Their recommendations of dosage are necessary to accurately maintain your INR (International Normalized Ratio). Blood thinner’s are difficult to regulate because of several circumstance; activity, food, illness and more. Writing down the time and dose everyday keeps you aware that you have taken your medication correctly. In addition you have a log to look back and see your dose, INR, and weeks between each pro-time appointment’s. I was born 6 weeks premature and immediately came down with pneumonia. My temperature rose to 104 degrees and I was close to death. During those first hours my body heat began to deform my Aortic Heard Valve. Penicillin was given to me and saved my life. Fifty years later I started to have symptoms of Atrial Fibrillation causing erratic heart beats. Several test were taken and I had to have my Valve replaced. Open heart surgery is not easy. Recovery was a beast! I couldn’t remember when I took a pill, what my heart rate was, my temperature and all the rest of my statistics. My sister created a book of daily charts for me to write all that I had done during the day. At the beginning my mother and sister took charge of all the entries until I was at the point of entering things for myself . Let me tell you, weeks down the road the diary was the central part of my recovery. I would take it to the doctors office and show him what had transpired from one appointment to the next. He finally asked me where I got the diary. I told him. He said “everyone should have one of these”! So… here it is. I revamped several items from the original diary to make it easy for anyone to use the forms. I have given copies of the diary to several of my clients to use for surgical services and they loved it. Nurses would enter all the information into the computer and the patient would not have to struggle to remember. They’re doctors, too, asked where they got the books and again stated “everyone should have one of these when recovering from surgery”.
