Dear Diley Parents,
Evening Parent-Teacher Conferences will be held this month. I would encourage parents to attend the
conferences for the purposes of understanding how your child is performing and how you can support him or
her at home. As always, we will continue to focus on the importance of learning at Diley Middle School.
With a strong partnership with families and community, we will strive for lifelong learning for our students
so they are prepared to meet the challenges of our ever-changing world.
As you are aware, our students will be assessed in February in the areas of ELA/Literacy, Math, 6th grade
Social Studies and 5th grade Science. Students will complete Performance-based assessments (PBA) in
ELA/Literacy and mathematics. In ELA/Literacy, students will be analyzing literature and a narrative
writing task. In math, students will be asked to solve problems involving the key knowledge and skills for
their grade level (as identified by the CCSS), express mathematical reasoning, construct mathematical
arguments, and apply concepts that solve real-world problems.
Just a reminder the state assessments have two components, Performance-based Assessment (PBA) and End
of Year Assessment (EOY). The student’s scores on the two are combined to create a final score for each
subject. Students must provide an answer and explain how and why they chose it on the performance-based
test. The end-of-year test requires students to select responses. The following links will provide you with
practice tests questions along with additional information regarding state assessments:

I would like to thank our district leaders, Board of Education members and teachers who have dedicated time
and finances to ensure students are prepared to take Ohio’s Next Generation Assessments.
Sincerely,
Mrs. Hedgepeth

IMMUNIZATION REQUIREMENTS
FOR STUDENTS ENTERING 7TH GRADE IN 2015-2016
Dear Parents of Current 6th Grade Students:
Per the Ohio Department of Health, all 7th graders must show documentation of receiving a Tdap
immunization before being allowed to enter school next year.
Previously, Pertussis (the “P” in the immunization) had not been required after the age of 7. Due to an
increase in outbreaks of Pertussis (also known as Whooping Cough) the Department of Health has decided
that a booster is needed.
You may take this letter with you to your doctor to use as documentation of this vaccination, or you may
submit an immunization report with the dates of this Tdap vaccination. Please return this documentation as
soon as possible to your child’s school nurse. If this is not provided during the 6th grade school year, you
will need to send this information to the Junior High that your child will attend, prior to the beginning of
next school year. The middle school nurses will forward on all immunization documentation received to the
junior high nurses at the end of this year.
