2015 Stanly Health Services TOTAL HEALTH INCENTIVE One YOUR TOTAL HEALTH MATTERS Stanly Health Services is committed to your Total Health! The CHS Stanly Total Health Incentive offers you the opportunity to earn up to a $600 reward for taking action to maintain or improve your health. The CHS Stanly Total Health approach is aimed at providing you with the tools and resources that not only support your physical health, but to also support your personal health. WHAT TO DO Total Health Survey REWARD $100 HOW TO DO IT Follow these steps to access the NEW Personal Total Health Survey that will help you learn about your Total Health. Lab results are not needed to take the survey. •Go to www.stanlywellness.org •Click on the “Total Health Survey” tab •Follow the instructions to complete the survey Plan to take the survey when you have plenty of time. If your time is interrupted, you can simply click on the pause button and resume the survey at a later time. Know Your Numbers $100 Knowing your numbers is an important step in taking action to maintain or improve your health. Labs should be obtained during 2015 to receive your incentive reward. There are two options to earn the reward: OPTION 1: Go to your primary care physician to get your yearly labs completed. OPTION 2: Go to Occupational Health to get your employee health labs completed during your anniversary of hire month. A reminder letter will be sent to you during your anniversary month. Complete the “Know Your Numbers” form and mail it with a copy of your lab results to receive your incentive reward. Forms can be picked up in Occupational Health at Stanly Regional Medical Center (SRMC) or online at www.stanlywellness.org. Total Health Classes $100 Five classes will be offered throughout the year focused on improving and maintaining your Total Health. Classes will be held in the private dining rooms of the SRMC cafeteria or available online by video anytime at www.stanlywellness.org. Class offerings will be announced to teammates by email. You must complete all 5 classes to get the incentive. No partial credit will be awarded. Complete the activities above to receive the amount indicated for your reward. The reward will be paid out in December 2015. The incentive will be added as cash to your paycheck for the Choice 20 plan and cash in your HSA for the Choice 30 plan. You must be enrolled in either the Choice 20 or Choice 30 plan to earn the Stanly Total Health Incentive. Printed on Recycled Paper EIGHT HEALTHY W REWARD Get your start weight early. Earn $150 or $300 to be paid in 2015. If you are in your healthy weight range, only one weigh-in is needed for the year. ES CHALLENG CHS D BY SPONSORE LL LiveWE New in 2015 Carolinas Healthcare System will hold quarterly prize drawings, followed by a year-end prize drawing, open to all teammates regardless of benefit status. Prizes will be awarded for participation in CHS LiveWELL Challenges and Events! Each time you submit your results from participation in a challenge or participate in one of the many CHS LiveWELL events, your name will be entered into the drawing. Multiple entries are encouraged, so get ready to participate!
