9:00 A.M. – 3:00 P.M.
**Inclement Weather call: 410-372-8169 (Rain Date: May 9, 2015)
1. Set-up time: 7:00 a.m. – 9:00 a.m.
2. Fee: $20.00 per 10’ x 10’ space. Fee is due with registration. **NO REFUNDS**
3. All vendors must check-in upon arrival. (NO VEHICLES LARGER THAN MINI-VAN ALLOWED
ALSO NO TRAILERS)
4. All exhibitors will bring their own tables, stands, chairs or other equipment needed for displaying their
merchandise.
5. All exhibits must be removed by 3:00 p.m. (With all trash and unsold items taken with you)
6. The exhibitor is solely responsible for collection and payment of any Federal, State or Local taxes,
including sales, amusement, admissions or receipt taxes, which may be collectible or payable by
reason of its activities under this agreement.
7. The Parkville Recreation and Parks Council reserves the right to refuse entry on the premises or sale
thereon of any merchandise that is deemed, in the discretion of the Council, unacceptable.
8. Vendors are not permitted to sell food or drink.
9. If we sell out of spaces, you will be contacted by phone and your check returned. YOU WILL NOT
RECEIVE A CONFIRMATION CALL.

The exhibitor hereby agrees to indemnify and name harmless the PARKVILLE RECREATION AND PARKS COUNCIL and its
members for any and all claims, causes of action and suits arising, occurring or resulting from any damage, injury or loss of
property of any nature caused by, arising out of, or in any way connected with the exercise by the Exhibitor or Parkville Recreation
and Parks Council or its members, of the privileges herein granted. The Parkville Recreation and Parks Council will not be
responsible for any loss or damage resulting from inclement weather.
I agree to abide by all conditions set forth in this agreement with the Parkville Recreation and Parks Council.

ACKNOWLEDGEMENT, WAIVER AND RELEASE OF LIABILITY:
I HEREBY CONFIRM PARTICIPANT IS IN GOOD HEALTH AND ABLE TO PARTICIPATE IN THE
ACTIVITY. I ACKNOWLEDGE THE ACTIVITY MAY INVOLVE RISKS AND DANGER OF BODILY INJURY OR
DEATH. I FULLY ACCEPT AND ACKNOWLEDGE THE ACTIVITY MAY INVOLVE RISK AND I HEREBY ASSUME
THE RISK AND RESPONSIBILITY FOR ALL DANGERS AND RISKS ASSOCIATED WITH PARTICIPATION IN THE
ACTIVITY. I acknowledge Baltimore County, Maryland, the Recreation Council, and their respective
employees, directors, officers, volunteers, members and any other participant, entity, party or person involved
in any regard with the Activity or the Activity premises, and their respective agents, personal representatives,
heirs, employees, contractors, successors and assigns (each an “Activity Representative” and collectively the
‘Activity Representatives”), SHALL NOT BE RESPONSIBLE OR LIABLE IN ANY REGARD OR MANNER FOR ANY
AND ALL PROPERTY DAMAGE OR BODILY INJURY (INCLUDING SERIOUS PHYSICAL INJURY OR EVEN DEATH)
INCURRED BY PARTICIPANT OR ANY PARTY RELATED THERETO AS A RESULT OF HIS/HER PARTICIPATION
IN THE ACTIVITY.
I have read, fully understand, and hereby freely sign, approve of, and agree to the terms of this
Registration Form. I HEREBY UNCONDITIONALLY RELEASE, DISCHARGE, CONVENANT NOT TO SUE, WAIVE
MY RIGHTS AND REMEDIES, AND AGREE TO HOLD HARMLESS THE ACTIVITY REPRESENTATIVE from any
and all claims, costs, demands, losses, damages, or expenses associated with, in whole or in part, participant’s
involvement with the Activity. I certify all answers and information provided on this Registration Form are to
the best of my knowledge true and correct thought the Activity. I shall inform the Recreation Council in
writing if any of the information provided in the Registration Form is incorrect or changes during the course of
the Activity. I understand Baltimore County and/or the Recreation Council do not perform criminal and/or
background checks on Activity Representatives. I shall present a government-issued photo identification card
including, but not limited to, my driver’s license, passport, or United States Visa to the Activity Representative
for review, if requested, at the time I submit this Registration Form to the Recreation Council.
