SARASOTA CUP TEAM CHECK-IN Check‐In is mandatory at one of the two check‐in nights. Due to the sheer number of games starting Saturday morning, there are NO team check‐ins Saturday morning unless arranged through the Tournament Director. Thursday, April 24th, 6:00 – 8:00 pm Twin Lakes Park – FC Sarasota Club House 6700 Clark Road, FL 34241 Friday, August 25th, 5:00 – 8:00 pm Twin Lakes Park – FC Sarasota Club House 6700 Clark Road, FL 34241 *Teams playing Friday night games may check in 1 hour before their game at their location. **All FC Sarasota teams must register with Mary Mixon prior to April 24 What to Bring to Check-In • 1 Completed Visitor Questionnaire & Emergency Contact Form (printed, completed & brought to team check‐in). • 1 Official Roster ‐ either FYSA or US Club Soccer (bring 5 adjusted copies, see below) • This is not the event roster or a print out of the roster page in your team acct. This is the Official roster provided to you by your club registrar. • On a copy of the official roster, write in at the bottom your club pass players & guest players. • On the same copy, cross off any of your primary players not attending the tournament • Make sure jersey #s are listed for each player • Now make 4 copies of this roster that has added guest players and crossed off players • • • • Guest Player Form ‐ 1 Guest Player or Loan Form for each guest player that is from OUTSIDE your club. You will need the form completed and signed by the guest player’s Club Registrar or Agent of Record. (Note: “Guest” players from within your own club are called “Club Pass” players and you do not need Guest Player forms for Club Pass players). Guest players must have the same type of player passes as the rest of the team (all USYSA passes or all US Club Soccer passes). Teams may have no more than 5 guest players! Form on located on www.sarasotacup.com Laminated Coach & Player Passes ‐ either USYS State Association or US Club Soccer passes. All coaches & players, including Guest Players & Club Pass players, must have valid, laminated player passes with photo. No pass, no play, no exceptions. Player Medical Release Forms – 1 for each and every player (they do not need to be notarized ‐ just signed & dated). Your club medical release form is acceptable. Permission to travel forms – only for teams traveling outside of Region III. After checking in your team you will receive back: • 4 stamped, approved rosters • Player passes (keep with you at all games) • Copy of any guest player forms • Medical release forms (keep with you at all games) Team Pre‐Game and Post Game Procedure • Give tournament stamped approved roster to the referee • Referee should not collect player passes (But they should be kept with you at all times) • Referee will check players for safe and proper equipment • The team that wins or ties is responsible for the score card and signatures from coaches and referees. • If you fail to turn in the game report card with 20 minutes of the game, then it can be counted as a forfeit. Last key point ‐ Players may only play for 1 team in the tournament. Please take a moment to re‐read the Tournament Rules in preparation for the weekend.
