Confirmation of Enrolment Form 1.2. Important Reminders You will have to login first using your applicant number and surname. 2.Check your confirmation of enrolment schedule and bring all the necessary documents found in your Confirmation of Enrolment Form. 3. Bring all the additional documents found in the College Instructions, when applicable. 4. Three (3) days after submitting all the required documents, login at http://pds.ust.edu.ph to accomplish the Personal Data Sheet (PDS) online and print the Acknowledgment Slip. There is no need for you to submit the Personal Data Sheet accomplished online. 5. The Acknowledgment Slip is a requirement for enrolment. With your Acknowledgment Slip, proceed to enrolment.
