Following the inflammatory stimulus, CRP concentration increases early, five hours after the onset of acutephase reaction, with a magnitude as much as 1000-fold. Conversely, the ESR rate of change, as well as its dynamic range, is much lower than that of CRP. CLINICAL SETTINGS ASSOCIATED WITH MAJOR ELEVATION OF SERUM CRP CONCENTRATION Infection: • Acute bacterial infections • Systemic fungal infections • Plasmodium (P. falciparum) infection • Systemic Pneumocystis or Toxoplasma infection • Systemic CMV or Herpes virus infection. Inflammatory disease: • Rheumatic fever • Rheumatoid arthritis • Juvenile chronic arthritis • Mediterranean fever • Reiter’s disease • Crohn’s disease • Polymyalgia rheumatica • Systemic vasculitis • Psoriatic arthritis • Ankylosing spondylitis. Malignant neoplasia: • Multiple myeloma • Non-Hodgkin lymphoma • Hodgkin’s disease Necrosis: • Waldenström’s disease • Carcinoma, sarcoma. • Myocardial infarction, acute pancreatitis, tumor embolization. Trauma: • Surgery • Burns • Fractures. Allograft rejection: • Renal transplantation.
