytes can alert the clinician to the presence of other less common conditions such as an absolute erythrocytosis due to chronic hypoxia, EPO-producing neoplasms, and polycythaemia vera. Reticulocytes are a crucial part of any haematological profile.
