BPL/EWS Proof (if applied) – BPL/AAY Ration card counter signed by the District Revenue Officer/ Tahesildar. 5. इकलौिी कन्या /Single girl child (if applied, affidavit required). 6. Medical Certificate – for Physically Challenged Candidate (issued by competent authority). 7. Service Certificate with residence proof – for serving personnel. 8.
