Page 5 Toronto | Canada March 24 – March 29, 2015 General Information Meals Most meals are included, with the exception of lunch on Thursday, lunch and dinner on Friday (if not included in your selected activity), lunch and dinner on Saturday (if not included in your selected activity) and breakfast on Sunday, as noted in the itinerary that follows. See page 11 for the information about the $750 CDN stipend to cover meals and sundry expenses. All dietary considerations indicated on the registration forms have been noted. Incidental Expenses The hotel will maintain an incidental account to cover your personal expenses such as telephone calls, room service, mini bar charges, etc. Upon check-in, you will be required to provide your credit card to establish your personal incidental account. The hotel will hold the amount of $200.00 CDN on your card. You will be responsible for settling this account before you leave the hotel. Early spring can be quite cool in Toronto with average highs of 3°C (37°F) and average lows of -3°C (27°F). Throughout March, the most common forms of precipitation are light snow, moderate snow, and light rain. The dress code for most activities is casual. We encourage you to advise your credit card company of your travel plans to avoid any inconveniences when making credit card purchases during the event. We are, whenever possible, eliminating paper and plastic, reducing waste, reusing and recycling, conserving energy and water and choosing sustainable products. We always strive to make choices that are good for us and good for the environment. Your guest room is equipped with a telephone that allows you to make direct-dial or 1-800 calls. You will be responsible for any charges (i.e. connection fees, access charges and airtime charges) associated with longdistance or non-local calls. Your guest room also offers complimentary Wi-Fi during your stay. Gratuities All gratuities for planned Vision in Action celebration events have been prepaid. This includes tips for porters, waiters (included meals only) and bellmen. Gratuities for personal activities such as hotel bars, room service, or mini-bar charges are not included and should be considered at your discretion. Hospitality Desk Throughout your stay, there will be a TD Vision in Action Hospitality Desk staffed by representatives from The Farm, the event management company which supports the Vision in Action program. On Wednesday and Thursday, this desk will be located on the 2nd floor foyer of the hotel. On Friday, Saturday and Sunday, this desk will relocate to the 4th floor foyer of the hotel. U.S. citizens must carry a passport valid for travel to Canada, which must not expire before the date of return to the U.S. However, as entry requirements are subject to change without notice, and in order to avoid delays and misunderstandings, we strongly recommend that passports be valid for six months after the final date of stay. For further U.S. passport information, visit www.travel.state.gov. Non-U.S. citizens who are Permanent Residents or Landed Immigrants in the U.S., in addition to a valid passport, may require additional documentation for entry into Canada. For further information, visit www.cbp.gov/travel. For international citizens, visas and passports with six months validity may be required. Based on your completed registration, McTavish Travel will advise entry requirements. International citizens are responsible for obtaining all necessary travel documents. For further information, visit www.cic.gc.ca. Please ensure that you carry your travel documents with you at all times while travelling. We recommend your travel documents be placed in your pocket or carry-on luggage and not in your checked luggage. Canada: Emergency Travel Medical coverage provides reimbursement for eligible emergency medical expenses not covered under your provincial health care plan when you are travelling, up to 60 days, on TD business1. You must be covered by a provincial health care plan or equivalent to be eligible for Emergency Travel Medical. If you have lost your Manulife benefits card, be sure to print off your Manulife Emergency Travel Assistance card from the Benefits & Pension website (under Forms and Resources) before you leave. This card provides you with the numbers to call in the event of a medical emergency: In Canada and U.S. call toll-free: 1 (800) 265-9977 Elsewhere call collect: 1 (519) 741-8450* A spouse that meets the definition of eligible dependents for the TD Employee Benefits Plan (Canada) is covered2 if they are travelling with the employee. For further information on eligible dependents, please review the Benefits and Pension website. Guests of Vision in Action recipients (who are not considered eligible dependents for the TD Employee Benefits Plan (Canada)) are responsible for their own Emergency Travel Medical Insurance. Employees enrolled in the TDI (GI) Employee Benefits Plan, must have medical coverage, which includes emergency travel medical coverage for personal or business trips up to 60 days. If you have lost your Manulife benefits card, contact Manulife before you leave to obtain a replacement. 1 A spouse must be enrolled in the medical coverage of the employee enrolled in the TDI (GI) Employee Benefits Plan to be eligible for emergency travel medical coverage. 2 Emergency Travel Medical Information Since the Vision in Action event is a business event, your emergency travel medical coverage will provide you with medical and accident coverage while travelling. Please refer below for information pertaining to your country of residence. Please also note information relating to coverage for your guest. * Some countries may not accept collect calls. You may be required to pay up front and submit for reimbursement. Page 7 Toronto | Canada March 24 – March 29, 2015 U.S.: Employees travelling outside of the U.S. are covered for emergency room visits, medically necessary follow up visits and emergency prescriptions by TD’s medical plan. The emergency services must be authorized by National Medical Excellence (NME) at (215) 775-6445 and will be paid at the network benefit level. You must submit any claims directly to Aetna for reimbursement. The member may obtain the proper claim forms by either logging into the Aetna Navigator to print them or by calling member services and having it mailed, faxed, or emailed to them. A spouse that meets the definition of eligible dependents for TD America’s Most Convenient Bank medical plan is covered if they are travelling with the employee. Guests of Vision in Action recipients (who are not considered eligible dependents for TD America’s Most Convenient Bank medical plan) are responsible for their own Emergency Travel Medical Insurance. UK: TD provides you with travel cover for emergency medical expenses while on business trips away from your regular place of employment. You are automatically enrolled under this provision upon your date of hire. Spouses or guests of Vision in Action recipients are responsible for their own Emergency Travel Medical Insurance. For the UK, Emergency Travel Assistance is provided by Chubb Assistance. An assistance card can be obtained from Human Resources. This card provides you with the hotline number to call in the event a medical emergency occurs: +44 (0) 20 7895 3364. Policy # 64788060. Travel from Home to / from the Airport or Train Station Transportation to and from the airport or train station has not been pre-arranged as part of the event. Expenses for travel to and from the airport or train station, including parking charges, are to be approved by your people manager and expensed to your cost centre or RC. When travelling on Bank business, travellers must make every effort to minimize the cost to the Bank and to our clients, and commit to environmentally efficient and responsible travel practices. Should you decide to drive to Toronto, TD will cover the cost of your mileage. Please refer to page 11 for information on how to report and cover the mileage expense. Parking costs at the hotel during the group dates will be covered by the program. Airline or Train Ticket When you receive your ticket via email, please review your flight or train itinerary carefully and report any discrepancies to Allyson Pelisek or Jina Aslanyan. The name on your electronic airline or train tickets must exactly match that of your personal identification (e.g., passport). Otherwise, boarding may be denied. Luggage Identification Please be sure to complete the TD Vision in Action luggage tags and attach them to every piece of luggage (carry-on included). Please remove all out-dated tags from your luggage. These tags expedite the identification and safe handling of your luggage at the hotel and at the airport and train station. We also suggest you include your address somewhere inside your luggage, just in case tags go astray. You are requested to check in at your designated airport a minimum of two hours prior to your flight departure to allow time for airline check-in, Immigration and Customs (for U.S. citizens), formalities and security clearance. Check-in will close 60 minutes prior to the scheduled departure. Passengers who arrive late may be denied boarding. All flight times are subject to change without prior notice. We recommend that you contact the airline before leaving for the airport to confirm your departure time. If you are having someone meet you at the airport on your return, they should also call the airline to verify arrival time. Baggage Allowance and Fees Baggage allowances and fees vary by airline and are subject to change at any time. To ensure you have the right information, remember to check with your airline to confirm this information before you depart for the airport. A portion of the $750 CDN stipend you will receive (see page 11) is to cover the cost of the airline’s charge for one piece of baggage per person for you and your guest. You will need to pre-pay your baggage fee (if applicable), which can be done during online check-in for your flight 24 hours prior to departure or at the airport. Please visit your airline’s website to confirm the amount of any applicable baggage fees. Should your luggage exceed maximum weight you will be required to pay the excess cost at your own expense. All carry-on items are subject to screening. Ensure that all electronic devices are operational. For security reasons, you may be asked to turn on these devices. The airline reserves the right to prohibit any item that they consider to be a security risk. For security and easy access, we advise you to carry your personal identification, valuables, airline tickets and any necessary medications in your carry-on bag, not in your checked luggage. Liquids on Board Transportation Security Administration will permit up to 100 ml (4 oz.) of non-prescription liquid medications through the security checkpoint and onboard airplanes. Low blood sugar treatments, including glucose gel for diabetics, is permitted. Travelers can bring 90 ml (3 oz.) containers of toiletries. Items must be in a one litre (quart) clear, plastic, zip-top bag. All lighters are prohibited in carry-on luggage. Lost Luggage It is unfortunate, but true – from time to time, luggage does go missing. We suggest you carry essentials in your carry-on baggage (necessary toiletries and a change of clothing and prescriptions, within the “Liquids on Board” guidelines above). Advise someone from The Farm travel staff if your luggage does not arrive, so appropriate forms can be completed before departing the airport. They will follow up with the airline to locate your luggage as quickly as possible. You are requested to check in at your designated train station a minimum of 45 minutes prior to your train departure to allow time for check-in. Passengers who arrive late may be denied boarding. All train times are subject to change without prior notice. We recommend that you contact VIA Rail before leaving for the station to confirm your departure time. If you are having someone meet you at the train station on your return, they should also call VIA Rail to verify arrival time. Page 10 Toronto | Canada March 24 – March 29, 2015 Taxable Benefit and Expense Information Cash Stipend/Taxable Benefits TD will provide you with a $750 CDN stipend, which will be reduced by the appropriate taxable benefit amount when the add-to-pay is processed. The after-tax amount can be used to pay for sundry expenses such as baggage fees and meals during the event. All taxable benefits for the Vision in Action program will be paid by TD on your behalf. Expenses Per the above, baggage allowance and sundry expenses are to be covered using the $750 CDN stipend TD is providing. If you are travelling by car, TD will cover the cost for mileage. For Canadian recipients, mileage expenses are to be submitted via the Travel & Expense system to cost centre 4252 no later than April 30, 2015. If your job level is 10 or below, please select Karey Stanley-Boyd as the approver. If your job level is 11, please select Jennifer Mena-Yiukay as the approver. Pre-arranged transfers have been organized from the airport to The Ritz-Carlton on group dates only. Throughout the day, Vision in Action recipients and guests will arrive at the hotel and will be greeted at the TD Vision in Action Hospitality Desk. At this time, you will receive your room key, name badge and other pertinent event information. If you are arriving by car, please have the hotel valet park your vehicle. Valet costs will be covered on group dates only. Depending on your arrival, you’re free to spend the remainder of the day at your leisure. Check-in time at the hotel is 3:00pm.
