works under NHM as detailed below ON TURN KEY BASIS and ON TWO BIDS SYSTEM.

Eligibility Criteria:

Registered APWD (Bldg) Class I-(A) (B) & (C) / CPWD/ Reputed Construction Company/ Agency
registered under Govt. /Public Sector undertaking.
Up to date Registration Certificate, VAT, PAN, work completion certificate with supporting Work
order, Labour Licence etc. must be produced at the time of purchasing bid document.

Contractor must have satisfactory completed civil works of value not less than 100% of the
estimated cost in the last 5 years
Contractor must have achieved annual turnover of equivalent to work value in any ONE YEAR
during last 5 years.
In case the evaluated Bid Amounts are equal, preference for allotment of the work will be given
to the bidder having the highest Bid capacity
The contractor who was earlier allotted any NRHM works by PWD (B), Assam or by NRHM and
could not complete the work in all respect in time/within the period up to which extension of time
granted without penalty will not be ELIGIBLE to participate in the bidding process
The contractor to whom extension of time was granted from NRHM imposing penalty will also
be not ELIGIBLE to participate in the bidding process
The intended bidders are asked to verify the site for the work prior to quoting their rates.
Cost of tender document should be drawn in favor of State Health Society, Assam, payable at
Guwahati in the form of Pay Order/Demand Draft from nationalized bank. Bid security with a
validity of 225 (i.e. 180+45) days should be in the form of FDR/CDR and drawn in favour of the
State Health Society, Assam, payable at Guwahati.
Date for issuing of Bid Document:-06 /02/2015 to 07/02/2015 during working hours
Date and time of submission of Bid: 13/02/2015 upto 14:00 hrs at the office of the Mission
Director, NHM, Assam
The bids will be opened at 15.30 hrs of the same days as mentioned above.
In case, if any Bandh /Holiday falls, the sequence will be held in the next working day
