Currently we have 2 forms: RT3181 Form – Line Blockage Certificate RT9909 Form – Record of Working Arrangements & Briefing Form Why need to Change? NEW Proscient e-permitting system with NEW Permit NEW SWL role Need to remove duplication Full RT9909 Form is not always required, only sections of it What’s changing? Colour perforated A5 bound Incident Response pad What’s changing?: Improvements Transition from the use of RT9909 into the use of 2 new forms: Incident Response Permit (v1.0 Jan 2015) Lookout / Site Warden Certificate (v1.0 Jan 2015) Basic content from the RT9909 form remains, with duplication being removed RT3181 content remains unchanged, only altered the look for easy identification What’s changing?: Improvements Use of colour to easily identify the forms Link Incident Response Permit to Lifesaving Rules Build in NEW SWL role and NEW permit requirements Make clearer which forms to use and when to use The new forms will remove duplication and simplify content NEW Process – Part 1 This new process is an extract from Section 3.2 of NR/L2/OHS/133 ‘Code of Practice for Planning and Delivering Safe Work (PDSW)’ This will be found on Safety Central from March 2015 The NEW Process is described in both words and diagrams on the next 4 slides Incident response shall be undertaken by a Performing Authority (PA) that is either Individual Working Alone (IWA) competent or, when more than one individual, be led by a competent SWL The Controller shall: select relevant incident response template (IRT) & create the incident response permit (IRP) enter the IRP at the correct location on the map within Proscient notify the PA of any temporary change notices (TCNs), lessons learned or additional hazards which are visible in Proscient and issue the permit number to the PA NEW Process – Part 2 The PA shall capture any TCNs, lessons learned or additional hazards on the incident response permit pad The PA is responsible for: completing the task and operational risk assessment, and capturing this, along with protection from trains (safe system of work), on the incident response permit pad taking the permit ‘live’, managing the response activity and closing the permit As part of Step #3, the Signaller should be contacted to arrange any relevant protection and assist with the completion of a Line Blockage Certificate if needed As part of Step #12, the Signaller should be contacted to cancel any protection arrangements if they were applied NEW Process – Part 3 Please note that steps 5, 6 & 7 do not exist, they are not missing. The next step is #8. NEW Process – Part 4 • Live from previous page • From Live the SWL has several options: • Suspend • Amend • Handover • Job Complete • Job Incomplete S • The SWL in charge of the Job can Suspend to Amend the permit via the “MOWP” • The SWL in charge of the Job shall make sure the work has stopped before making amendments to a permit. • Once the permit has been Amended, the permit will need to be Re-Authorised by the AA A • The SWL in charge of the Job can Job Complete the permit via the “MOWP” • The SWL in charge of the Job shall make sure the work area is left in a safe clean and tidy condition • The SWL in charge of the Job must make sure the Work Group have left the work area • The SWL in charge of the Job can Job Incomplete the permit via the “MOWP” • The SWL in charge of the Job shall make sure the work area is left in a safe clean and tidy condition • The SWL in charge of the Job must make sure the Work Group have left the work area • The SWL must enter the reason why the Job has been left as Job Incomplete • The SWL in charge of the Job can Suspend the permit via the “MOWP” • The SWL in charge of the Job shall make sure the work area is left in a safe clean and tidy condition • The SWL in charge of the Job must make sure the Work Group have left the work area NOTE: If the SWL is unable to use the Mobile Optimised Web Page “MOWP” the SWL shall request either the Planner or RA to carry out the required action within Proscient. I • The SWL in charge of the Job can Handover the permit via the “MOWP” • The SWL’s performing the Handover must have a face to face conversation prior to handing the Job over. • The incoming SWL will Accept the Handover of the permit via the “MOWP” • Both must also confirm the Handover by signing the Permit. H • • • • The Planner records any Lessons from the paper copy of the permit into Proscient. The AA must validate any lessons learnt. Example #1: Line Blockage Certificate (RT3181) This form is the RSSB RT3181 Certificate which came into force 06 Dec 2014 Example #1: Lookout / Site Warden Certificate Pg1 Use the Sighting Distance Charts at the front of the Incident Response Pad to assist Example #1: Lookout / Site Warden Certificate Pg2 Example #2: Incident Response Permit Pg1 Use the Lifesaving Rules & Considerations on the inside cover of the Incident Response Pad to assist Example #2: Incident Response Permit Pg2 Example #2: Lookout / Site Warden Certificate Pg1 Use the Sighting Distance Charts at the front of the Incident Response Pad to assist Example #2: Lookout / Site Warden Certificate Pg2 Pre PDSW ‘go-live’ message Please be advised that the NEW Incident Response Permit will take effect Nationally from the date the first Route goes live with the introduction of the NEW PDSW Protocol (NR/L2/OHS/133) East Midlands is the first scheduled Route to Go-Live, as advised on Safety Central Other Routes, still in a pre-PDSW Go-Live status, shall commence using this new Incident Response Pad which contains the NEW Incident Response Permit at the same time In doing this, some fields will be ‘not applicable’ until such time as your Route goes live The next 2 slides indicate in red which fields on the Incident Response Permit this relates to Pre PDSW ‘go-live’ message – IRP Pg1 Permit Number not required pre Go-Live Pre PDSW ‘go-live’ message – IRP Pg2 The Work Group declaration is still required, however, it will not apply to the permit Pre PDSW Go-Live for your Route. In this scenario, the SWL will be the COSS before PDSW Go-Live has occurred for your Route. Times of permit changes are not required Pre PDSW Go-Live has occurring for your Route. Lessons learnt are still required, however there is no requirement to add these into Proscient until PDSW Go-Live has occurred. Pre PDSW ‘go-live’ non-compliance pending change RT9909 is currently used to record briefing arrangements including when associated with Incident Response This new standard (NR/L2/OHS/133) contains the new Incident Response procedure and use of the Incident Response Permit and Lookout/Site Warden Certificate alongside (where appropriate) the existing RT3181 Line Blockage Certificate A non-compliance pending change will be in place for each Route in order to introduce an Incident Response Pad to replace the use of the RT9909 form for Incident Response until the Route goes live with the PDSW Protocols and NR/L2/OHS/133 becomes live These can be found in the new Incident Response Pad: Incident Response Permit RT3181 Line Blockage Certificate Lookout / Site Warden Certificate Next Steps We have distributed a number of the Incident Response Pads out to each Route / Depot so that you have a base stock If further stocks are required, either immediately or in the future, these will need to be self ordered and purchased either via i-store or RCOS (Rail Components Ordering System) using Network Rail catalogue numbers, which can be found on Safety Central If not already done so, you will need to register for a log-on in order to use the RCOS system Alternatively: these 3 forms will be downloadable directly from the NEW Proscient system these 3 forms will be saved on Safety Central under the Product Specifications section the Line Blockage Certificate (RT3181) can be obtained from RSSB in the usual way Please direct any queries to [email protected]
