Contest Rules:
1. You must work during the 10-week contest period—2/9/15 through 4/19/15.
2. You must be actively working for BAYADA Home Health Care as a field employee at the time of the grand prize drawing
to qualify.
3. You may complete and submit tickets for every week you demonstrate The BAYADA Way through our core value of
reliability. You may also submit tickets on behalf of a coworker who demonstrates reliability.
4. Office staff, clients, and other BAYADA caregivers will be encouraged to submit tickets on your behalf.
5. Winners are subject to taxes on prizes as applicable by law.

How the contest works:
One winner per week will be randomly chosen in every service office. These preliminary winners will select a prize from the
Way to go! prize flyer, available online at www.bayada.com, or within your service office. Managers will order each prize
selected, and ensure proper delivery to the winners.
On Thursday, April 30, 2015, companywide grand prize and runner-up winners will be
randomly selected from BAYADA’s preliminary winners. They will be presented with a
certificate, redeemable for their own personal reward through GiftCertificates.com.
